Box 5189 Horse Shoe, N.C. 28742 Dear Mr. Dumas: You have our approval to place fill material in 0.01 acres of wetlands or waters for the purpose of extending pipes at SR 1567, as you described in your application dated 25 April 1995. After reviewing your application, we have decided that this fill is covered by General Water Quality Certification Number 2735. This certification allows you to use General Permit Number 031 when it is issued by the Corps of Engineers. This approval is only valid for the purpose and design that you described in your application. If you change your project, you must notify us and you may be required to send us a new application. For this approval to be valid, you must follow the conditions listed in the attached certification. In addition, you should get any other federal, state or local permits before you go ahead with your project. If you do not accept any of the conditions of this certification, you may ask for an adjudicatory hearing. You must act within 30 days of the date that you receive this letter. To ask for a hearing, send a written petition which conforms to Chapter 150B of the North Carolina General Statutes to the Office of Administrative Hearings, P.O. Box 27447, Raleigh, N.C. 27611-7447. This certification and its conditions are final and binding unless you ask for a hearing. C. 28801 SUBJECT: Proposed Pipe Extension SR 1567, Baynard Road Transylvania County Dear Mr. Baker, I am attaching a straight line diagram with erosion control plan, an application form for a Nationwide Permit, and a county map section locating SR 1567, Baynard Road in Transylvania County. SR 1567 is a gravel road that we propose-to clear, grub, pave, and replace/extend pipes for a distance of 0.24. miles. Grading will be moderate. We are requesting permission to replace and Extend a pipe in a live stream noted on plans at survey station 6+00. The pipe will be replaced using our standard method of pipe installation described in the erosion control details - Appendix A of the erosion control plans. NCDOT Best Management Practices and proper phasing of erosion control items will be used to minimize and control sedimentation and erosion. By copy of this letter, we are asking Mr. David Yow, N. C. Wildlife Resources Commission, to comment directly to the Corps of Engineers concerning the Nationwide Permit request. Upon review for the Corps of Engineers we are requesting that Mr. Yow forward a copy to us (NCDOT) as well. All pipe installed on the project will be installed using the standard method of pipe installation - Method "A", Standard No. 300.01B of the NCDOT - "Roadway Standard Drawings". All pipes shall be laid in accordance with section 300 of the North Carolina Department of Transportation "Standard Specifications for Roads and Structures", January 1, 1990. Pipes will be installed in the dry wherever possible. Basins to be cleaned out after every storm event and when basins are half full. SILT BASIN TYPE-B: A silt basin type-B is a temporary basin built to collect sediment flowing through a drainage way. The silt basin is built at the base of the ditch with its length being at least 2 times the width of the basin. The type-B silt basin is generally built in conjunction with temporary rock silt checks and other devices that control or slow down water flow. This device must be monitored closely and cleaned out on a regular basis until grass is established along the ditch line and sedimentation is no longer a problem. AREAS OF USE: • In conjunction with temporary rock*silt checks and other devices used in drainage ditches. IUI° Ip 111 Seclicn to I Htcr FabrIc cc Gravcl Llycr June, 1992 TEMPORARY SLOPE DRAIN w= EARTH BERNI: ?-- {-- A temporary slope drain is a flexible tubing or pipe used to carry concentrated runoff from the top to the bottom of a cut or fill slope without causing erosion along the slope. An earth berm is an earth barrier designed to channel runoff into a number of erasion control devices. A berm is usiially constructed with typical 'on- site material. T -sections and elbows at the inlets are used to intercept the directional flow of the runoff, to protect the temporary slope drains from washouts. AREAS OF USE: • At construction areas where storm water runoff above a cut or fill slope will cause erosion if allowed to flow over a slope. . DESIGN CRI'TERIA: • Peak runoff from 10 year storm. •. T section shall be used at inlet for multiple directional flow. • Elbows shall be used at inlet for single directional flow.
