Page 8 of 8 PCN Form -Version 1.3 December 10, 2008 Version A. 0 WP J d -0 U d 3 N - Fill o. 0 L? a u 0 3 N ? v 4 Q ? a 3 N y It Existing Site Condition: The shoulder and associated fill slope have failed along SR 2353 (Morphus Bridge Road) approximately 200ft west of the intersection at SR 2352 (Corbin Road). A contributing debris jam will be removed from just across the channel. The resulting failure has left a vertical raw bank for a distance of approximately 60ft at a depth of around 8ft. The conditions here are unsafe in that this cut face is as close as 1 ft away from the edge of pavement in some spots. Due to the instability in this area, continued erosion is sending sediment loads downstream and the fill slope and shoulder are getting more and more dangerous. This site must be repaired and the Wake County Maintenance forces are pursuing several options which are presented below. Please note that additional repair beyond the raw portion may be required by taking the repair up to the face of the headwall to the pipe crossing under Morphus Bridge Road. The repair may be less than but will not exceed 120 liner feet of hard armoring to the stream bank. Repair Description(s): Option # l: Reset the fill slope in the original location by using steel sheeting as a retaining wall. Soil material would be back filled against the linked sheeting and then the shoulder and resulting fill slope would rest against the top of the sheeting. Soil slope above the sheeting would be armored with rip rap underlined with filter fabric. The shoulder section will be vegetated by use of seeding and matting for long term stabilization. This option would provide room to install guardrail. Option #2: Re-set the fill slope in the original location by backfilling with soil material and then stabilize the new slope with rip rap armoring underlined with filter fabric. If the slope is too steep to support rock on fabric, less fabric will be used higher up the slope and replaced with thicker amounts of rip rap armoring. The shoulder section will be vegetated by use of seeding and matting for long term stabilization. Option # 3: Re-set the fill slope in the original location by backfilling with soil material and then stabilize the new slope with driven ground anchors with pressure plates underlined with filter fabric and bi-axil geo grid matting, plated with rip rap armoring. The shoulder section will be vegetated by use of seeding and matting for long term stabilization. Each of these options would involve the same methods of maintaining a dry work area. -Dewater segment of stream next to the failed fill slope using sand bags wrapped in plastic and/or sheet piling upstream and sandbags wrapped in plastic downstream of repair. -Set clean water pump on shoulder of road west of failure and run hose out of way through wooded area across stream. -Pump around water to re-enter stream just below downstream sandbags (use a piece of plastic in the streambed or another energy dissipator method to allow water to exit hose without causing turbidity or scour to the streambed or stream bank). -Prior to repair, pump out any clean water trapped within the work limits on downstream so that site is dry. -Use mud pump and sediment bag to pump out and treat any dirty water, should the work limits become wet during the repair. -Remove debris jam of accumulated sediments and woody debris (minimize disturbance to leaf litter and vegetation. -If raw dirt areas result on the stream bank from debris removal, stabilize using native seed and coir fiber matting with wooden stakes). -Repair failure. -Shoulder section between edge of pavement and rock armoring would be seeded and matted for restoring vegetation here. - No remaining disturbed areas will exist once rock armoring and/or matting is in place. -All dewatering measures will be removed.
