ATTN: Mr. Ronnie Smith NCDOT Project Manager: Dear Sir: Subject: Notification for Nationwide Permit 14 (Minor Road Crossing) NCDENR 401 WQC 3820 Culvert Replacement SR-1751 Columbus County Rd, Bladen County State Project Number: 6C.009053 The North Carolina Department of Transportation (NCDOT) has scheduled to clear, grade, drain, base and pave SR-1751, Bladen County. Total project length is 0.57 miles. The roadway will be widened from 45 feet maintenance limits to 60 foot Right of Way. A 20 foot road bed with drainage ditches plus stabilized grass shoulders of 6 feet will be provided. This project will require the replacement of one (1) wet cross line, including fill of wetlands as shown below: Crossing Roadway Station Existing Culvert Proposed Culvert Permit site #1 Sta. 4+70.33 24 LF 18" RCP 48 LF 36" RCP New Stream Impacts = 24 linear feet Wetland Roadway Station Wetland Impacts Permit Site #1 Sta. 3+70 - Sta. 5+40 0.05 acres New Wetland Impacts = 0.05 acres Construction of this project will require impacts to waters of the United States in the Lumber River Basin. Permit site 91 is an Intermittent unnamed tributary of Boggy Branch (Class C; Sw:+). The project is located within the 03040203 Hydrologic Unit, sub basin 03-07-56. SR 1751 is a dead end road that stops in Columbus County. Only that last 500ft of SR 1751 is it Columbus County and is basically a driveway into a junky field. The Columbus County portion will be graded to the typical secondary road section, although it will not be paved. The driveway P. O. Bladen County's portion of SR 1751 serves several home sites. Permit Site 41 is located from between Station 3+70 to Station 5+40. This site compasses both wetland and stream impacts. An Intermittent unnamed tributary of Boggy Branch is located at Station 4+70.33. The current culvert is 24 LF 18" RCP with a proposed increase to a 48 LF 36" RCP. This stream is associated with a Headwater Forest and encompasses both sides of the road. Total wetland impacts are 0.05acres. RESOURCESTATUS In general, there will be minor impacts proposed as a result of the replacement of the culvert along SR-1751 (Columbus County Rd.) in Bladen County. This culvert site has undergone human manipulation. A Nationwide Permit 914 / NCDENR 401 WQC 3820 is required for the project. Endangered Species: Plants and animals with Federal classification of Endangered (E) or Threatened (T) are protected under provisions of Section 7 and Section 9 of the Endangered Species Act of 1973, as amended. As of September 22, 2010, the U.S. Fish and Wildlife Service lists seven federally protected species for Bladen County. T(S/A)- Threatened due to similarity of appearance, a species that is threatened Due to similarity of appearance with other rare species and is listed for its Protection. These species are not biologically endangered or threatened and Are not subject to Section 7 consultation. A biological conclusion of "No Effect" was reached for all species during the December 17, 2010 survey for protected species. Protected species were not identified, nor, was the general habitat for each species identified. REGULATORY APPROVALS Application is hereby made for a Department of the Army Nationwide Permit 14 / NCDENR 401 WQC 3820 as required by the above-described activities. The National Register was reviewed in addition to conducting a field view of the project for significant historic features. The project impacts are minor in nature and should not remove or degrade existing water quality uses. Pipe sizes proposed are the minimum necessary to create a safe highway while accommodating the 25-year design storm. There is no record within the NCDOT District 3 Office (Bladen County) that the road experiences flood overtopping. By utilizing the erosion control measures, downstream water quality standards will be protected. All appropriate BMP's will be utilized during construction. The existing culvert is deteriorated and needs to be replaced. Impacts will be minimized as much as possible. The culverts will be replaced utilizing a track-hoe excavator.
