Upon review of your request for 401 Water Quality
Certification to place fill material in 0.04 acres of wetlands or
waters which are tributary to Long Shoals Creek for bank
stabilization and road improvement located at SR 1575 in Ashe
County, we have determined that the proposed fill can be covered
by General Water Quality Certification No's. 2665 and 2732 issued
January 21, 1992. Copies of the General Certifications are
attached. These Certifications may be used in qualifying for
coverage under Corps of Engineers' Nationwide Permit No's. 13 and
14. Culverts shall be placed into the stream by 6 to 12 inches.

Dear John
The Department of Transportation is scheduled to grade, drain, base,
and pave'`priaarily`along the existing roadway of SR 1575 (Shelter
Church Rd), in Ashe County during the 1992-1993 fiscal year.
Due to the;'proximity of said stream, bank stabilization is required
with Class-II Rip/Rap for armor protection, we are requesting 404
Nationwide permit no. 13. Also there are 3 existing cross line
drainage?structures that will need replacing, or upgrading, in which
we `are requesting a 404 Nationwide permit no. 14.
Attached is a straight line sketch with proposed erosion control,
stream,'stream embankment, pipe crossings, and construction limits.
In'addition I am sending sketch of areas along the road, with cross
sections`to'cover same where project encroaches inside the 25 ft.
buffer zone, and marked county map.
By copy':of this letter I am asking Mr. Joe Mickey, Jr., of the North
,Carolina`.Wildlife Resources Commission District Fisheries Biologist
- to comment directly to you concerning the 404 Nationwide Permit
requests
Also by-,copy of this letter, I am requesting authorization,to proceed
under Section 401 of the Clean Water Act from the Division of
Environmental Management.

RIP RAP MATERIALS
1042-1 PLAIN RIP RAP.
Stone for plain rip rap shall consist of field stone or rough unhewn quarry
stone. The stone shall be sound, tough, dense, resistant to the action of air
and water, and suitable in all other respects for the purpose intended.
Where broken concrete from demolished structures or pavement is avail-
able, it may be used in place of stone provided that such use meets with the
approval.of the Engineer. However, the use of broken concrete which
contains reinforcing steel will not be permitted.
The stone or broken concrete shall be graded to meet the following
requirements:
1. For Class 1 Rip Rap. Stone shall vary in weight from 5 to 200 pounds.
At least 30 percent of the total weight of the rip rap shall be in
individual pieces weighing a minimum of 60 pounds each. Not more
than 10 percent of the total weight of the rip rap may be in individual
pieces weighing less than 15 pounds each.
2. For :Class 2 Itip Rap. Stone shall vary in weight from 25 to 250
pounds. At least 60 percent of the total weight of the rip rap shall be in
individual pieces weighing a minimum of 100 pounds each. Not more
than 5.. percent of the total weight of the rip rap may be in individual
pieces weighing less than 50 pounds each.

EROSION CONTROL SCHEDULE
(1) All adequate temporary erosion control measures will be
installed before clearing and grubbing operations begin.
(2) As construction progresses temporary and permanent
erosion control devices shall be installed in a timely
manner.
(3) All siltation and erosion control devices shall be
properly maintained until no longer needed.
(4)` As project progresses, request will promptly be made to
the Division Roadside Environmental Unit for stage and
permanent seeding.
(5) Any removal of siltation devices will be based on the
condition of established ground cover of that area the
device is serving.

Dear John : WATER UALITY SECTION
The `Department of Transportation is scheduled to grade, drain, base,
and pave'priiharily along the existing roadway of SR 1575 (Shelter
Church Rd.`), in Ashe County during the 1992-1993 fiscal year.
Due to ahe;proximity of said stream, bank stabilization is required
with Class'II'Rip/Rap for armor protection, we are requesting 404
Nationwide permit no. 13. Also there are 3 existing cross line
drainage"structures that will need replacing, or upgrading, in which
we are=,ireQnesting a 404 Nationwide permit no. 14.
Attached is a straight line sketch with proposed erosion control,
stream,..stream embankment, pipe crossings, and construction limits.
In'additian I am sending sketch of areas along the road, with cross
sections to cover same where project encroaches inside the 25 ft.
buffer zone, and marked county map.
By copy of this letter I am asking Mr. Joe Mickey, Jr., of the North
Carolina Wildlife Resources Commission District Fisheries Biologist
to comment directly to you concerning the 404 Nationwide Permit
request.
Also by copy of this letter, I am requesting authorization,to proceed
under Section 401 of the Clean Water Act from the Division of
Environmental Management.
