Marsh transmitting an original, completed Affidavit of Notification and list of adjoining property owners and the chief administrative officer for the above referenced application. In order for this office to confirm that the Notice of Application was received by each person listed, please forward to my attention copies of all certified mail return receipts cards. This office needs this information no later than Tuesday, May 4, 1993. You may fax this information to me directly at 919-733-2876 and mail a hard copy to me at the following address: Land Quality Section P.O. A copy of the current tax map f or this site, indicating owners of record adjoining the site, should be forwarded to this office to clarify this issue. A separate letter from this office will be sent to you in the near future regarding any additional information and clarification that is needed regarding your mining permit application.- Any information requested will need to be addressed1before this office can complete its review of the application materials.
