Upon review of your request for 401 Water Quality Certification
to place fill material in 0.06 acres of wetlands or waters which are
tributary to S. Fork New River for road maintenance located at SR 1307
in Alleghany County as described in your submittal dated 17 November
1993, we have determined that the proposed fill can be covered by
General Water Quality Certification No. 2745. A copy of the General
Certification is attached. This Certification may be used in
qualifying for coverage under Corps of Engineers' Nationwide Permit
No. 3.
If this Certification is unacceptable to you, you have the right
to an adjudicatory hearing upon written request within thirty (30)
days following receipt of this Certification. This request must be in
the form of a written petition conforming to Chapter 150B of the North
Carolina General Statutes and filed with the Office of Administrative
Hearings, P.O. Box 27447, Raleigh, N.C. 27611-7447. Unless such
demands are made, this Certification shall be final and binding.

Re: Proposed NCDOT improvements to SR 1307 (Jack Caudle
Road) to include grade, drain, base and pavement for
1.26 Miles in Alleghany County.
Dear Mr. Jolly:
The North Carolina Department of Transportation, District I
Maintenance proposes to grade, drain, base and pave 1.26
miles of SR 1307 (Jack Caudle Road) in Alleghany County.
Work to be performed includes the replacement and/or .
extension of pipes that involve live streams at Sta. 4+00,
Sta. 5+65, Sta. 24+35, Sta. 30+50, Sta. 31+75, Sta. 34+00,
Sta. 37+75, Sta. 51+65, and Sta. 66+00 into unnamed streams
and replacement or extension of various other pipes
throughout the project which do not involve streams.
We bel eve th grading can be accomplished under Nation Wide
Permit`(3). We are applying for issuance of a 404 Permit.
Please sider this letter, accompanying sketches of
planned activities, and copy of letter to WRC as formal
application to allow this work to proceed.
These streams do not appear to be designated Public Trout
Waters; however, by copy of this letter I am asking Mr.
Joseph Mickey, Jr., N. C. Wildlife Resources Commission,
District Fisheries Biologist to comment directly to you
concerning the 404 Nationwide Permit Request. We will
implement all necessary erosion and sediment control
measures during p-rojort construction and will expend every
effort t comp ete the work quickly.

GENERAL NOTES ON EROSION CONTROL
si]t Basin Type B's will be constructed to the dimension of
Aix feet by three feet by three feet unless otherwise noted
on the plane.
Generally, the order of installation of the erosion control
meagureb Will be as follows:
Temporary Silt Basins shall be Installed before
clearing and grubbing begins.
Silt Fences and temporary silt ditches shall be
installed after clearing and before grading.
Temporary Stone ditch checks shall be installed as
soon as ditchline is established.
Pipe Outlet and inlet protection will be done as
boon as the pipe is installed.
Seeding and ottier permanent erosion control
measures are to be implemented as goon as practical.
