Please reference the original minor modification request dated February 24 20015
concerning proposed reconstruction and minor channel re alignment of a previous stream
relocation The hillside adjacent to the stream experienced a slope failure necessitating the
proposed work It has come to our attention that our February 24 2005 re uest did not contain
the required accommodations for _paytnent of the modification fee
The NCDOT asks that processing of this modification proceed as soon as is feasible, so
that the slope failure can be corrected and the site restored as soon as possible
