Dear Sirs:
On May 17, 2010 at approximately 6:00 p.m., Mr. Garcy Ward with the North Carolina
Division of Water Quality conducted a site inspection on the US 17 Washington By-pass
project, R- 2510A. Specifically, the inspection covered the area just north of the DOT
field office near the intersection of US 17 and SR 1127 (Possum Track Rd) at station
56+00.
Results of the inspection revealed that sediment and erosion control measures were either
insufficient or lacking, thereby leading to loss of sediment from the project and a
significant increase in turbidity in the receiving stream, an unnamed tributary (UT) to
Horse Branch (C;NSW). The road shoulder and roadside ditch on the south bound lane
of US 17 had been recently graded from a point approximately 1500' north of and
draining to the UT. 'The area was not stabilized and there were no sediment and erosion
control measures in place to treat runoff There is a skimmer basin near the base of the
