The North Carolina Department of Transportation (NCDOT) proposes to extend the structure and widen the narrow and unsafe shoulders above both ends of the pipe (Figures 1 - 3). Impacts to Waters of the United States The project crosses a perennial stream, an unnamed tributary to Toms Creek, [NCDENR- DWQ Index No. 27-24, Best Usage Classification C NSW; HUC 03020201] (Figure 3) in the Neuse River Basin. Please note that the roadway shoulders above the existing concrete structure are narrow and unsafe (Photographs 1 and 2). In order to widen the associated roadway shoulders, two joints will be added to both ends of the pipe. Once the joints are set, the associated roadway shoulders will be rebuilt and the areas surrounding both pipe ends will be stabilized using riprap for armoring. Please note that dire to the alignment of the stream on the outlet, a slight channel change is necessary to connect the new pipe outlet to the stream channel. The NCDOT proposes to temporarily dewater the work zone during the construction. This will involve the temporary placement of sandbags in the creek upstream and downstream of construction. Once the stream has been dewatered, the pipe will be extended. Permanent impacts at the site involve a total of 16 feet of surface water fill (for four new pipe segments), 8 ft of additional stream loss (due to the channel change), and 16 feet of rip rap placed along the streambanks to protect the pipe ends. Please note that Oft of rip rap armoring overlaps the reported impacts for the channel change. Therefore the grand total is 36 linear feet of impacts. Concurrent temporary impacts to the stream from this activity will result in the dewatering of 36 linear feet of stream. The work zone will be dewatered for approximately 3 days. All materials used for dewatering will be removed after construction. Please note that the soils surrounding this cross line are mapped as hydrie: Worsham sandy loam. However no wetland areas exist within the project limits. Therefore this maintenance x•ork does not include proposed impacts to wetland areas. Division 5, 2612 N. Therefore this maintenance work does not include proposed impacts to wetland areas. Application is hereby made for a Department of the Army Section 404 Nationwide Permit (NWP) 3 and NWP 33. Additionally, this application provides notification to the NCDWQ as Section 401 Water Quality Certification (WQC) No. 3494 and No. 3366 will be utilized for construction of this project. Describe the overall project in detail, including the type of equipment to be used: So that the shoulder sections above the pipe ends can be widened to safer widths, the NCDOT will add two joints to both ends, back fill, and armor with rip rap. Please note that due to the alignment of the stream on the outlet, a slight channel change is necessary to connect the new pipe outlet to the stream channel. The NCDOT will also dewater the work zone during construction. Equipment may include excavator, backhoe, bulldozer, grader, and crane. 1 1. Explain the purpose of the proposed work: NCDOT will extend the existing_pipe in order to widen the narrow and unsafe shoulders above the nice ends. IV. Proposed Impacts to Waters of the United States/Waters of the State It is the applicant's (or agent's) responsibility to determine, delineate and map all impacts to wetlands, open water, and stream channels associated with the project. Each impact must be listed separately in the tables below (e.g., culvert installation should be listed separately .from riprap dissipater pads). Be sure to indicate if an impact is temporary. All proposed impacts, permanent and temporary, must be listed, and must be labeled and clearly identifiable on an accompanying site plan. All wetlands and waters, and all streams (intermittent and perennial) should be shown on a delineation map, whether or not impacts are proposed to these systems. Wetland and stream evaluation and delineation forms should be included as appropriate. Photographs may be included at the applicant's discretion. If this proposed impact is strictly for wetland or stream mitigation, list and describe the impact in Section VIII below. If additional space is needed for listing or description, please attach a separate sheet. Provide a written description of the proposed impacts: A total of 16ft of surface water fill will result from adding two joints to both ends of the pipe. Also an additional 8ft of stream loss will result on the outlet end due to a necessary channel change. Additionally 8 ft of rip rap armoring will be added to inlet and outlet end.
