HDR calculated the acreage, and revised the sheets. They are in the attached email. If its possible for you to proceed
with the modification for only DEF, that would be greatly appreciated. The contractor is hoping to start next week. Is it
possible for them to proceed while we are sorting out the permitting, or can you get it to us early next week? I know
that is a big ask, but anything you could do would be greatly appreciated!

Amy: Unfortunately, I cannot authorize this without a mod, particularly since it is for a different purpose than the other
temp impacts. However, I can modify it fairly easily, and maybe quickly, if you will provide me with the square footage
of temporary impact from the 65’ of impacts.
Dave: let us know if this changes your response.
There are options for how to deal with the WQC. If Dave can confirm that no formal WQC change is needed (in-field
mod?), then I can go ahead with a mod for site 20.
I would prefer to combine this mod and the U-2579AB mod (since they are under the same IP and DA#), if the U-2579AB
mod WQC will be done soon. This would especially work well if Dave does need a formal WQC mod for Site 20, and the
U-2789AB mod; and a combined mod, or 2 separate mods, would be done soon.
I don’t want to make it more complicated than it has to be. If you have any questions, please give me a call.

Below is an email from Vickie Miller with HDR. They are the Design team on the U-2579DEF project. The contractor
needs to install a temporary crossing to bring a crane to the site. After your review, let me know if you do or do not
have any objections to us proceeding. We no longer need to do the previously permitted work pad (site 20C-165’), but
instead need 60’ of temporary impacts at the same site but at a different location on the stream. The pdf labelled “RW-
21_L-STA+50 Rev.3 markup” is shows the location of the proposed site, and the pdf labelled “Site 20 Permit Sheet
U2579DEF” shows the site as it was permitted.

Hi Amy,
I have attached the permit plan sheet, the summary impact table, and a marked up sheet of the location that Flatiron
would like to put a temporary crossing at Mill Creek. You will notice they are not showing the need for a temporary
workpad in the channel but instead a 60’ long pipe in Mill Creek for a crossing to move cranes, etc. The original
permitted length of impact for the temporary work pad (Site 20C) was 165 ft. This would be less impact from that
standpoint. The proposed location for the temporary pipe is in an area where we will have impacts from a ditch tie in
(Site 20A). Site 20 A currently is permitted for 16 lf of permanent impact and 21 lf of temporary impact. We would need
to increase the temporary impact by approximately 44 lf to cover the length requested as well as making sure we have
temporary impacts for dewatering/installation of the pipe (assumed 10 ft downstream and the 11 ft upstream which
was previously permitted).

Dear Ms. Amy Euliss:
Reference the Department of the Army (DA) permit issued on January 22,
2018, and subsequent modifications, to the North Carolina Department of
Transportation (NCDOT), for impacts associated with the new location project, TIPs
U-2579 C, Phase 2 and U-2579 D, E, & F, Winston-Salem North Beltway, in Forsyth
County, North Carolina.
On January 8, 2021 , the U.S. Army Corps of Engineers (Corps) received a
request from NCDOT to modify the standard permit for the U-2579 DEF project to
include an additional 43 linear feet of temporary stream impacts (Permit Site 20A),
and removal of 165 linear feet of temporary stream impacts (Permit Site 20C) that
will not be needed. The additional impacts are needed for a temporary pipe culvert
stream crossing for construction access for a crane. There are no changes to
wetland or permanent stream impacts.
The Corps has completed the evaluation of your request and has determined
that it is appropriate and reasonable, and no public notice is required for this
modification. Therefore, the permit is modified as requested and as shown on the
enclosed revised Permit Drawings, Sheets 64 and 96 of 97, received on January 8,
2021 .
Special Condition 1 is modified as follows:
1 . All work authorized by this permit shall be performed in strict compliance with
the attached sets of plans, "U-2579DEF: PERMIT MODIFICATION PACKAGE"
Sheets 1-97, dated December 18,2018, with revisions to Sheets 60, 61 , 72, 73, 74,
75, 96, & 97, received on November 5, 2020; Sheets 1-34, dated September 16,
2019; and Sheets 64 and 96 of 97, received on January 8, 2021 , which are part of

this permit. The Permittee shall ensure the construction design plans for this project
do not deviate from the permit plans attached to this authorization. Any modification
to the attached permit plans must be approved by the U.S. Army Corps of
Engineers (Corps) prior to any active construction in waters or wetlands.
All other conditions of the permit, including the mitigation requirements, and
permit expiration date of December 31 , 2028, remain in effect as written. Should
