(EI) has completed a Phase II Environmental Site Assessment (Phase II ESA) of the commercial property located at 3400 Yonkers Road in Raleigh (Wake County), North Carolina. On February 17, 2013 EI mobilized to the property along with Troxler Geologic Services, a licensed direct push technology/drilling contractor to implement the soil boring program for the soil and groundwater quality assessment. This report includes a brief summary of the scope of work conducted, findings, and conclusions. Figure 1 shows the location of the site. Figure 2 contains an aerial photograph of the site. Table 1 and Table 2 summarizes the laboratory analytical results for the soil and groundwater assessment of the subject property, respectively. Soil boring logs and well construction records are presented in Appendix A. The laboratory analytical report is presented in Appendix B. The following report is provided for the sole use of Capital Bank and John S. Carbone, LLC or their affiliates, agents, or designees with a security interest on the subject property for the project for which it was prepared. Use of this report by any third parties will be at the party’s sole risk, and EI disclaims any liability for any such use or reliance by third parties. 1.1 Prior Assessments EI performed a Phase I Environmental Site Assessment (Phase I ESA) at the Subject Property in February 2013. According to the February 2013 Phase I ESA, Kimshe, LLC has owned the property since 2005. The subject property was improved with one light industrial building and is occupied by Hughes Kitchen and Bath Collection, and has been since approximately 2005. Prior to 2005, the subject property was occupied by a light industrial facility from the time of its development in 1965. Prior to 1965 and back to 1949, the subject property existed as undeveloped land. The Phase I ESA identified one recognized environmental condition in connection with the Subject Property:  One REC associated with the long term industrial/light industrial use of the subject property, including unknown nature of all former onsite electronic laboratory operations. On March 14, 2013, EI conducted a Limited Phase II Environmental Site Assessment (Ltd. Phase II ESA) to determine whether the Subject Property was impacted from prior usage and onsite electronic laboratory operations. The Ltd. Phase II ESA was conducted under the supervision of Mr. Darren R. Lockhart, Professional Geologist with EI. Specifically, five (5) soil borings (SB-1 through SB-5) were advanced at presumably downgradient locations adjacent to the rear of the site building and east of the building for continuous soil screening and sampling (refer to Figure 3). There were no olfactory, visible and instrumental indications of impacts observed in the soil cores down to groundwater. As there were no indications of impacts (i.e., visual, olfactory, and instrumental) in the soil cores at shallow depths, soil samples were collected in unsaturated soils just above the water table. SB-1, SB-4 and SB-5 were advanced to intersect the water table and install temporary monitoring wells (TWs) for groundwater sampling. The boreholes were converted to temporary wells by inserting a clean 1-inch slotted PVC column across the water table. All borings were properly plugged and abandoned following field activities. The March 2013 assessment disclosed the presence of two PAH compounds in subsurface soils at one location; however, the reported concentrations were at low levels and did not exceed the respective Soil-to-Groundwater MSCCs. Based on the location and depth of the sample and the lack of field indications, the reported presence of these compounds were attributed to borehole cave-in from the asphalt surface and road base material; and such, not regarded as a concern. Additionally, the March 2013 assessment disclosed the presence of TCE in two of the three groundwater samples (SB-4 TW and SB-5 TW) with one of the sample SB-4 TW containing TCE at 9.8 µg/L, which exceeded the NC 2L groundwater quality standard of 3.0 µg/L. Refer to Figure 4. As this exceedance is reportable to the NCDENR, DWM, Inactive Hazardous Sites Branch (IHSB), it was noted that upon review of this information, further assessment may be required to determine the source(s), nature and extent of contamination at the subject property; and corrective action may be necessary to bring the site into compliance with applicable State rules. In December 2013, EI was engaged to re-assess the Subject Property via a Phase I ESA subsequent to acquisition of the Subject Property by Wells Fargo via foreclosure. Reportedly, there were no noteworthy differences in site conditions, usage and occupancy, except that the March 2013 Ltd. Phase II discovered a NC 2L violation due to TCE groundwater impacts. As such, further assessment was recommended to re-evaluate groundwater quality. 1.2 Purpose This Phase II ESA was performed to re-evaluate the groundwater impacts previously identified and to determine the extent of those impacts. In addition, the Phase II ESA would include groundwater mapping and a survey of the surrounding vicinity for potential receptors and sensitive environments in order to evaluate their potential to be affected by environmental impacts originating from the Subject Property. 