On April 14, 2010, Sue Homewood and Ron Boone, staff from the Division of Water Quality Winston-Salem
Regional Office, conducted a site visit on your property at the above noted address. The purpose of the site visit was
to determine compliance with the 401 Water Quality Certification issued to you on July 3, 2008 for the purpose of
constructing a pond at this property. At the time of the inspection, your pond was deemed to be compliant with the
requirements of the Certification and we did not note any negative impacts to the stream channel above your property
line.
