The field activities for this project consisted of sampling of existing monitoring and water supply wells. All activities were conducted in accordance with the NCDEQ guidelines and the requirements of 15A NCAC 2L .0115. Free Product: Free product has not been observed at this site during the only 2 sampling/gauging events conducted since approved soil/free product remedial excavation was conducted in September-October, 2021 and completed in March, 2022. Free product at the site has apparently been remediated via the (2) soil remediation projects conducted at this site based on post-excavation monitoring. 2. Contaminant Mass: Contaminant mass calculations cannot be completed at this time since the residual contamination which remains in place is predominantly situated beneath the former store building prohibiting the definition of the vertical and horizontal extent of contamination; however, dissolved constituent concentrations observed in the installed monitoring network have decreased significantly since the final soil excavation and placement of biodegradation materials conducted at the site on March 8, 2022. 3. Remedial Action Plan: Soil remediation and biological treatment activities have been conducted at this site as follows:  2,722.76 tons of soil/free product were excavated from September-October, 2021 at this site and transported for off-site disposal.  117.27 tons of soil/free product were excavated on March 8, 2022 at this site and transported for off-site disposal.  Nutrients, straw, and concentrated bio-degradation liquids were placed back into the remedial excavation prior to and during backfill of the 3/8/22 remedial excavation to accelerate natural processes in the UST release source area of this site. 7. Impacted Receptors: No water supply wells are located within 1000’ of the release source at this facility. Little Crabtree Creek borders the southern edge of the property. This creek was tested several times by Terry Environmental Services throughout the monitoring history of this site and has failed to indicate any impact from the petroleum release originating at the Hot Spot 2002 facility. A monitoring well network installed at the site in 2010- 2012 included (3) monitoring wells which were situated along Crabtree Creek and are located between the UST source area and this receptor. None of these wells, (MW-12, MW-22 and MW-23), have ever indicated any 6200B compounds (including benzene and toluene) above laboratory detection limits throughout the 10+ years of site monitoring. Since source area free product has been remediated, and dissolved benzene/toluene have been remediated to < 10X 15 NCAC 2B Standards, it is highly unlikely that this surface water receptor will ever receive groundwater impact as a result from the petroleum release at the former Hot Spot 2002 facility. Groundwater flow velocity was estimated at 7 ft/year by Terry Environmental during CSA activities. Jordan Oil Company of North Carolina, Inc. owned USTs which were operated at the Hot Spot 2002 facility located at 704 East Main Street in Burnsville, North Carolina. Figure 1 illustrates the location of this facility on the Burnsville Quadrangle U.S.G.S. Topo Map. This facility contains one permanent structure which is currently un- occupied. The site formerly contained (5) registered and (2) unregistered USTs which were last used at this facility between 1971 (unregistered USTs) and 2019 (registered USTs). On June 19, 2019, the registered USTs were put into temporary closure. During closure of the regulated USTs on September 29, 2021, the (2) unregistered USTs were discovered at the Hot Spot 2002 facility. It is believed that the (2) unregistered USTs were last used around April, 1971 when the registered USTs were installed (April 25, 1971). Figure 2 illustrates the overall layout of this site and the UST locations. Table 1 summarizes the UST history for the Hot Spot 2002 site. The original release from the UST system was reported to NCDEQ around May 14, 2010 upon receipt of analytical results from soil samples collected during a site check conducted on 4/27/2021. Groundwater and free product were observed during soil remediation activities. The site and adjacent properties are used for commercial purposes. Municipal water supplies are available to all properties within 1,500’. A surface stream is located within 500’ of the release source at this facility. With NCDEQ approval, the (7) USTs and 2,722.76 tons of soil/free product were excavated from September-October, 2021 at this site and transported for off-site disposal. Based on recurring free product in replacement well MW-1A subsequent to the 2021 remedial excavation, 117.27 tons of soil/free product were excavated at the site on March 8, 2022 at this site and transported for off-site disposal. Nutrients, straw, and