Subject: Request for a NW 3 Permit and DWQ 401 for the replacement of Bridge # 20 over Sawyer's.
Creek on NC 343 in Camden County.
The North Carolina Department of Transportation is requesting the issuance of a NW 3 permit
and corresponding DWQ certification for the replacement of bridge # 20 in Camden County. The bridge is
located on NC 343 approximately 0.5 miles west of the intersection of US 158.,The bridge spans Sawyer's.
Creek, which is classified by DWQ as class "C, SW" waters. Bridge # 20 is currently a concrete structure
that is 132 ft. long by 26 ft. wide. This structure will be removed using NCDOT's guidelines for bridge
removal. The proposed bridge will be replaced on its existing alignment. The new bridge structure will be
172 ft. in length by 32 ft. wide and built with precast cored slab concrete deck, support piles and bents. The
bridge roadway approaches will have to be reconstructed. An off-site detour will be used during
construction. This project will be funded through federal stimulus money.

3a. Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this
application:
The existing bridge is a 26' X 132' concrete bridge. The existing roadway is a 20 ft. asphalt surface roadway with 8 ft.
grassed shoulders. This bridge crosses Sawyer's Creek. Sawyer's Creek is classified as a Class "C, SW" water by the
Division of Water Quality. Landuse within the vicinity of the project consists of forrested wetlands, residential, and a
school building with athletic fields.

0.074 acres
3c. List the total estimated linear feet of all existing streams (intermittent and perennial) on the property:

3d. Explain the purpose of the proposed project:
Replace bridge # 20, which has been identified as structurally eligible for the federal bridge replacement program
3e. Describe the overall project in detail, including the type of equipment to be used:
The existing concrete bridge deck and timber piles will be removed in accordance with NCDOT guidelines for bridge
demolition and removal. The new bridge will be 172 feet in length by 32 feet wide. The new bridge will be a four span
cored slab concrete structure supported by concrete piles. The old fill approaches will be removed and graded to the
adjacent wetland elevation. The new roadway approaches will be widened to accommodate the bridge structure.
Equipment used during this project will include a crane, excavator, grading equipment, and dump trucks.
