Sample points will be identified after planting and trees within each point will be field located (using a wooden stake or other means) and numbered for easy identification in future monitoring visits. During monitoring visits each of the 10 trees within each sample point will be evaluated for survivability. A photo record of site observations will also be prepared for established photo points. The overall site survival rate and number of surviving planted stems per acre will be calculated by averaging the survival rate of all of the sample points. The site will be deemed successful if the average survival rate of planted stems within the sample points is equal to or greater than the required 320 stems per acre at the end of the five year monitoring period and success is confirmed by DWQ. Monitoring frequency will be annually for the required five (5) year duration or until DWQ has determined success of the site. Annual reports will include quantitative and qualitative monitoring results, a map showing the location of vegetation sample points and photo points, and a map showing any problem areas. Our report will also include recommendations for supplemental stabilization and planting as needed. The annual monitoring report will be submitted to DWQ by December 31" of each monitoring year. The Permittee will attempt to locate a land trust, stewardship fund or other qualified easement holder to determine if they will accept a conservation easement on the restoration areas. If so, the conservation easement will be conveyed to the land trust or other qualified holder after five (5) years of monitoring and confirmation of success by DWQ. If not, the owner will cause a perpetual deed restriction to be placed on the restoration areas and have it recorded with the county register of deeds. Limitations Our evaluations, conclusions, and recommendations are based on project and site information available to us at the time of this report and may require modification if there are any changes in the project or site conditions, or if additional data about the project or site becomes available in the future.
