GOVERNOR Division of Coastal Management 1367 US Hwy. 17 South Elizabeth City, NC 27909 December 21, 2000 ATTENTION: Ed Harrell, Pasquotank Field Representative DAVID McCOY SECRETARY "401 ISSUED" SUBJECT: Request for a GP 2300 permit authorization for Anticipated Impacts to Waters Resulting From Bridge Replacement of Bridge #42 in Pasquotank County The Department of Transportation plans to replace bridge #22 in Bertie County with three 112" x 75" corregated aluminum pipe arch at the Halls Creek crossing on Body Road (SR 1139). All work will be done within our right of way. An aluminum headwall with wings, and erosion control stone will be used on unstable slopes. No wetlands will be effected by this project. I have attached a DCM-MP-5 Bridges and Culverts form, a vicinity map, with a plan view of the proposed work, and certified notification of landowners along. The NC DOT requests this work be authorized under the provisions of a General Permit.2300 to replace the bridge. A check for $100.00 is attached to cover the permit fees. Work will begin as soon as the permits are issued. Thank you for your attention to this important matter.
