We have completed a review of your construction drawings, drainage/stormwater, erosion and sediment control, and/or special study and have noted comments both in red on the attached drawings and on the attached "Review Comments," which includes a construction checklist and general comments, drainage/stormwater, erosion and sedimentation control comments, and/or special study comments. Note that if there is an attached set of red-lined drawings it is the only set and must be returned, along with 3 sets of revised drawings and three sets of individual written responses to all comments Monday by 12:00 noon, in order to continue in the review process. If these items are not returned with the next submittal, that submittal will be considered incomplete and returned to submitter. 2. With each re-submittal you make, please complete and supply the attached "Certification Plan Resubmittal" on page 2 of this packet. 3. Please contact Bobby Boling at 919-557-3919 for clarification of construction drawing comments, Darin Eyster at 919-557-2921 for clarification of drainage/stormwater, erosion and sedimentation control comments, and/or if applicable, the staff person indicated above at 919-557-3939 for clarification of special study comments. 4. After all review cycles and related comments have been addressed and you are notified accordingly by this office, please: ¦ Submit six complete sets of construction drawings including the Drainage/Stormwater, Erosion and Sediment Control plan and one dxf file format on CD; ¦ If applicable, two copies of the final paper copy and one dxf file format on CD of any special study; ¦ Contact Heather Keefer at 557-2909 to schedule a pre-construction meeting (to be held in the office). Since the percent grade of the proposed HDPE pipe is steeper than 10% please provide manufactures specifications and verify that the pipe meets these specifications. Special attention should be made to the pipe joints and fittings to prevent sliding and separating. 2. A Land Disturbance Permit will be required for this project since the amount of disturbed area is greater than 20,000 square feet. This permit will be issued after all applicable NC DWQ approvals have been obtained. Please provide all applicable NC DWQ approvals including but not limited to any approved and recorded Operation and Maintenance Agreements or Deed restrictions. 3. Please note that a condition of plan that stream restoration and sediment removal must occur before the Land Disturbance Permit can be closed out for this project. 4. Please extend Tree Protection Fence around the entire limits of disturbance. 5. Please show all erosion control graphically on the plans this includes but is not limited to a construction entrance and inlet protection. 6. Please show the existing temporary bypass system graphically on this plan. 7. Please add more detail in the sequence, notes and on the plan to verify that the location of the pipe is adequate throughout construction and that it is feasible to construct the pipe at the end of every day of construction. Consideration of weather, location and stabilization of the pipe in respect to the proposed grading activities. 8. Please graphically note the amount of disturbed area on the plans. 9. Please provide cross sections of the sloe and the drainage system on the plan. Both existing as-built conditions and proposed conditions should be shown on the plan. 10. Provide verification that NC DWQ will allow the BMP to be located on two properties. 11.A private drainage easement must be recorded prior to issuance of CO for the house and closing out of the Land Disturbance Permit. 12. Please provide both graphic and sequence information to address the stockpiling of good material and bad material while it is being hauled on and off of the site. 9009A Rev 611812008 Page I 13. Please notes regarding safety during the excavation and fill of material on the slope. 14. Please note in the sequence that a Certificate of Compliance will be required to verify that the erosion control has been installed properly prior to starting construction on the site. Drew Johnson should be contacted to set up this inspection. (possibly after step #3 in the sequence) 15. A preconstruction meeting will be required in the office to go over the permit conditions and sequencing of construction. A second preconstruction meeting may be required on site prior to issuance of the Certificate of Compliance based on any sequencing concerns that may have occurred during the first preconstruction meeting. 16. Due to the sensitivity of this a condition of plan approval will require that the slope stabilization will occur within 7 calendar days of completion of any phase of grading on the slope. 17. Please change the sequence to contact the Engineering Department at 557- 3926 to set up the preconstruction meeting. Note: Once plan review comments have been adequately addressed the Town will request 6 sets of plans to be submitted for approval and notify the owner that they can set up a preconstruction meeting. 18.All storm drainage manhole lids should say stormwater "drains to stream do not dump waste" or similar text. 19. Please provide more information regarding the orifice in the outlet structure will this be a square or round hole? Please show graphically on the detail. 20.An engineer's certification will be required for the construction of the slope and the stormwater system. 21. Digital and Mylar as-builts of the stormwater system will be required prior to CO and closing out of the Land Disturbance Permit. 22.The minimum orifice size is 2". 23. Considering the soil type and erosive nature of the in-situ soils, will the proposed dry detention lead to future slope failure?