1.3 Scope of Work To meet the objectives of the Limited Phase II ESA, EI implemented a soil and groundwater sampling and analysis program using Direct Push Technology (DPT) and solid-stem rotary auger methods for monitoring well installation. This report describes the investigation and represents a reasonable effort to quantify these potential environmental concerns. Limited Phase II Environmental Assessment 3400 Yonkers Road-Kimshe 3400 Yonkers Road, Raleigh, North Carolina EI Project ENMO130235.01 3 Specifically, EI followed the proposed scope-of-work, which included:  Underground utility location  Completion of ten (10) soil borings advanced to desired depth or groundwater using DPT methods for sampling and analysis;  Installation of seven (7) Type-II, 2-inch permanent groundwater monitoring wells to assess groundwater quality;  Horizontal and vertical survey of the monitoring wells for mapping of groundwater flow;  Containerizing and preserving selected samples via laboratory-prepared containers and standard chain-of-custody for laboratory analysis;  Lab analysis of samples, ten (10) soil and seven (7) groundwater, for volatile organic compounds (VOCs);  Survey of surrounding vicinity for potential receptors and sensitive environments;  Preparation of a final report that documents the field activities and presents a summary of findings along with our conclusions and recommendations. Limited Phase II Environmental Assessment 3400 Yonkers Road-Kimshe 3400 Yonkers Road, Raleigh, North Carolina EI Project ENMO130235.01 4 2.0 SITE CHARACTERISTICS 2.1 Site Location The subject property is located in Raleigh, Wake County, North Carolina and is addressed as 3400 Yonkers Road in the Wake County records. The subject property is located on the south side of Yonkers Road and I-440, approximately 1,300 feet northwest of the Yonkers Road/I-440 and New Bern Avenue intersection. The subject property is contained by one irregularly-shaped, 5.68-acre parcel of land referred to by parcel identification number (PIN) 1724252035 in the Wake County property records. The subject property is partially described as “being all of Lot # 1 as shown on that certain plat entitled ‘Bras D’Or Subdivision Plat, Prepared for Bras D’Or Nominee Realty Trust, City of Raleigh, Wake County, NC’ dated October, 2004…” The subject property is better described in Deed Book 11703, Page 2299 at the Wake County Register of Deeds office in Raleigh. 2.2 Site Description The current owner of the subject property parcel is recorded as Wells Fargo Bank, N.A. which appears to have owned the subject property since January 10, 2014. The subject property is occupied by Hughes Kitchen and Bath Collection (showroom) and Hughes Supply (equipment sales), and has been since approximately 2005. The subject property is developed with one light industrial building, reportedly completed in 1965. The brick and concrete block building reportedly contains 68,109 square feet of floor space, which is divided into two tenant spaces. The building contains a showroom, office, hardware sales, and distribution storage warehouse spaces in the southern occupied portion, and office, storage, and former laboratory spaces in the northern vacant portion. The remainder of the subject property consists of asphalt-paved areas in the north, south, and west portions of the site, grassed areas in the northeast and southeast portions, and undeveloped wooded area in the west corner. Electricity is provided to the subject property by Progress Energy. Water and sanitary sewer services are provided to the subject property by the City of Raleigh. The subject building is not connected to natural gas. Limited Phase II Environmental Assessment 3400 Yonkers Road-Kimshe 3400 Yonkers Road, Raleigh, North Carolina EI Project ENMO130235.01 5 3.0 FIELD ACTIVITIES The Phase II field activities were conducted from February 17 to 19, 2014. Specifically, the scope of work was implemented under the supervision of Mr. Darren R. Lockhart, Professional Geologist with EI along with Troxler Geologic Services, a licensed drilling contractor. Prior to advancing the soil borings and monitoring well locations, EI conducted a site walkover to identify the areas of concern for the Limited Phase II ESA and to verify that the underground utilities had been marked. No conflicts were observed. The soil boring and monitoring well locations of the February 2014 Phase II are shown on Figure 5. 3.1 Monitoring Well Installation Seven boring locations were selected for permanent monitoring well installation (MW-1 through MW-7) based on data obtained in the March 2013 Phase II in order to re-evaluate groundwater quality at the former SB-4 TW location, at one upgradient location, and at downgradient locations near the property boundary (Figure 5). The well boreholes were advanced using solid stem rotary-auger drilling methods. Each well borehole was converted into a Type-II monitoring well by installing a 2-inch diameter PVC casing with a 0.010-inch slotted bottom section (15-foot well screen) down to depths ranging from 23 to 28 feet below ground surface. The annular space of each well borehole was filled with a sand pack up to 2.0 feet above the well screen, and then completed to the surface with a bentonite plug and cement grout. The wellheads were completed with a bolted, steel manhole cover/lid with a locking well cap for the well casing. Table 1 summarizes the well construction information. A copy of the driller’s well construction records and boring log are presented in Appendix A. 3.2 Monitoring Well Elevation Survey Following monitoring well installation, EI conducted a level survey to measure the relative elevations differences between MW-1 through MW-7 in order to map groundwater flow across the Subject Property. MW-7 was assigned an arbitrary benchmark elevation of 100 feet above mean sea level (msl) for reference. The survey was conducted using a survey level instrument on a tripod, and a telescoping stadia rod. Each foresight measurement was taken on the top-of-casing to the nearest hundredth of a foot (0.01 ft) in order to determine the elevation at each well. Depth to groundwater measurements were subsequently used to determine groundwater elevations and flow vectors. Refer to Table 1. 