The purpose of this letter is to request a revision to the Section 404 and 401 permit application submitted
for U-3334 The project, located north of Smuthfield in Johnston County, is referred to as the Booker
Daisy Rd extension and is approximately 4 1 mules in length The project has been divided into two
sections, Section A and Section B The previously submitted permit application requested authorization
for the construction on Section A, between US 70 Business and the Booker Dairy/Buffalo Rd
intersection
This revision request is for additional temporary impacts of <0 01 acre in jurisdictional wetlands on
Section A
The revised design will not change the existing permit application with respect to
• Protected Species
• Proposed Aquatic Moratoria
• FEMA compliance
• Cultural Resources
• Avoidance and Minimization
Summary of Changes
The additional impact involves excavation in a jurisdictional wetland outside of proposed fill for the
project to allow installation of a proposed 10 inch diameter ductile iron gravity sanitary sewer line
Excavation will be approximately three feet wide and ninety feet in total length The excavated material

will be placed back in the wetland and restored to preconstruction levels after the proposed sanitary sewer
line is installed As a maintained sewer line easement already exists where the installation will occur, the
NCDOT Roadside Environmental Unit will re-vegetate this area with a seed crux that contains native
wetland grasses
Additional impacts to jurisdictional wetlands are proposed at -L- 62+18 (see attached utility impact
drawing), which is included in permit drawing Site 1 (wetland WW) in the original application Total
additional temporary wetland impacts for the project are projected to be <0 01 acre Permanent impacts
to wetlands have not changed from what was requested in the original permit application
Compensatory Mitigation
Since the additional wetland impact is temporary, no additional compensatory wetland mitigation is
proposed
Regulatory Approvals
Section 404 Please mcorporate these revisions into the previously submitted Individual Section 404
permit application
Section 401 We are also submitting this as a revision to the Section 401 Water Quality Certification
application from the NCDWQ In compliance with Section 143-215 3D(e) of the NCAC, we have
provided $570 00 with the original application to act as payment for processing the Section 401 permit
We are providing five (5) copies of this revision to the NCDWQ, for their review and approval
