Please find attached the Certificate of Completion (Attachment 1) for the emergency
construction of two stream crossings associated with a gravel road near Four Oaks NC in
Johnston County (DWQ Project No: 03-0752). The crossings are located at tributaries to White
Oak Branch and were necessary to recover an F-15E jet.
This action was completed in December 2003. The original smaller culverts had to be
replaced with larger (24 inch diameter, 12 feet long) culverts. A memorandum for records
addressing this replacement is attached (Attachment 2).

SUBJECT: F-15 E Crash Site in Johnston County and Compliance with
Nationwide Permit # 14 (Action ID 200320939) and Section 401
Certification (DWQ Project No. 03-0752)
DETAILS:
Two stream crossings were necessary to provide emergency access to the F-
15E crash site in Johnston County (North Carolina). In order to provide quick
access for recovery of the jet, readily available culverts were installed during the
emergency construction phase in June of 2003. These were later determined to
be too small to facilitate the water flow in the two streams.
In December of 2003 the original smaller culverts were replaced with 24-inch
corrugated pipe culverts.
Stream crossing 1 was provided with two 24-inch corrugated pipe culverts (figure
1 and 2) and stream crossing 2 was provided one 24-inch corrugated pipe culvert
(figure 3 and 4).
A cross section drawing of the culverts and road crossing is attached.
Environmental Office personnel conducted a final inspection on 4 February 2004
and verified the free water flow through the installed culverts.

Protection Rules (15A NCAC 2B.0233)
Dear Mr. Lee:
You have our approval, in accordance with the attached conditions, to place fill within or otherwise impact 24 feet of
perennial streams and approximately 2,400 square feet (sf)(0.06 acres) of protected riparian buffers for the purpose
improving roads at the subject property, as described within your application dated June 9, 2003. After reviewing your
application, we have decided that the impacts are covered by the N.C. Division of Water Quality (DWQ) General Water
Quality Certification Number(s) 3404 (GC3404). The Certification(s) allows you to use Nationwide Permit(s) 14 when
issued by the US Army Corps of Engineers (USACE). This letter shall also act as your approved Authorization
Certificate for impacts to the protected riparian buffers per 15A NCAC 26 .0233. In addition, you should obtain or
otherwise comply with any other required federal, state or local permits before you go ahead with your project including
(but not limited to) Erosion and Sediment Control and non-discharge regulations. Also, this approval will expire when
the accompanying 404 permit expires unless otherwise specified in the General Certification.
This approval is only valid for the purpose and design that you described in your application. If you change your project,
you must notify us and you may be required to send us a new application. If the property is sold, the new owner must
be given a copy of this Certification and approval letter and is thereby responsible for complying with all conditions. If
total fills for this project (now or in the future) exceed one acre of wetland or 150 linear feet of stream, compensatory
mitigation may be required as described in 15A NCAC 2H .0506 (h). For this approval to be valid, you must follow the
conditions listed in the attached certification.
Violations of any condition herein set forth may result in revocation of this Certification and may result in criminal and/or
civil penalties. This Certification shall become null and void unless the above conditions are made conditions of the
Federal 404 and/or coastal Area Management Act Permit. This Certification shall expire upon expiration of the 404 or
CAMA permit.
If you do not accept any of the conditions of this Certification (associated with the approved wetland or stream impacts),
you may ask for an adjudicatory hearing. You must act within 60 days of the date that you receive this letter. To ask for
a hearing, send a written petition, which conforms to Chapter 150B of the North Carolina General Statutes to the Office
of Administrative Hearings, P.O. Box 27447, Raleigh, N.C. 27611-7447. This certification and its conditions are final
and binding unless you ask for a hearing.
Any disputes over determinations regarding this Authorization Certificate (associated with the approved buffer impacts)
shall be referred in writing to the Director for a decision. The Director's decision is subject to review as provided in
Articles 3 and 4 of G.S. 150B.

