Dear Mr. Safrit:
The Division of Water Quality (DWQ) received your application for a 401 Water Quality Certification
for the subject project on April 29, 2008. 1 have reviewed the application and the site (May 9th); the
following information is needed by this office in order to complete our review:
a) This office prior to issuance of a certification must receive an application to, and letter of
acceptance from, the Ecosystem Enhancement Program.
b) During the site visit it was mentioned that the possible construction of the blood bank is not
definite and the area of construction was maximize. Please provide details/site diagram for
the minimum size allowable for construction.
C) Please determine if the stream can be relocated along the periphery of the site.
d) For the current plan, a headwall should be utilized at the downstream end of the culvert to
further minimize stream impacts.
Pursuant to 15A NCAC 2H .0507(h), please be advised that this project will be placed on hold.
Failure to submit the requested information by June 8, 2008 may result in the application being withdrawn
from consideration. You may reapply, but a new fee and application will be required. In addition, this
office is requesting that the Army Corps of Engineers withhold processing the 404 permit until the
requested information is provided. If you have any questions, please contact me at (704) 663-1699.
