A completed copy of the form is attached. The purpose of this letter is to briefly summarize the findings of the receptor survey. Geoscience conducted a walking survey of the area surrounding the subject site on July 8, 2014. All properties within approximately 500 feet of the subject property were observed from adjacent public roads or sidewalks. Upon completion of the walking survey, the properties between 500 and 1,000 feet from the subject property were observed during a vehicular reconnaissance. Geoscience also reviewed information available on the Mecklenburg County GIS website. The following findings were made as a result of the receptor survey: 1) No basements were noted within 500 feet of the site. 2) No schools or day care centers were noted within 1,000 feet of the site. Although the Mecklenburg County GIS website identifies the Academic Illumination Day School as being located approximately 825 feet west of the site (see Figure No. CH14.045-A), that location is not current. It is our understanding that the school has moved to 1106 Catawba Avenue, approximately 2,000 feet west of the site. 3) A storm sewer line extends across the eastern portion of the site. The approximate location of this line is depicted on Figure CH14.045-B. 4) A sanitary sewer line extends along Zion Avenue adjacent to the subject site. The approximate location of this service is depicted on Figure CH14.045-B. 5) A water main is present along the western side of Zion Avenue in close proximity to the property. The locations of nearby water lines are depicted on Figure CH14.045-B. 6) A natural gas line is present along Zion Avenue.
