Army Corps of Engineers Raleigh Regulatory Field Office 3331 Heritage Trade Drive, Suite 105 Wake Forest, NC 27587 ?- Subject: Nationwide Permit #13 & 33 -- Slope Failure under Bridge #66 on SR 1145 in Lenoir Caldwell County, North Carolina Project Number: 11B.201411 (No DWQ Permit Fee Required) Dear Mr. Matthews: Due to recent heavy rain events a large portion of the fill slope under the subject bridge has failed and washed away. The old abutment depicted in the attached sketch has also been lost since these plans were drafted. This has left a vertical earthen wall under the bridge that is threatening the structural integrity of the structure. In order to correct this situation, we need authorization to reestablish the fill and armor plate the toe with approximately 80 linear feet of Class I and II riprap. The upper portion of the new fill will either be seeded and matted with coin fiber or armor plated as well. Matters are complicated at this site because the bridge is located at the intersection of SR 1145 and US64/NC18. The close proximity of the main highway as well as numerous utilities makes accessing the repair area from the north side nearly impossible. Safety regulations also prevent working from the bridge deck. As a result, a 0.001 acre clean stone temporary stream crossing is needed to access the repair area. A rubber tracked vehicle can then drive across the stone to conduct the repairs. Due to the shallow water depth, no pipes will be installed in the crossing since doing so would substantially increase the size and scope of the crossing. Our hydraulics unit has reviewed this proposal and concurs with this design. We will attempt to conduct the repairs by lining the floor of the entire repair area with large stone supplemented with class B stone to fill the voids to a point above the water mark. Fill materia!, likely ABC, will then be placed on top of this stone bed to reestablish the 2:1 slope under the bridge. The face of the fill can then be armor plated for protection. We believe that constructing the fill in this manner will minimize any turbidity issue. If for some unforeseen reason this P. O. Box 250, N. Even doing this will not prevent a fair amount of subsurface water from entering the work area. Installing the dike will literally require hundreds of sandbags and-will be very labor intensive. That is why we have proposed lining the floor of the repair with stone instead. We do however request authorization to install the diversion dike in the event it becomes necessary. If needed, this diversion will add 0.0014 acres to our dewatered total for the project. Any dirty water located inside of the diversion area can be pumped into a silt bag if necessary. Total dewatered area authorization requested for the project is 0.0024 acres. Once the new armor plated slope is established, the impervious dike and the temporary stream crossing will be removed and preconstruction streambed elevations will be restored. Bridge 466 crosses Lower Creek (Class: Q. This project was reviewed for the presence of threatened and endangered species in January of 2010. This project will have no effect on any federally listed endangered plant or animal species. Historic compliance work will be scheduled and concurrence with the State Historic Preservation Office will be obtained if required. The project impacts are not significant in nature and should improve water quality be eliminating this ongoing sedimentation source. All requested BMPs will be used during construction. Attached is a Preconstruction Notification Form, plan sheets, a marked county map, a marked topography map and photographs. By copy of this letter, it is requested that Mr. Ron Linville, Mountain Region Coordinator, with the North Carolina Wildlife Resources Commission comment directly to you concerning the 404 Nationwide Permit application with a copy sent to me.
