The purpose of the inspection was to document if installed structural best management practices (BMPs) and other water quality features met the approved designs as required by the Surety Bond dated November 9, 2006 and to document maintenance issues. This the attached report contains comments on the preliminary inspection. For clarity, the current golf course hole numbers are referenced below. The following summary comments are provided for your consideration: 1. Golf Hole #1: a. No comments 2. Golf Hole #2: a. No comments 3. Golf Hole #3: a. No comments 4. Golf Hole #4: a. Construction Issue: The water level in Wetland 4-1 was lower than the permanent pool height shown in the approved plan. The dam was visibly leaking water from its base and must be repaired. b. Maintenance Issues: See attached report. 5. Golf Hole #5: a. Construction Issue: The outlet pipe could be located during the inspection, and the water level in Wetland 5-1 was lower than the permanent pool height shown in the approved plan. The pipe must be located (uncovered) and the water level issue resolved. b. Maintenance Issues: See attached report. Mr. Jack Blythe Inspection of Timberlake Golf Course BMPs Page 2 of 3 6. Golf Hole #6: a. No comments. 7. Golf Hole #7: a. No comments. 8. Golf Hole #8: a. No Comments. Please note this wet pond BMP is not required by the 401/404 wetland application / permit; however, Timberlake requests this BMP be installed as mitigation for two BMPs that will not be installed (5-2 and 5-3). CMSWS approves this as mitigation technique for these two pocket wetlands. 9. Golf Hole #9: a. Construction Issue: The spillway for Wetland 9-2 is eroding around the edges and needs to be repaired. b. Construction Issue: The elevation of the pond bottom, the inverts of the outlet pipe, the permanent pool height, and the depth of the wetland zones don’t match the approved design for Wetland 9-2 and must be corrected. c. Maintenance Issues: See attached report for Wet Pond 9-1 and Wetland 9-2 maintenance issues. 10. Golf Hole #10: a. No comments 11. Golf Hole #11: a. Construction Issue: The dam for Wetland 11-1 has been compromised via sink hole and is allowing water to leak through causing a lower water level than shown on the approved design. b. Maintenance Issues: See attached report. 12. Golf Hole #12: a. Maintenance Issues: See attached report for Wetland 12-1 and Wet Pond 12-2 maintenance issues. 13. Golf Hole #13: a. No comments. 14. Please be aware that CMSWS will not release the Surety Bond (dated November 9, 2006) until all the provisions have been completed by Timberlake. As part of the provisions of the bond, CMSWS will need to inspect and approve all water quality BMPs after receipt of final as-built surveys and engineering certification. CMSWS requests submittal of a revised final report by February 15, 2015. Timberlake should allow 2 weeks for CMSWS to complete the final inspection of the BMPs once the final report is received.
