Beazer Homes ESP Associates P.A submits the attached completed well abandonment record for 40-foot deep 2-inch diameter PVC monitor well The well was located within parking lot on Tax Parcel No 073-142-01 with an address of 1025 Pitcher Street in Charlotte North Carolina see Figures and The wells ID plate identified the well as 332 installed by Law Engineering on March 28 1990 The monitor well was completed with upright steel protective cover and concrete surface pad The well was abandoned on May 31 2006 in accordance to North Carolina requirements bySAEDACCOofFortMillSouthCarolinaNorthCarolina-certified well driller The well was chlorinated and then tremie grouted with cement/bentonite grout from its base to the ground surface The wells protective cover and concrete surface pad were removed and the well casing was cut off flush with the ground surface Mr Eddie Rogers an ESP staff scientist was on site to document the abandonment
