Dear Mr. Smith:
You have our approval, in accordance with the attached conditions and those listed below, to replace
two pipes on Chaseford Road (SR 3450) in Guilford County as part of a project to upgrade the road.
The project should be constructed in accordance with your application dated 6 April 2001 (received
10 April 2001). After reviewing your application, we have decided that this fill is covered by
General Water Quality Certification number 3289 corresponding to the Corps of Engineers Regional
(General) Permit Number 198200031. In addition, you should acquire any other federal, state or local
permits before you proceed with your project including (but not limited to) Sediment and Erosion
Control, Non-Discharge and Water Supply Watershed regulations. This approval will expire with the
accompanying 404 permit unless otherwise specified in the Water Quality Certification.
This approval is valid solely for the purpose and design described in your application (unless
modified below). Should your project change, you must notify the DWQ and submit a new
application. If the property is sold, the new owner must be given a copy of this Certification and
approval letter, and is thereby responsible for complying with all the conditions. If total wetland fills
for this project (now or in the future) exceed one acre, or of total impacts to streams (now or in the
future) exceed 150 linear feet, compensatory mitigation may be required as described in 15A NCAC
2H .0506 (h) (6) and (7). For this approval to remain valid, you must adhere to the conditions listed
in the attached certification.
If you do not accept any of the conditions of this certification, you may ask for an
adjudicatory hearing. You must act within 60 days of the date that you receive this letter.
To ask for a hearing, send a written petition which conforms to Chapter 150B of the North
Carolina General Statutes to the Office of Administrative Hearings, 6714 Mail Service
Center, Raleigh, N.C. 27699-6714. This certification and its conditions are final and
binding unless you ask for a hearing.

SUBJECT: Guilford County, Replacement of two pipes on Chaseford Road (SR 3450)
The North Carolina Department of Transportation (NCDOT) proposes to replace pipes at Stations
04+05 and 08+40 (measured from south to north) under Chaseford Road (SR 3450) as part of a
single project to upgrade Chaseford Road from an unpaved secondary road to a paved secondary
road. Both pipe replacements are necessary to bring the road up to current construction standards
and increase safety. The replacements will be as follows:

CMP means corrugated metal pipe
A special condition exists at station 04+05. Due the presence of a rock outcrop (see attached
photos) burial at 1-foot below the streambed elevation may not be achieved. Efforts will be made to
bury the pipe to a depth that allows for aquatic life movement. Blasting opportunities are limited due
to the close proximity of a private residence.
No Wetlands, High Quality Waters, or Outstanding Resource Waters occur in the project area. No
protected species were observed during a survey conducted on December 12, 2000 by Lenwood E.

Rip Rap sediment dam is required.
Stilling basin is not required.
Silt Fence will be installed as needed.
Seeding with be performed by Roadside Environmental as scheduled.
Comments:
We will replace the downstream half of the existing pipe with traffic running in one lane
on the upstream side. Once the downstream side is complete, we will shift traffic to the
downstream side so we can replace the upstream half of the pipe. Once the pipe is
installed, we will install rip-rap outside the stream bed stabilize the ends of the pipe.
New pipe will be buried 20% below the existing stream bed. Once work is complete, the
temporary dam will be completely removed.

SUBJECT: Guilford County, Replacement of two pipes on Chaseford Road (SR 3450)
The North Carolina Department of Transportation (NCDOT) proposes to replace pipes at Stations
04+05 and 08+40 (measured from south to north) under Chaseford Road (SR 3450) as part of a
single project to upgrade Chaseford Road from an unpaved secondary road to a paved secondary
road. Both pipe replacements are necessary to bring the road up to current construction standards
and increase safety. The replacements will be as follows:

RCP means reinforced concrete pipe
CMP means corrugated metal pipe
A special condition exists at station 04+05. Due the presence of a rock outcrop (see attached
photos) burial at 1-foot below the streambed elevation may not be achieved. Efforts will be made to
bury the pipe to a depth that allows for aquatic life movement. Blasting opportunities are limited due
to the close proximity of a private residence.
No Wetlands, High Quality Waters, or Outstanding Resource Waters occur in the project area. No
protected species were observed during a survey conducted on December 12, 2000 by Lenwood E.
