Dear Mr. Shaver:
Please find enclosed a PCN application from the North Carolina Department of Transportation (NCDOT)
for a Nationwide 14 permit for the secondary road improvement project on SR 1801 Captain Westbrook
Road in Sampson County.
The scope of the project will be to widen (in some locations), grade, drain, add base, and pave the 1.34
mile of existing unpaved road to current NCDOT secondary road standards to improve the roadway safety
and stability. These standards include 20 feet of travel lanes and 5 feet of grassed shoulders on both
sides within the 50 feet of right of way. Also enclosed are wetland data forms and maps for the issuance
of a jurisdictional determination based on our field review on April 26, 2011. The jurisdictional forms have
been supplied to you electronically.
By copy of this letter and enclosures, NCDOT is also requesting issuance of 401 Water Quality
Certification (WQC) #3820 from the Division of Water Quality (DWQ). A method of debiting $240 to
process the WQC permit request has been provided as noted in the subject line of this application.

Zone. The AE Zone does not cross the subject road as only portions of the AE Zone encroach into the northern side of
the ROW. The only proposed road construction that may affect this area is the changing of a 24° pipe to a 30° pipe and
this will not affect the flood plain in any way that would be considered greater than deminimis. As previously noted, the
road bed in this area will not be widened and therefore fill will not be pushed out into the flood plain area except for the

Project Description: Project will grade, base and pave SR 1801 [Captain Westbrook Road] within the
existing 50 foot Right of Way (ROW) in Sampson County. To reduce wetland impacts no new ditches
will be constructed and all work will be within the existing roadbed. All work will be within the existing
NCDOT Right of Way [ROW] and there will be no ditch work
SUMMARY OF CULTURAL RESOURCES REVIEW
Brief description of review activities, results of review, and conclusions:
Review of HPO quad maps was undertaken on April 21, 2011. Based on this review, there are no
existing NR properties in the Area of Potential Effects. No properties appear to be present in the project
APE.
Brief Explanation of why the available information provides- a reliable basis for reasonably predicting
that there are no unidentified historic properties in the APE:
The propose project will not extend beyond the existing ROW. The absence of any recorded cultural
resources in the project vicinity precludes any impact to new location or cultural resources. No further
work is warranted.

designations roster, and inclexes and HPOWeb reviewed on 10 May 2011 and yielded one NR, and no SL,
LD, DE, or SS properties in the Area of Potential Effects (APE)- The APE is centered on existing SR t8o1,
and extends 100 feet from the centerline to the N and S and 100 feet from either end of the proposed
7tW-foot project length to the E and W to encompass proposed construction activities. As the project is
state funded, GS 1.71-12(a) applies; the three pipe replacements are likely Federal permit sites also
requiring Section 106 review. The Sampson County architectural survey records only the National
Register-listed property, 't'hirteen Oaks (SP 923), in the APE (no APE properties appear in the related
Survey publication (Tors 0urr--i2a, An h1v-.?,)rory Cl hi5: or.? Are?n:a:;re - Sa,^;^son Coirny, NCrop Cdru`na, Ciinton: City of
Cln,ton, 1981)). Sampson County current GIS mapping and aerial photography, as well as Google Maps
"Street Vie-.%" (viewed 10 May 2011) identified cultivated fields, woodland, and an additional property
(PIN 19106739001) containing pre-1960 resources in the APE. NCDOT Division 3 confirmed that a barn
and several trees belonging to this property stand outside the right-of-way and will not be disturbed by
the proposed construction (see at ached correspondence). One of the pipe replacements is located at the
caestern edge of the property and will not impact any critical resources. The project as currently defined
is contained v:ithin the existing right-of-way, and will not adversely impact any buildings or landscapes in
the APE, Including the National Register property. A finding of "no historic properties affected" will satisfy
GS 121-12{a) and Section 106 compliance requirements.
Should the design, funding, or permit requirements of the project change,
please notify NCDOT Historic Architecture as additional review may be
necessary. No vehicles, other equipment, materials, or any construction
activities should be staged or stored on the National Register property
outside of existing ROWS (map with NR boundary attached). Failure to do so
will forfeit compliance with GS 121-12(a) and Section 106 for historic
architecture and adversely compromise the project.