3.3 Site Hydrogeology Groundwater was encountered in each of the boreholes at approximately 18.0 to 21.0 feet bgs under semi-confined conditions. The survey confirmed groundwater movement at the Subject Property to generally be towards the west over an approximate horizontal hydraulic gradient of 0.066 (ft/ft). The nearest surface water body is a tributary to Crabtree Creek. The tributary is located less than 100 feet northwest of the property boundary, and its confluence with Crabtree Creek is located approximately 2,000 feet southwest. 3.4 Groundwater Quality Sampling On February 19, 2014, groundwater was sampled at MW-1 through MW-7 using new disposable polyethylene bailers to withdraw fresh groundwater from approximately midway the screen interval, and then transfer the groundwater directly into laboratory- prepared containers. The sample containers were placed in a cooler with ice and delivered to the laboratory, Prism Laboratories, Inc., under chain-of-custody protocol. It should be noted here that although standard field sampling protocol was followed, EI was informed by the laboratory that the samples were received at 7.2° Celsius, and therefore outside of protocol for temperature. As such, EI requested that the laboratory discard the groundwater samples. On February 24, 2014, EI re-sampled MW-1 through MW-7 using new disposable polyethylene bailers to withdraw fresh groundwater and then transfer the groundwater directly into laboratory-prepared containers. The samples were delivered to the laboratory by laboratory courier under chain-of-custody protocol. The samples were received in good condition within acceptable temperature hold limits. 3.5 Groundwater Quality Analysis The groundwater samples were delivered under chain-of-custody protocol to Prism Laboratories, Inc. (Prism) in Charlotte, North Carolina for VOC analysis. 3.6 Soil Test Borings As mentioned earlier, EI personnel subcontracted with Troxler Geologic Services, Inc. to advance the soil test borings using DPT methods. Mr. Darren R. Lockhart supervised the advancement of soil borings in the areas of concern. DPT boring activities were performed with a Geoprobe®. DPT refers to tools and sensors that are inserted into the subsurface without the use of drilling to remove soil and make a path for the tool. The Geoprobe® is a hydraulically-powered probing machine, which uses static force and a percussion hammer to advance small diameter sampling tools into the subsurface to collect soil cores, groundwater samples, and soil gas samples. A Geoprobe® relies on a relatively small amount of static (vehicle) weight combined with percussion as the energy for advancement of a tool string. The soil advancement tooling equipment and sampling equipment were decontaminated between each soil boring with a DI water/alconox rinse solution. During boring activities, soil samples were classified in the field utilizing the Unified Soil Classification System (USCS). The borings were backfilled at the base or bottom of the boring with hydrated bentonite pellets capped with soil boring cuttings. Limited Phase II Environmental Assessment 3400 Yonkers Road-Kimshe 3400 Yonkers Road, Raleigh, North Carolina EI Project ENMO130235.01 7 A total of ten soil borings (SB-6 through SB-16) were advanced for continuous soil screening and sampling: five soil borings on the rear/east of the building adjacent to two concrete pads (SB-7 through SB-11) and the remaining five soil borings (SB-12 through SB-16) advanced on the rear/south of the site building adjacent to a small concrete pad and adjacent to the site building (refer to Figure 5). 3.7 Soil Lithology Subsurface soils encountered in the area of study generally were described as follows: light brown and reddish-brown to tan and gray sandy CLAY (CL) with silt down to approximately 12.0 feet bgs; reddish-brown and yellowish-brown clayey, SILT (ML) - saprolitic down to 18.0 feet bgs; and then partially weathered rock down to 28.0 feet bgs, the maximum depth of investigation. Groundwater was encountered under semi-confined conditions at approximately 18.0 to 21.0 feet bgs. After well completion, groundwater levels increased to within eight to twelve feet within feet of the top of casings. As mentioned earlier, groundwater flows west toward a tributary to Crabtree Creek. Soil boring logs are presented in Appendix A. 3.8 Soil Sampling Representative soil profiles from the soil borings were collected in 5.0-foot increments using acetate liners contained in a steel cased macro sampling tube. There were no significant olfactory, visible and instrumental indications of