NCDOT Greenville Const., 252 -830 -3495 Mr. Kincannon called the meeting to order and the following items were reviewed with discussion as noted: CONTRACT TIME AND I,I U[DATED DAMAGES: The date of availability for this contract is March 3. 2014. The completion date for this contract is the date that is One I lundred and Eighty Days (180) consecutive calendar days after completion oflntcrmediate Contract Time #1. The liquidated damages for this contract are Two Hundred Dollars ($200.00) per calendar day. INTERMEDIATE CONTRACT TIME 41: the date of availability for this intermediate contract time is March 3, 2014. The completion date of this intermediate contract time is the date which is One I lundred and Fifty (150) consecutive calendar days after beginning Step 2 in the Traffic Management Plans or by October 15. 2014, whichever is earlier. The liquidated damages for this intermediate contract time are Six Hundred Dollars ($600.00) per calendar day. The Contractor plans shut down the road on March 24. 2014, MAJOR CONTRACT ITEMS: The only major contract item for this contract is Lite Item 82 Prestressed Concrete Cored Slabs. SPECIALTY ITEMS: Specialty items for this contract consist of guardrail, signing, utility construction. erosion control, and reforestation. FUEL PRICE ADJUSTMENT: The base price index for Diesel #2 Fuel is $3.1244 per gallon which applies to asphalt line items as listed on Page 3 of the Contract. DBE GOALS: The DBE goal was met for this project. Mr. Kincannon referred to Page 12 and 13 of the contract regarding DBE must perform a commercially useful function and DBF replacement. Mr. Kincannon reminded the Contractor that in the event they need to replace a DBE to be sure to follow the guidelines as outlined in the contract beginning on Page 13. DOMESTIC STEEL: All steel and iron products that are permanently incorporated into this project shall be produced in the United States. Domestically produced high strength fasteners are required. 12 MONTH GUARANTEE: The Contractor shall guarantee materials and workmanship against latent and patent defects arising from faulty materials, faulty workmanship or negligence for a period of twelve months following the date of final acceptance of the work for maintenance and shall replace such defective materials and workmanship without cost to the Department. This guarantee provision shall be invoked only for major components of work in which the Contractor would be wholly responsible for under the terms of the contract. Examples would include pavement structures. bridges, and sign structures. This provision will not be used as a mechanism to force the Contractor to make repairs or perform additional work that the Department would normally compensate the Contractor for. Routine maintenance activities such as mowing grass, debris removal, etc. are not part of this guarantee. EROSION CONTROL: Reference was made to page 21 of the contract regarding the Contractor's responsibility for operations and subcontractors' operations to ensure that the Erosion and Sediment Control /Stormwater Pollution Prevention Plan is implemented and maintained over the life of the contract. The Contractor stated that Neal 'Thompson will be the Level lI person on this project. CLEARING AND GRUBBING: Clearing and grubbing will be by Method III as shown on Standard Drawing No. 200.03 of the 2012 Roa&kav Standard Drawings. BRIDGE APPROACII FILLS: Mr. Kincannon referred to Pages 30 and 31 of the contract regarding bridge approach fills and advised the Contractor to review and become familiar with these provisions. UTILITIES: According to I leather Causey, CenturyLink will have their work completed the week of March 17 "'. She stated CenturyLink is aware the road is closing on March 24`}'. Mr. Kincannon stated the waterline work would need to begin promptly when the road is closed and asked if there were any questions regarding this. Mr. Ward asked if all utilities would be bored. It was stated that the waterline will be bored but Mr. Kincannon stated CenturyLink will be overhead. Mr. Ward stated there is very little impact authorized and wanted to have a clear understanding of what vas being done in the area. Heather stated she would find out if CenturyLink is relocating any of the poles. ENVIRONMENTALLY SENSITIVE AREAS: "Phis project is located in an Environmentally Sensitive Area. The ESA is defined as a 50 -foot buffer zone on both sides of the stream or depression measured from top of streambank of center of depression. In areas identified as ESA. the Contractor may perform clearing operations but not grubbing operations until immediately prior to beginning grading operations as described in Article 200 -1 of the Standard Specifications. Erosion control devices shall be installed