concentrated bio-degradation liquids were placed back into the remedial excavation prior to and during backfill of the 3/8/22 remedial excavation to accelerate natural processes in the UST release source area of this site. 1.2 Scope of Services Contained herein are the results of a groundwater sampling event as approved by the NCDEQ. This Groundwater Monitoring Report contains documentation concerning the following activities which have been conducted at this facility: o Sampling/gauging of (12) existing monitoring wells conducted by Henry Nemargut Engineering on November 26, 2022. Page - 2 o Laboratory analyses of groundwater samples conducted by Pace Analytical, Inc. of Huntersville, NC. 2.0 Presentation of Current Site Assessment Information / Comparison to Historical Assessment Information 2.1 Historical Soil Assessment Information The soils at the project site consist of sand, clay, & silt of varying proportions. Soil boring logs are contained on well construction records included as Appendix A. On October 14 & 19, 2021, following remedial excavation, soil samples were collected from the perimeter of the remedial excavation above the shallow water table to obtain risk-based analytical data for the site. The soil samples collected at 704 East Main Street were labeled with sample location, depth, and date collected. The soil samples were obtained as specified in the collection, preservation, and handling section of the specific method. Each container was labeled with sample location, analyses to be performed, time, date, and the sampler's name. They were then placed in a cooler and chilled with ice to approximately 4oC in preparation for transportation to the analytical laboratory utilizing EPA approved chain of custody procedures. Samples collected at the limits of excavation were above current lowest MSCCs for sample UB2 for (11) compounds/compound groups, with C9-C22 aromatics noted above Residential MSCCs. These soils are located under the on-site structure and cannot be remediated without demolition of the building. Sample S1 collected near the retaining wall on the southern boundary of the remedial excavation contained C9-C22 Aromatic compounds above lowest MSCCs. No other perimeter soil samples obtained exhibited contaminants above lowest MSCCs. It should be noted that none of the wall samples collected were obtained from beneath the high seasonal water table at this site. Figure 3 illustrates the locations of the excavation perimeter soil samples collected at the subject site following remedial excavation activities. Table 2 summarizes the laboratory results for the soil samples from 704 East Main Street in Burnsville, NC. A copy of the soil analytical report and the chain of custody records for the risk-based soil samples collected at 704 East Main Street is contained in the previously submitted IAA report. 2.2 Ground Water Flow Direction and Gradient In order to estimate the groundwater flow direction in the vicinity of the petroleum release, elevations of the monitor well casings were measured using surveying equipment. During the November 26, 2022 sampling event, static water levels were obtained from the monitor wells (see Table 3) and the data was used to compute a potentiometric surface map. Figure 4 illustrates the current potentiometric surface map as calculated based on the measurements obtained on November 26, 2022. Page - 3 Groundwater data and monitoring well information were also used to estimate the hydraulic gradient at the project site. Using the horizontal distance between the monitor wells and the static water levels, the groundwater was calculated to flow toward the southeast with a hydraulic gradient of approximately 0.0175 ft/ft. 2.3 Monitor Well Sampling and Analyses On November 26, 2022, monitor wells MW-1B, 2R, 4R, 5R, 6, 7, 8, 10R, 11, 20, 21R, & MW-23 were sampled for analyses utilizing Standard Method 6200B + oxygenates. Gauging of these wells failed to indicate the presence of free in any of the monitoring wells formerly containing free product or their replacement monitoring wells. Before collecting the samples, the monitor wells were purged by removing a minimum of three well volumes to ensure that the samples were representative of the actual groundwater conditions. The wells were developed using disposable plastic bailers suspended with unused nylon string. All samples were placed in laboratory supplied glassware, labeled with sample location, analysis to be performed, time, date and sampler's name. The sample jars were then immediately placed in a cooler, chilled with ice to approximately 4oC in preparation of transportation to an analytical laboratory utilizing EPA approved chain-of-custody procedures. Laboratory analytical results indicate the presence of (3) compounds above the current listed 2L Standards