On June 19, 2003 the Division of Water Quality (DWQ) was notified by receipt of your application
regarding your plan to fill waters for the purpose of constructing a road crossing at the proposed F-15E crash site
in Johnston County. Approval from DWQ is required to disturb these areas. Please provide 7 copies of the
following information and refer to the DWQ # (03-0752) listed above in your reply. Please show these on maps
of suitable scale (for instance 1" = 100 feet) so we can begin to determine your project's compliance with 15A
NCAC 2H.0500.
After reviewing the PCN it is the staff's understanding that the crossing is needed for temporary
access. If this is accurate, please detail your plans to remove the road crossing and re-establish the
streamside 50' foot wide buffer. (Note: satisfactory riparian zone restoration can be accomplished by
the implementation of restoration where at least two native tree species are planted at a density
sufficient to provide 320 trees per acre at maturity. This density can be achieved by planting
approximately 436 (10x10 feet spacing) to 681 (8x8 feet spacing) trees per acre. Your plan
must include the types of native woody vegetation selected, methodology of planting, and
site map indicating the location of the plantings how you plan to stabilize disturbed
streamside areas. )
2. Conversely, if the road crossing is intended to be permanent then please provide plans that depict the
crossing and how stormwater will be diverted to diffuse flow through the 50' wide buffer.

III. Project Information
Attach a vicinity map clearly showing the location of the property with respect to local
landmarks such as towns, rivers, and roads. Also provide a detailed site plan showing property
boundaries and development plans in relation to surrounding properties. Both the vicinity map
and site plan must include a scale and north arrow. The specific footprints of all buildings,
impervious surfaces, or other facilities must be included. If possible, the maps and plans should
include the appropriate USGS Topographic Quad Map and NRCS Soil Survey with the property
boundaries outlined. Plan drawings, or other maps may be included at the applicant's discretion,
so long as the property is clearly defined. For administrative and distribution purposes, the
USACE requires information to be submitted on sheets no larger than 11 by 17-inch format;
however, DWQ may accept paperwork of any size. DWQ prefers full-size construction
drawings rather than a sequential sheet version of the full-size plans. If full-size plans are

9. Describe the existing conditions on the site and general land use in the vicinity of the project
at the time of this application: The land use in the vicinity is general agriculture, wheat
fields and pastures Some wooded areas occur. The current unimproved road is an
aizricultural dirt road used by the farmer to access his fields. It crosses two small streams

using unimproved stream crossing methods (mud crossings). The first (when accessing the
site from White Oak Road) is an unnamed tributary to the White Oaks Branch (blue-lined on
topomap), the second is also an unnamed tributary to White Oaks Branch, but does not
appear on the topomap.
10. Describe the overall project in detail, including the type of equipment to be used: The first
stream crossing will consist of two 6" PVC pipes (12' width) and gravel; the second crossing
will consist of one 18" corrugated black plastic pipe (12' width) and gravel. Type of
equipment used: dumptruck, backhoe, gravel, PVC pipe, corrugated pipe.
11. Explain the purpose of the proposed work: The purpose of this work is to provide safe
access for heavy equipment, trucks and cranes to recover the F-15E wreckage in a nearby
wooded area.
IV. Prior Project History
If jurisdictional determinations and/or permits have been requested and/or obtained for this
project (including all prior phases of the same subdivision) in the past, please explain. Include

VI. Proposed Impacts to Waters of the United States/Waters of the State
It is the applicant's (or agent's) responsibility to determine, delineate and map all impacts to
wetlands, open water, and stream channels associated with the project. The applicant must also
provide justification for these impacts in Section VII below. All proposed impacts, permanent
and temporary, must be listed herein, and must be clearly identifiable on an accompanying site
plan. All wetlands and waters, and all streams (intermittent and perennial) must be shown on a
delineation map, whether or not impacts are proposed to these systems. Wetland and stream
evaluation and delineation forms should be included as appropriate. Photographs may be

VII. Impact Justification (Avoidance and Minimization)
Specifically describe measures taken to avoid the proposed impacts. It may be useful to provide
information related to site constraints such as topography, building ordinances, accessibility, and
financial viability of the project. The applicant may attach drawings of alternative, lower-impact
site layouts, and explain why these design options were not feasible. Also discuss how impacts
were minimized once the desired site plan was developed. If applicable, discuss construction
techniques to be followed during construction to reduce impacts.
The crash site cannot be accessed from anv other direction. The area to the west of the crash site
is heavily wooded, the terrain is steeper and pre-existing infrastructure (agricultural roads) is not
present. Crossing the two streams cannot be avoided.

