Dear Mr. Alsmeyer,
In an effort to obtain the necessary permits for the subject bridge repair, we are providing a pre-
construction notification application with supporting documentation for the repair of the bridge over the
Yadkin River in Forsyth County. This is a pennit for a defined river with distinct riverbanks. One of the
piers has shifted and needs to be repaired. Lt order to do this work, a temporary work platform will need to
be constructed to reach the pier and also to slow the swill current during the repair. The platform will be
130 feet long by 40 feet at the base, for a total of 5200 square feet. The pier repair will involve (lie
placement of 260 square feet of large riptap around two picas to prevent. scour. No cndanf crcd species
were found on 5/2/05. We believe this project can be accomplished under Nationwide Permits 3 and 33.

Attach a vicinity imp clearly showing the location of the property with respect to local
landmarks such as towns, rivers, and roads. Also provide a detailed site plan showing property
boundaries and development plans in relation to surrounding properties. Both the vicinity map

Permits, published in the Federal Register on March 9, 2000, mitigation will be required when