impacts observed in the soil cores down to groundwater. As mentioned earlier, the water table was encountered between 18.0 and 21.0 feet bgs across the site. As there were no indications of impacts (i.e., visual, olfactory, and instrumental) in the soil cores at shallow depths, soil samples were collected at shallow depths adjacent to concrete pads or at deeper depths to adjacent to the building. Soil samples were “hand grabbed” from the soil sampler with disposable nitrile gloves, and then and placed into a sealable plastic bag for analysis with a Photo- Ionization Detector (PID) and subsequent collection. A total of ten soil samples were placed into clean laboratory prepared containers, labeled and placed in a cooler with ice. 3.9 Soil Analysis All soil samples were delivered under chain-of-custody protocol to Prism Laboratories of Charlotte, North Carolina to be analyzed for VOCs by EPA Method 8260B and for SVOCs-PAHs by EPA Method 8270D. As mentioned earlier, although standard field sampling protocol was followed, EI was informed by the laboratory that the samples were received at 7.2° Celsius. Although the samples were received outside of protocol for temperature, the soil samples were analyzed. Limited Phase II Environmental Assessment 3400 Yonkers Road-Kimshe 3400 Yonkers Road, Raleigh, North Carolina EI Project ENMO130235.01 8 4.0 LABORATORY ANALYTICAL RESULTS The laboratory analytical results for soil and groundwater quality for this assessment were compared to the regulatory standards listed under the NC Department of Environment and Natural Resources (NCDENR), Division of Waste Management’s (DWM), the Maximum Soil Contaminant Concentrations (MSCCs), the Inactive Hazardous Sites Branch’s Preliminary Soil Remediation Goals, and the 2L Groundwater Quality Standards. 4.1 Laboratory Analytical Results - Soil According to the laboratory analytical report, 1,1-dichloroethane (1,1-DCA) and 1,1,1- trichlorethane (1,1,1-TCA) were detected in SB-14 at estimated concentrations (J-values) of 0.0024 and 0.0071 milligrams per kilogram (mg/kg) or parts per million (ppm), respectively; however, these concentrations were below the lowest applicable MSCC standards of 0.38 and 1.6 mg/kg, respectively. Furthermore, the reported concentrations do not exceed the IHSB’s PSRGs for the protection of groundwater for 1,1-DCA (0.03 mg/kg) and 1,1,1-TCA (1.2 mg/kg). There were no other detectable concentrations of VOCs in SB-14 and in any of the other soil samples. Table 2 summarizes the soil sample analytical results. The laboratory analytical results are summarized on Figure 7. A copy of the laboratory analytical report is included in Appendix B. 4.2 Laboratory Analytical Results – Groundwater According to the laboratory analytical report, trichloroethylene (TCE) was detected in MW-3, MW-4, MW-5 and MW-6 ranging in concentrations from 1.2 to 2.9 µg/L. The reported TCE concentrations do not exceed the NC 2L standard for TCE of 3.0 µg/L. In addition, 1,1-DCA, 1,1-dichloroethylene (1,1-DCE), and cis-1,2-DCE were detected in MW-3 and MW-5; however, the reported concentrations do not exceed the respective NC 2L standards. Acetone was detected in MW-3 at 6.4 µg/L, which does not exceed the NC 2L standard for acetone of 6,000 µg/L. There were no other detectable concentrations of VOCs in any of the groundwater samples. Table 3 summarizes the groundwater sample analytical results. Refer to Figure 7. A copy of the laboratory analytical report is included in Appendix B. Limited Phase II Environmental Assessment 3400 Yonkers Road-Kimshe 3400 Yonkers Road, Raleigh, North Carolina EI Project ENMO130235.01 9 5.0 Potential Receptor Survey Information on the presence/absence of water supply wells was determined through a visual survey of the site vicinity within a one-half mile radius. No water supply wells were identified within the specified radius of the subject property during the survey, and no wells were listed within the radius according to the Wake County GIS iMaps website. Figure 3 shows an aerial photograph of the survey area and one-half mile radius. Potable water supply is provided to the site and vicinity by the City of Raleigh. In addition, EI conducted an evaluation of the site and adjacent properties for the presence of any environmentally sensitive areas according to the IHSB REC Program Guidance. Agencies concerned with oceanic/atmospheric and coastal/tidal environments were deemed “not applicable” and either not contacted, or their online resources were not reviewed based on the physiographic location of the site. A summary of the agencies reviewed and information obtained is presented below. This assessment disclosed the presence of chlorinated solvents in four of the seven groundwater samples; however, the reported concentrations do not exceed the NC 2L groundwater quality standard for any of the compounds. Although the 2L violation found during the March 2013 assessment was not replicated during this assessment, the assessment confirmed chlorinated solvents at low levels in groundwater at/near the limits of the property. The potential receptor survey found no wells on the site and in the vicinity. The sensitive evaluation did not identify any of the listed items on the site; however, the evaluation identified a riparian buffer within the western limits of the property, and a wetland area located adjacent-southwest of the site.
