Flyrock Incident of June 21, 2004 Bamboo Road Quarry Mining Permit No -95-03 Watauga County Dear Mr. Cecile: This letter is in response to your letter and the accompanying flyrock investigation report prepared by Mr. Bob Shaver of Orica dated June 27,2004. The report indicated that the flyrock at the above referenced mining operation was likely caused by inadequate stemming of the boulder shot 0, After reviewing your letter 'and Mr, Shaver's report,- your company is hereby approved to- resume blasting activities at the above referenced site. Please be aware that this approval is contingent upon your company incorporating the recommendations outlined in your letter and Mr. Shaver's report into your blasting procedures,at this mine site. These recommendations are as follows: No blasting of boulders shall occur at this site 2) Only one half inch diameter clean stone shall be used for stemming all blast holes. 3) Blasting operations shall be avoided during,inclemeht weather conditions. Again, please note that the'above recommendations will be considered part of the blasting conditions of Mining. Permit No. 95-03. This office appreciates your company's cooperation in this matter. Furthermore, we are confident that your company will take all the necessary steps needed at this facility to prevent similar incidents from occurring in the future.
