Forest Service is proposing to do work on Point Lookout Trial (FSR 203) in Pisgah National Forest, McDowell County, NC. The proposed work would consist of reconstruction of a 0.2 mile segment of Mainline Point Lookout Trial. The work includes the installation of four culverts, grading, reirifo`rcirig soil slope, aggregate base, drainage, and other miscellaneous work. Three of the culverts are strictly for drainage purposes (K 1 b, K 1 c, and K 1 d). Culvert K 1 a is located on an intermittent stream (Site 2, Station 21+28). Culvert Kla is an in-kind replacement to minimize impacts to the stream. The proposed culvert replacement will have 83 linear feet of impact, 713 square feet of permanent impact and 213 square feet of temporary impact. Enclosed please find two plan sets 11x17, five PCN copies, five GIS maps, five copies of all photos, and the $240 fee. This work indudes the installation of four culverts, grading, reinfo�cing of soil slope, aggregate base, drainage, and other miscellaneous work. . Soil distu�bing activities indude dearing and grubbing, slope and culvert excavatron, vnd roadway grading. The total disturbed area for the project is approximately 0.63 acres. The receiving water is Swannanoa Creek. Prohi6ited Discnarges Tlie following discharges are prohi6ited: Wash-water from concrete, paint, curing compounds, and other construction materials Fuels, oils, equipment-related compounds Soaps, so/vents used for vehide washing Waste, garbage, sanitary waste Inspect and maintain on a regular basis, a/1 mechanized equipment used in or near surface water to prevent contamination from fuels, lubricants, hydraulic fluids, or other toxic materials. Solid waste generated from the project will consist of construction debris, ga�bage, and empty containers. Collect and store all waste in dumpsters, or in metal or plastic drums, as appropriate. Hazardous waste will not be generated from normal construction activities. Equipment fueling and maintenance cou/d generate spills, leaks, and hazardous wastes like motor oil, diesel, gasoline, and battery fluid. If feasible, conduct these activities in a covered area to avoid contact with sto�m water. Store all haza�dous waste materia/s in appropriate and dearly marked containe�s away from other non- waste materials. Do not dispose of hazardous water materials rnto the on-site dumpsters. Dispose of material according to Federal, State, and local regulations. Develop and implement a Spill Prevention Control and Countermeasures (SPCC) plan following the requirements under 40 CFR 112. Report spi/ls large enough to discharge to surface waters to the National Response Center at 1-800-424-8802. General GuideJines The E'rosion & Sediment Control Narrative is meant as a guideline fo� preventing erosion and controlling sediment. The work consists of applying measures throughout the life of the p�oject to control erosion and to minimize the sedimentation of rivers, streams, and impoundments such as lakes, �eservoirs, bays, and coastal waters. The measures consist of soil erosion control measures which are also defined and outlined in the Standard Specifications for Construction of Roads and Bridges on Federal Highwvy Projects, FP-14, and the Special Contract Requirements. Do not modify the type, size, o� location of any control or practice without prior approval from the Contracting Office� (CO). No construction access will be permitted through a wet/and or wate�way. Do not �llow constructron vehides to track sediment outside the project limits. Do not allo�v any construction equipment to operate on or access the down-slope side of the perimeter cont�ol measures. Direct sto�m water to vegetated buffer areas and do not discharge directly into surface waters. Saquence of ConsYruction Phase I EstaGlish Perimeter Controls Prior to any clearing, gru6bing, or excavation, construct perimeter contro/s to ensure that disturbed sediment does not leave the project site. Perimeter controls inc/ude silt fence and other specified measures outside the construction /imits. Phase II Intermediate Controls Apply intermediate controls during rough grading operations. Install silt fence in areas surrounding the culverts as called out in the Erosion vnd Sediment Control plans. Install filter berms in ditches along the road wa y. Apply temporary tu�f establishment in disturbed areas that will remain exposed for o'ver 14 calendar dvys within 7 days. Appl/ permanent turf establishment to the finished slopes according to Section 625. At the end of each day's grading operations, shape earthwork to minimize and control erosion from storm runoff. Install in/et protection prior to diverting water through inlets. Upon completion of culverts, ensu�e that culvert entrances, outlets, and outlet channels are at final grade and are stabilized (with vegetation, riprap, o� pavement) before routin9 drainage through completed cul verts. Provide silt fence around all stockpiled excavated roadway mate�ial. Apply temporary turf establishment to stockpiles remaining in place longe� than 14 days within 7 days of stockpiling. Provide watering for dust control ��vithin the construction limits, on active haul roads, and in pits and sta9ing areas. Phase III Final Construction / Stabilization �fter completion of roadway construction, do the foUowing as directed by the CO: Finish grading, plvice riprap, and apply permanent turf establishment to any remaining disturbed areas. �+/here necessary, replace eroded topsoil and re-apply permanent tirrf establishment to distu�bed areas where vegetation has not established. Inspect, clean, and repair all culvert out/et protection, riprap basins, and stabilized channels. Remove all devices used for dewatering. Remove silt fence only after all upslope areas are stabilized and vegetation is well established. Remove all other perimeter controls when directed by tlie CO. Maintenance and Inspection Procedures Unless stated otherwise, construct and maintain all vegetated and structural erosion control practices according to Section 157, the details shown in the plans, and the individual permitting requirements. Check and maintain erosion control measures once every 7 days and within Z4 hours after a rain of 0.25 inches or more, and daily during wet weather. Repair or replace any damaged measures by the end of the day. Silt fence - inspect for buildup of excess sediment, undercutting, sags, and other failures. If the fabric � becomes damaged, repair or replace as necessary. Remove sediment from behind the silt fence when it becomes 0.5 fe.et deep at the fence. Filter berm - remove sediment at the base of the berm when it has reached'/3 of the exposed height of the tierm, or as directed by the CO. Rolled Erosion Control Product - unroll and lay matting out down slope face (in direction of flow). Anchor at top of slope. Do not stretch matting. For culvert inlets and ditches, insra/l matting along the bottom of the ditches first and then up the side slopes. Filter bag - check the filte� bags daily during dewatering operations for punctures, tears, or other damage, and for capacity.
