SITE DESCRIPTION: Type of Petroleum Hydrocarbon Suspected (Gasoline, Diesel, Kerosene, etc.): Diesel, Gasoline and Fuel Oil Hazard: Hydrocarbon fumes, skin irritation, explosion, petroleum fumes. Area Affected: Former Gas Station and Dry Cleaner location; prior assessments at this site by S&ME and another consultant in 2005-2007 indicated low levels of petroleum hydrocarbons in one soil sample. Security: Coordinated during site work by the S&ME site supervisor. Weather Conditions: Anticipate 40-50 degrees F, no precipitation B. ENTRANCE OBJECTIVES: The objectives of entry to the site are to examine possible impacts to soil and groundwater of gasoline or diesel fuel constituents at the former gasoline station UST and dispenser locations. C. ON-SITE ORGANIZATION AND COORDINATION: The following personnel are designated to carry out the stated job function on-site (Note: One person may carry out more than one job function.) All activities on the site must be cleared through the site manager. D. ON-SITE CONTROL: 1. Roger Smith has been designated to coordinate access control and security on-site. A safe perimeter will be established at limits defined by the Site Safety Officer on location. Unauthorized personnel are not allowed within this area. 2. The on-site Command Post (i.e., the pickup truck containing respirators) and staging area will be established by the Site Safety Officer dependent upon site conditions. 3. The location of the Command Post will be established upwind or distant location (20 feet) from the Exclusion Zone. 4. Control boundaries will be established, and the Exclusion Zone, Hot Line, Contamination Reduction Zone, and Support Zone will be established by the site safety officer as site conditions dictate. November 28, 2012 GENERAL HEALTH AND SAFETY PLAN Page 2 E. HAZARD EVALUATION: The following substance(s) are known or suspected to be on-site. The primary hazards of each substance is identified. FUEL OIL PEL/TLV: 5 mg/m3 as mist ACGIH TLV-TWA: 100 mg/m3, skin, A3 IDLH: N/A Symptoms/Effects of Acute Exposure: Excessive exposure may cause irritation to the nose, throat, lungs, and respiratory tract. Central nervous system (brain) effects may include headache, dizziness, loss of balance and coordination, unconsciousness, coma, respiratory failure, and death. NAPHTHALENE PEL/TLV: 10 ppm IDLH: 500 ppm Symptoms/Effects of Acute Exposure: Eye irritation, confusion, malaise, nausea, vomiting, abdominal pain, profuse sweating, hematuria, hemoglobinuria, renal shutdown, dermatitis. Kerosene/Diesel PEL/TLV: not established IDLH: N/A Symptoms/Effects of Acute Exposure: Eye irritation, nose respiratory system irritation, dizziness, nausea, and fatigue. Respiratory protection not normally required. GASOLINE PEL/TLV: 300 ppm IDLH: N/A Symptoms/Effects of Acute Exposure: Irritated eyes, nose respiratory system, headache, nausea, fatigue. The following additional hazards are expected on-site: Possible on-looker curiosity. Care should be exercised to maintain Exclusion Zone. If on-lookers do not observe the Site Manager's efforts to respect the Exclusion Zone, on-site activities involving contaminants should be suspended and the S&ME Project Manager and client contact notified. F. DETECTION OF NON- LISTED COMPOUNDS OR 5 PPM EXPOSURE TO ORGANIC VAPORS DETECTED BY OVA WILL REQUIRE November 28, 2012 GENERAL HEALTH AND SAFETY PLAN Page 3 LEVEL C RESPIRATORY PROTECTION. When conditions warrant the Site Safety Officer or field team leader will upgrade Level of Protection to the appropriate level for the protection of on-site and off-site health and safety. OPERATIONS WILL CEASE IMMEDIATELY. PERSONNEL WILL LEAVE THE AREA IMMEDIATELY AND MEET AT THE DESIGNATED LOCATION. SUBSEQUENT TO INCREASED LEVEL OF PROTECTION IMPLEMENTATION, SITE PERSONNEL WILL ALLEVIATE ANY THREAT TO THE NON-EXCLUSION ZONES AS DIRECTED BY THE SITE MANAGER. OPERATIONS OTHER THAN THOSE REQUIRED TO PROTECT HUMAN HEALTH AND SAFETY WILL NOT PROCEED WITHOUT THE AUTHORIZATION OF THE SITE MANAGER. Specific protective equipment for each level of protection is as follows: Level C: Full-face respirator Impermeable boots with steel toe and shank Hard hat Sol-Vex outer gloves/latex inner gloves. Level D: Sol-Vex gloves Steel toe and shank boots