Attached for your review is a low impact bridge permit request for Bridge No. 45 on Mountain View Church Road in
Stokes County. I have attached for you Attachments G and L per the Low Impact Bridge program. I have also attached
permit drawings and "no survey required forms" from NCDOT Cultural Resources, also included is a wetland impact
summary, storm water management plan, and Section 7 Concurrence Request on the Northern-long eared bat.
A written authorization is requested from the USACE and DWR, with comments requested from WRC and
USFWS. Please let me know if you need anything else.