immediately following the clearing operations. Once grading operations begin in identified ESA. work shall progress in a continuous manner until complete. The Contractor was reminded the orange safety fence must be installed prior to any land disturbing activities. Mr. Kincannon advised the Contractor to have this area staked out and have our survey party check to be sure it is located in the appropriate areas before the fence is installed. The Contractor stated the East Group will be performing the surveying operation. Mr. Blankenship advised the Contractor to ensure the surveyor uses the permit drawings (wetland and buffer zone drawings) not the erosion control plans when staking the projects. STABILIZATION REOUIREMEN S: Reference was made to Page 50 of the contract regarding the stabilization requirements. Temporary or permanent ground cover stabilization shall occur within 7 calendar days from the last land- disturbing activity with the following exceptions in which temporary or permanent ground corer shall be provided in 14 calendar days from the last land - disturbing activity: Slopes between 2:1 and 3:1, with a slope length of 10 ft. or less Slopes 3:1 or flatter, with a slope of length of 50 lt. or less Slopes 4:1 or flatter PERMIT- Garcy Ward stated there is a large amount of wetland in the area with very little impact authorized. Mr. Ward discussed the following conditions of the 401 Water Quality Certification: Condition 2, 6, 7. 10. 11. 17, 18.. 19.20.21, 22. 2324.25, 26, 27. and 28. Mr. Ward also discussed some of the conditions of the Water Quality 3883 such as Conditions 3 (All sediment and erosion control devices shall be removed and the natural grade restored within two (2) months of the date that the Division of I:and Resources or local delegated program has released the specific area within the project), and Condition 4. Mr. Blankenship asked if the Contractor where would be their waste and borrow site and the Contractor stated they would be utilizing a commercial site. Also Andy asked who would be do the seeding and erosion control for the project and the Contractor advised it would be M & J Seeding. CRIMPING STRAW MULCH: Crimping shall be required on this project adjacent to any section of roadway where traffic is to be maintained or allowed during construction. In areas within six feet of the edge of the pavement straw is to be applied and then crimped. PRICF ADJUSTMENT ASPHALT BINDER FOR PLANT MIX: The base price index for asphalt binder for plant mix is $562.19 per ton. Final surface testing is not required. S. T. Wooten will be performing the paving operation for this project. SUBMITTALS: The Contractor has submitted the following the day of the preconstruction meeting: Crane Safety Bridge Demo Plan Steel Certification EEO Office Traffic Control Devices Project Personnel Date of Estimate Request Level II Personnel Authorized Signers for Supplemental Agreements Source of Materials Subcontractors Progress Schedule Heather stated she has also received the PDA and Concrete Mix Designs. Mr. Kincannon advised the Contractor they would need submittals for Crane Safety as well as a Bridge Demo Plan. These two submittals «ere subsequently been received by the RE Office on March 14, 2014. OTHER ITEMS DISCUSSED: Mr. Kincannon advised the Contractor that if they observe a lack of flow during high water to contact the RE Office. Ms. Causey advised that road closure is being scheduled for 8:30 am on March 24. 2014. The Contractor stated that Traffic Control Safety Services will be handling the signs and traffic control devices for the road closure. Mr. Kincannon reminded the Contractor that when the signs must be covered up until the actual road closure takes place. Mr. Kincannon noted that on Page 2 of the plans which shows the pavement schedule and C 1 as an I '/," surface course 9.5A this is to be changed to 1 '/2 Mr. Kincannon stated the surveyors need to tie into the existing. grade. Milton Rudd asked the Contractor who would be handling the barrier rail and the Contractor stated they would be using Watts. The guardrail will be installed by Reynolds. Mr. Rudd directed attention to Page 36 of the contract regarding the guardrail anchor unit. Also, Mr. Kincannon advised the Contractor they must use a pre- approved grout mix and testing will be done on this mix. NCDOT Lead Inspector — Travis Stain Contractor Superintendent Neal Thompson It was discussed if there would need to be a notice to shut off the water and Mr. Briley stated once the Contractor installs their valves he can cut the water off at the bridge and the water will be rerouted and citizens will not be cut off. STRUCTION CONFERENCE MEETING: The first Construction Conference Meeting will he scheduled at a later date. Meeting was adjourned.