Hard hat Safety Glasses Stages of personal decontamination for each level are as follows: Level C: 1. Alconox and potable water scrub for gloves, boots and hard hats. 2. Potable water rinse for gloves, boots and hard hats. 3. Remove Tyveks as needed and place in trash bags. 4. Store trash bags on-site. Level D: 1. Wash gloves and boots as needed with potable water and soap. 2. Remove gloves and place in plastic trash bags. If air-purifying respirators are authorized, GMC-S is the appropriate cartridge for use with the MSA respirators provided given the involved substances and concentrations anticipated. A competent individual has determined that all criteria for using this type of respiratory protection have been met. NO CHANGES TO THE SPECIFIED LEVELS OF PROTECTION SHALL BE MADE WITHOUT THE APPROVAL OF THE SITE SAFETY OFFICER AND THE SITE MANAGER G. ON-SITE WORK PLAN: 1. Go over Health and Safety Plan and look for overhead utilities and possible underground utilities 2. Perform hand-auger borings in the three areas of concern as stated in Paragraph B of this plan. The drilling areas have previously been cleared of underground utilities by Priority Underground Locating. November 28, 2012 GENERAL HEALTH AND SAFETY PLAN Page 4 H. COMMUNICATION PROCEDURES: The following standard hand signals will be used if vocal communication is not possible: Hand gripping throat - I can't breathe. Grip partners' wrist or both hands around waist - Leave area immediately Hands on top of head - Need assistance. Thumbs up - OK, I'm all right; I understand Thumbs down - No, negative. I. DECONTAMINATION PROCEDURES: Personnel and equipment leaving the Exclusion Zone shall be thoroughly decontaminated as required. The standard Level D decontamination protocol shall be used with the following decontamination stations: All procedures will be followed, unless site conditions warrant otherwise. Upgraded decontamination procedures must be approved by Site Manager. Decontamination procedures are outlined above. J. SITE AND SAFETY PLAN: 1. Roger Smith is the designated Site Safety Officer and is directly responsible to the Site Manager for safety recommendations on site. 2. Environmental Monitoring: The following environmental monitoring instruments shall be used on site at the specified intervals: Organic Vapor Meters - continuous, 15-minute-interval, or as needed monitoring of breathing air space, monitor soil cuttings, and samples. 3. Emergency Procedures: The following standard emergency procedures will be used by on-site personnel. The Department Manager (Mike Cashio) and Regional Safety Manager (Joe Weatherford) shall be notified of any on-site emergencies and the Site Safety Officer shall be responsible for ensuring that the appropriate procedures are followed. 3A Emergency Escape Route: the most accessible escape route should be taken to obtain a clear safe distance from the source of the emergency. 3B Local Emergency Response Assistance: 911. Personnel Injuries in the Exclusion Zone: Upon notification of an injury in the Exclusion Zone the Site Safety Officer shall evaluate the nature of the emergency and determine appropriate action. Decontamination will be based upon the nature of the injury and the condition of the injured person. Fire/Explosion: Upon notification of fire or explosion, on-site, operations shall terminate immediately, the fire department shall be alerted, and all personnel moved to a safe distance from the involved area. Personal Protective Equipment Failure: If any site worker experiences failure or alteration of protective equipment that affects the protection factor November 28, 2012 GENERAL HEALTH AND SAFETY PLAN Page 5 that person and his/her buddy shall immediately leave the Exclusion Zone. Re-entry shall not be permitted until the equipment has been repaired or replaced. Other Equipment Failure: If any other equipment on-site fails to operate properly, the Site Manager and Site Safety Officer shall be notified and then determine the effect of this failure on continuing operation on-site. If the failure affects the safety of personnel or prevents completion of the work plan tasks, all personnel shall leave the Exclusion Zone until the situation is evaluated and appropriate actions are taken.
