Box 2513 Reidsville, NC 27323-2513 Dear Mr. Hunsinger: You have our approval, in accordance with the attached conditions and those listed below, to impact 18 linear feet of an unnamed tributary to Dan River for the purpose of replacing one 72-inch corrugated metal pipe under SR 2178 (Philpott Road) in Rockingham County. The project should be constructed in accordance with your application dated 12 February 2001 (received 2 March 2001). After reviewing your application, we have decided that this fill is covered by General Water Quality Certification No. 3289 corresponding to the U.S. Army Corps of Engineers General [Regional] Permit Number 198200031. In addition, you should acquire any other federal, state or local permits before you proceed with your project including (but not limited to) Sediment and Erosion. Control, Non-Discharge and Water Supply Watershed regulations. This approval will expire with the accompanying 404 permit unless otherwise specified in the Water Quality Certification. This approval is valid solely for the purpose and design described in your application (unless modified below). Should your project change, you must notify the DWQ and submit a new application. If the property is sold, the new owner must be given a copy of this Certification and approval letter, and is thereby responsible for complying with all the conditions. If total wetland fills for this project (now or in the future) exceed one acre, or of total impacts to streams (now or in the future) exceed 150 linear feet, compensatory mitigation may be required as described in 15A NCAC 2H.0506 (h) (6) and (7). For this approval to be valid, you must follow the conditions listed in the attached certification and any additional conditions listed below: Placement of culverts and other structures in waters, streams, and wetlands must be placed below the elevation of the streambed to allow low flow passage of water and aquatic life • _ Existing stream dimensions are to be maintained above and below locations of culvertextensions. Use of rip-rap for bank stabilization is to be minimized; rather, native vegetation is to be planted when practical. • Mowing of existing vegetated buffers is strongly discouraged, so that they may beutilized for stormwater sheet flow. If you do not accept any of the conditions of this certification, you may ask for an adjudicatory hearing. You must act within 60 days of the date that you receive this letter. To ask for a hearing, send a written petition which conforms to Chapter 150B of the North Carolina General Statutes to the Office of Administrative Hearings, 6714 Mail Service Center, Raleigh, N.C. 27699-6714. This certification and its conditions are final and binding unless you ask for a hearing. Replacement with a longer and larger pipe is necessary to bring the road up to current construction standards and increase safety. No Wetlands, High Quality Waters, or Outstanding Resource Waters occur in the project area. No federally protected species were observed during surveys conducted on September 19, 2000 or October 29, 2000. The September 19, 2000 survey was conducted by Lenwood Smith, II to determine the presence or absence of protected plant species. The October 29, 2000 survey was conducted by Tim Savidge and Lenwood Smith, II to determine the presence or absence of protected mussel species. The NCDOT anticipates that this activity will be authorized under General Permit 19820031. Enclosed please find the project site map and the pre-construction notification form for the above referenced projects. Facing downstream February 12, 2001 Construction Sequence and Erosion Control Plans for Pipe Replacement with Pipe Rockingham County SR - 2178 We are scheduled to start in late March Construction is anticipated to take weeks. 1. Dig a diversion channel to dewater the construction area. The diversion channel will be approximately the same width as the existing channel width. 2. Line the channel with plastic to prevent sediment from the diversion channel. 3. Install silt dams to allow water to flow into the diversion channel. 4. Install a rock silt dam downstream of the diversion channel outlet to prevent incidental downstream sediment loss from the diversion channel. 5. Breach the inlet and outlet dams of the diversion channel. 6. Remove existing line of 42' long 72" CM pipe. 7. Excavate to sufficient depth to allow burial of the pipe to 1', place base stone, and single line of 60' long 84" X 55" CM pipe arch. 8. Backfill this line and place rip-rap at ends. 9. After the new pipe is placed, ABC aggregate will be placed at each end of the pipe forcing the flow through the pipe. The diversion channel will then be dewatered and a suitable backfill material will be placed and compacted. 10. Upon completion of the construction activity, notify landscape department for final seeding. 11. Silt fences to be placed as site conditions require. 12. Facing downstream February 12, 2001 Construction Sequence and Erosion Control Plans for Pipe Replacement with Pipe Rockingham County SR - 2178 We are scheduled to start in late March Construction is anticipated to take weeks. 1. Dig a diversion channel to dewater the construction area. The diversion channel will be approximately the same width as the existing channel width. 2. Line the channel with plastic to prevent sediment from the diversion channel. 3. Install silt dams to allow water to flow into the diversion channel. 4. Install a rock silt dam downstream of the diversion channel outlet to prevent incidental downstream sediment loss from the diversion channel. 5. Breach the inlet and outlet dams of the diversion channel. 6. Remove existing line of 42' long 72" CM pipe. 7. Excavate to sufficient depth to allow burial of the pipe to 1', place base stone, and single line of 60' long 84" X 55" CM pipe arch. 8. Backfill this line and place rip-rap at ends. 9. After the new pipe is placed, ABC aggregate will be placed at each end of the pipe forcing the flow through the pipe. The diversion channel will then be dewatered and a suitable backfill material will be placed and compacted. 10. Upon completion of the construction activity, notify landscape department for final seeding. 11. Silt fences to be placed as site conditions require. 12.
