You have our approval to place fill material in 3.0 acres of wetlands or waters for the purpose of
relocating an existing lightguide cable, as you described in your application dated 12 December 1996. After
reviewing your application, we have decided that this fill is covered by General Water Quality Certification
Number 3022. This certification allows you to use Nationwide Permit Number 12 when it is issued by the
Corps of Engineers.
This approval is only valid for the purpose and design that you described in your application. If you
change your project, you must notify us and you may be required to send us a new application. If total
wetland fills for this project (now or in the future) exceed one acre, compensatory mitigation may be required
as described in 15A NCAC 2H .0506 (h) (6) and (7). For this approval to be valid, you must follow the
conditions listed in the attached certification. In addition, you should get any other federal, state or local
permits before you go ahead with your project. Also this approval will expire when the accompanying 404 or
CAMA permit expires unless otherwise specified in the General Certification.
If you do not accept any of the conditions of this certification, you may ask for an adjudicatory hearing.
You must act within 60 days of the date that you receive this letter. To ask for a hearing, send a written
petition which conforms to Chapter 150B of the North Carolina General Statutes to the Office of
Administrative Hearings, P.O. Box 27447, Raleigh, N.C. 27611-7447. This certification and its conditions
are final and binding unless you ask for a hearing.
This letter completes the review of the Division of Water Quality under Section 401 of the Clean Water

U.S. Hwy 17 (NCDOT Project R2405C) and (NCDOT Project R2405BA: Part 2)
Permit Requirements
To Whom It May Concern:
Due to the North Carolina Department of Transportation's (NCDOT) proposed construction along Hwy
17, AT&T has been requested to relocate their existing lightguide cable. EA Technical Services, Inc.
(EA) is performing the design engineering for the proposed relocation.
Enclosed for your review is the Joint Application ( 7 copies) and two Location Drawings (7 copies of
each) for the proposed AT&T cable relocation on the above referenced projects. The location of the
existing cable and the proposed relocation are indicated on the drawings.
Current construction procedures are to place the facility in 2" steel pipe by open trench method at a
depth of 48". Pipe (2" steel) will be installed under all asphalt drives and roadways by bore method.
Stream crossings will be by open trench or bore method as directed by environmental permits. The
tentative construction schedule is to begin in March 1997. Construction will be completed within 2 to 3
months after the start date.
We need to know what permits or authorizations, if any, we are required to apply for. Please review the
enclosed Location Drawings and advise as to any permitting requirements or procedures we need to
comply with.