VIII. Mitigation
DWQ - In accordance with 15A NCAC 2H .0500, mitigation may be required by the NC
Division of Water Quality for projects involving greater than or equal to one acre of impacts to
freshwater wetlands or greater than or equal to 150 linear feet of total impacts to perennial
streams.
USACE - In accordance with the Final Notice of Issuance and Modification of Nationwide
Permits, published in the Federal Register on March 9, 2000, mitigation will be required when
necessary to ensure that adverse effects to the aquatic environment are minimal. Factors
including size and type of proposed impact and function and relative value of the impacted
aquatic resource will be considered in determining acceptability of appropriate and practicable
mitigation as proposed. Examples of mitigation that may be appropriate and practicable include,
but are not limited to: reducing the size of the project; establishing and maintaining wetland
and/or upland vegetated buffers to protect open waters such as streams; and replacing losses of
aquatic resource functions and values by creating, restoring, enhancing, or preserving similar
functions and values, preferable in the same watershed.
If mitigation is required for this project, a copy of the mitigation plan must be attached in order
for USACE or DWQ to consider the application complete for processing. Any application
lacking a required mitigation plan or NCWRP concurrence shall be placed on hold as
incomplete. An applicant may also choose to review the current guidelines for stream restoration
in DWQ's Draft Technical Guide for Stream Work in North Carolina, available at
http://h2o.enr.state.nc.us/ncwetlands/strmgide.html.

at the time of this application: The land use in the vicinity is general agriculture, wheat
fields and pastures. Some wooded areas occur. The current unimproved road is an
agricultural dirt road used by the farmer to access his fields. It crosses two small streams

using unimproved stream crossing methods (mud crossings). The first (when accessing the
site from White Oak Road) is an unnamed tributary to the White Oaks Branch (blue-lined on
topomap) the second is also an unnamed tributary to White Oaks Branch, but does not
appear on the topomap
10. Describe the overall project in detail, including the type of equipment to be used: The first
stream crossing will consist of two 6" PVC pipes (12' width) and gravel; the second crossing
will consist of one 18" corrugated black plastic pipe (12' width) and gravel. Type of
equipment used: dumptruck, backhoe, gravel, PVC pipe, corrugated pipe.
11. Explain the purpose of the proposed work: The purpose of this work is to provide safe
access for heavy equipment, trucks and cranes to recover the F-15E wreckage in a nearby
wooded area.

VII. Impact Justification (Avoidance and Minimization)
Specifically describe measures taken to avoid the proposed impacts. It may be useful to provide
information related to site constraints such as topography, building ordinances, accessibility, and
financial viability of the project. The applicant may attach drawings of alternative, lower-impact
site layouts, and explain why these design options were not feasible. Also discuss how impacts

The crash site cannot be accessed from any other direction. The area to the west of the crash site
is heavily wooded, the terrain is steeper and pre-existing infrastructure (agricultural roads) is not
present. Crossing the two streams cannot be avoided.

VIII. Mitigation
DWQ - In accordance with 15A NCAC 2H .0500, mitigation may be required by the NC
Division of Water Quality for projects involving greater than or equal to one acre of impacts to
freshwater wetlands or greater than or equal to 150 linear feet of total impacts to perennial
streams.
USACE - In accordance with the Final Notice of Issuance and Modification of Nationwide
Permits, published in the Federal Register on March 9, 2000, mitigation will be required when
necessary to ensure that adverse effects to the aquatic environment are minimal. Factors
including size and type of proposed impact and function and relative value of the impacted
aquatic resource will be considered in determining acceptability of appropriate and practicable
mitigation as proposed. Examples of mitigation that may be appropriate and practicable include,
but are not limited to: reducing the size of the project; establishing and maintaining wetland
and/or upland vegetated buffers to protect open waters such as streams; and replacing losses of
aquatic resource functions and values by creating, restoring, enhancing, or preserving similar
functions and values, preferable in the same watershed.
If mitigation is required for this project, a copy of the mitigation plan must be attached in order
for USACE or DWQ to consider the application complete for processing. Any application
lacking a required mitigation plan or NCWRP concurrence shall be placed on hold as

agricultural dirt road which was used for access to the F-15E crash site.
Two stream crossings were constructed during the emergency improvement
of this road. No buffer zone existed prior to improvements.

Please find attached seven copies of the Pre-Construction Application (PCN) Form for the
emergency construction of two stream crossings associated with a gravel road near Four Oaks
NC in Johnston County. This gravel road provides safe access for recovery of the F-15E
wreckage from the wooded crash site.
We are simultaneously coordinating the Section 404 permit (Nationwide Permit 14) with the
US Army Corps of Engineers Raleigh Regulatory Field Office.
The required check in the amount of $200.00 for the Section 401 water quality certification is
attached to the front of this package.
