Sullins Director September 21, 2010 Subject: Additional Information Request Stormwater Permit Application TIP No. B-4183 NCSR 1526-Replacement of Bridges 28 and 29 Madison County Dear Mr. Henegar: Dee Freeman Secretary { On September 10, 2010, the Division of Water Quality, Asheville Regional Office received your application for a State Stormwater Management Permit for the replacement of two (2) bridges with box culverts on North Carolina Secondary Road 1526 (Crooked Creek Road). Both streams, Crooked Creek and Middle Fork Creek, are!classified as WS-11-HQW waters. Based on review of the application, narrative and design plans, it appears that the surface area of the project is being increased by approximately 500 square feet. We have found the submitted application to be insufficient and not in accordance with North Carolina Administrative Code 2H .1000 Stormwater Management. Therefore, we are requesting additional information in order to continue processing your application. Please submit additional information addressing the items listed below: NCDOT will have to provide stormwater treatment to the maximum extent practicable and control runoff from all surfaces generated by a one inch rainfall. Stormwater control measures and operation and maintenance plans shall be developed in accordance with Rule .1008 of the Section and designed in accordance with the latest version of NCDOT's Stormwater BMP Toolbox. As a last resort, when all other stormwater BMPs have been shown to be infeasible and impracticable, only then will the DWQ approve the use of grassed swales that have been designed according to the latest version of NCDOT's Stormwater BMP Toolbox. respond in writing within thirty (30) day of receipt of this letter.
