Section 401 Individual Water Quality Certification, issued 3/4/2009, NCDENR-DWQ Water Quality Certification No. 3782, NCDWQ Project No. 20081797 Notification of Unauthorized Activity/Permit Noncompliance, issued 1/20/2010, USACE Action ID: 2009-00032 Debit $570.00 from WBS Element 35007.1.1. The North Carolina Department of Transportation (NCDOT) proposes to extend SR 4126 (Bridford Pkwy, new route) from SR 1541 (Wendover Ave.) at Hornaday Rd. to Burnt Poplar Rd. at Swing Rd. in Greensboro, Guilford County, North Carolina. The proposed 1.1 mi. extension consists of a four-lane curb and gutter section with a raised grass median, 12-ft. inside lanes, and 14-ft. outside lanes, as well as the realignment of Big Tree Way with Bridford Pkwy and structures over I-40. The purpose of this submittal is to request a permit modification to the Section 404 permit and Section 401 Water Quality Certification for U-4006. Please see the enclosed copies of the revised permit drawing sheets, and revised summary table. The revised design does not compromise NCDOT's compliance with the existing permit conditions. Protected Species • Aquatic Life passage • FEMA compliance • Cultural Resources Changes in impacts are due to culvert alignment at Site 1, UT to South Buffalo Creek (see enclosed revised permit drawings sheets 15 of 15 and 4a of 15). NCDOT was permitted eight feet of bank stabilization at the culvert outlet only. Impacts to this site have increased to include an additional 30 feet of bank stabilization at the culvert inlet and 20 feet of bank stabilization at the culvert outlet. Mitigation for the additional 50 feet of bank stabilization is not required from the USACE or NCDWQ. NCDOT will further monitor. culvert placement for natural sedimentation fill-in and possible head cutting.
