Dear Mr. Curtis:
On September 3, 2008, the Division of Water Quality (DWQ) received your application dated
August 18, 2008, to fill or otherwise impact 0.016 acres of 404/wetland and 88 linear feet of
perennial stream to construct the proposed residential subdivision at the site. The DWQ has
determined that your application was incomplete and/or provided inaccurate information as
discussed below. The DWQ will require additional information in order to process your
application to impact protected wetlands and/or streams on the subject property. Therefore,
unless we receive five copies of the additional information requested below, we will place this
project on hold as incomplete until we receive this additional information. If we do not receive
the requested information, your project will be formally returned as incomplete. Please provide
the following information so that we may continue to review your project.
Additional Information Requested:
1. Please indicate all stream impacts including all fill slopes, dissipaters, and bank
stabilization on the site plan. Crossing No. 3 is not shown.
2. Please indicate all wetland impacts including fill slopes on the site plan. Stream impacts
should not be counted as wetland impacts.
3. The stream crossing labeled as Private Driveway No. 1 on the plans does not appear to be
necessary. Please eliminate the crossing or provide additional information as to why it is
necessary for this project.
4. Parcel No. 12 appears to have adequate land on the east side of Crows Creek to construct
a sizeable home. Please expound on why a driveway crossing is necessary at this site.

Additionally, please provide culvert calculations requiring the double barrel culvert and
provide a detail showing how aquatic life passage will be achieved without over-
widening the stream (e.g. sill on second culvert, second culvert is a floodplain culvert,
etc.).
5. Please clearly label impacts on the plan, including linear footage. The impacts on the
plan should correspond exactly with the impacts on the PCN form.
Please submit this information within 30 calendar days of the date of this letter. If we do not
receive this requested information within 30 calendar days of the date of this letter, your project
will be withdrawn and you will need to reapply with a new application and a new fee.
This letter only addresses the application review and does not authorize any impacts to wetlands,
waters or protected buffers. Please be aware that any impacts requested within your application
are not authorized (at this time) by the DWQ. Please call Ms. Cyndi Karoly or Mr. Ian
