Box 5189 Horse Shoe, N.C. 28742 j Dear Mr. Swain: You have our approval to placing fill material in 0.05 acres of wetlands or waters for the purpose of road maintenance at SR 1109 (Hannah Ford Road), as you described in your application dated 6 October 1995. After reviewing your application, we have decided that this fill is covered by General Water Quality Certification Number 3026 or 2735. This certification allows you to use Nationwide Permit Number 3 or General Permit 031 when it is issued by the Corps of Engineers. This approval is only valid for the purpose and design that you described in your application. If you change your project, you must notify us and you may be required to send us a new application. For this approval to be valid, you must follow the conditions listed in the attached certification. In addition, you should get any other federal, state or local permits before you go ahead with your project. If you do not accept any of the conditions of this. certification, you may ask for an adjudicatory hearing. You must act within 60 days of the date that you receive this letter. To ask for a hearing, send a written petition which conforms to Chapter 150B of the North Carolina General Statutes to the Office of Administrative Hearings, P.O. Box 27447, Raleigh, N.C. 27611-7447. This certification and its conditions are final and binding unless you ask for a hearing. SR 1109 is a gravel road that we propose to clear, grub, grade, replace/extend pipes, and pave for a distance of 2.5 miles. Grading will be moderate. We are requesting permission to replace and extend pipes in live streams noted on plans at survey stations 47+15, 94+50, 96+65 and 123+00. Pipes will be replaced using our standard method of pipe installation described in the erosion control plans. NCDOT "Best Management Practices for Protection of Surface Waters" and proper phasing of erosion control items will be used to minimize and control sedimentation and erosion. All pipes shall be laid in accordance with section 300 of the North Carolina Department of Transportation "Standard Specifications for Roads and Structures", January 1, 1990. Pipes will be installed in the dry wherever possible. Pipes laid in streams large enough to support trout will be installed six (6) inches below the stream bed to allow the bottom of the pipe to silt over so that trout movement will be over a natural stream bed. 0 1 :1 > CL ? ? r b . ? I ? • I I I I SILT BASIN TrPE-B; ' E A silt basin type-B is a temporary basin built to collect sediment flowing through a drainage way. The silt basin is built at the base of the ditch with its length being at least 2 times the width of the basin. The type -B silt basin is generally built in conjunction with temporaryfrock silt checks and other devices that control or slow down water flow. This device must be monitored closely and cleaned out on a regular basis until grass is established along the ditch line and sedimentation is no longer a problem.