in the monitor well network. Benzene was detected at a maximum concentration of 66.1 ug/l in MW-5R, above its 2L standard of 1 ug/l. Naphthalene was detected at a maximum concentration of 62.7 ug/l in MW-8, above its 2L standard of 6 ug/l. MTBE was detected at a maximum concentration of 71 ug/l, above its 2L Standard of 20 ug/l. No GCL violations were noted during this monitoring event. Toluene was detected at a maximum concentration of 15.8 ug/l in MW-2R, below its 2L Standard and also at a concentration of < 10X 15 NCAC 2B Standard. No compounds were detected at concentrations > 10X 15 NCAC 2B Standards. Table 4 summarizes the laboratory analytical results, and Appendix A contains a copy of the laboratory analytical report and chain of custody records. Figure 5 contains a benzene concentration map, Figure 6 contains a toluene concentration map, Figure 7 contains a Naphthalene concentration map, and Figure 8 contains a total 6200B concentration map. Figure 8H contains the baseline historical Total 6200B concentration map as measured on 2/3/2022. 2.4 Regional / Site Geology and Hydrogeology The site is situated in the Blue Ridge Geologic Province. According to the Geological Map of North Carolina local geology of the region is comprised of a series of thrust sheets. According to Terry Environmental, Saprolite was encountered at a depth of 38 feet below surface, but competent bedrock was not encountered to a depth of 50’, which was the maximum depth explored during subsurface investigations at this site. Soils encountered at the site during the CSA consisted of clayey silt and silty clay. Page - 4 The partial Burnsville Quadrangle U.S.G.S. Topographic Map included as Figure 1 indicates that the release site is located at an approximate elevation of 3282’ amsl. Surface waters in the vicinity of the release area generally drain south towards Little Crabtree Creek which borders the southern boundary of the Hot Spot 2002 parcel. During CSA activities, slug tests performed by Terry Environmental indicated an average hydraulic conductivity of 0.41 ft/day, with an estimated ground water flow velocity of 7 ft/year at the Hot Spot 2002 release site. 2.5 Plume Development The monitoring well network at this release site was installed between 2010 and 2012, with replacement free product area wells installed in February, 2022 following completion of October, 2021 initial excavation, and again in March, 2022 following the MW-1A area IAA excavation. In the only prior comprehensive sampling event conducted at the site following the October, 2021 soil excavation project, 2L violations for (8) 6200B compounds were noted in monitor wells MW-1A, MW-2R or MW-5R. The site maximum Total 6200B level was noted in MW-1A on February 3, 2022 was observed at a concentration of 4,862 ug/l. Both benzene and toluene were present in MW-1A on that date at a concentration > 10X 15 NCAC 2B; however, no other monitoring wells at the site were noted to exceed 10X the surface water standards. Following additional MW-1A/MW-2R area excavation at the site on March 8, 2022, nutrients, straw, and concentrated bio-degradation liquids were placed back into the remedial excavation prior to and during backfill to accelerate natural processes in the UST release source area of this site. The second MW-1 replacement well, MW-1B, was installed on March 9, 2022 and sampled on March 10, 2022 and was found to contain significantly reduced benzene and toluene concentrations significantly < 10X 2B Standards. During the latest sampling event conducted on November 26, 2022, natural processes appear to be reducing both the benzene and toluene source area concentrations, with the maximum toluene concentration at the site noted in MW-2R at 15.8 ug/l, and the maximum benzene concentration level noted in MW-5R at a level of 66.1 ug/l. Both the benzene and toluene values are currently significantly < 10X 2B standards. 3.0 Monitored Natural Attenuation 3.1 Natural Source Zone Depletion Status Historically, 2L violations for 8 compounds were observed at this site in MW-1A/MW- 2R during the only previous comprehensive sampling event conducted on February 3, 2022. Only (3) compounds were noted above 2L Standards during the latest post soil remediation monitoring event, with no 10X 2B exceedences. Due to the low groundwater flow velocity (7 ft/yr), the absence of free product, and no compounds > 10X 2B Standards, the risk of impact to Crabtree Creek is considered to be de minimis. Page - 5 4.0 Conclusions 4.1 General Summary A Groundwater Monitoring event at Hot Spot 2002 located at 704 East Main Street in Burnsville, NC has been completed. From a review of all information gathered during this monitoring project, Henry Nemargut Engineering makes the following conclusions: o On November 26, 2022, monitor wells MW-1B, 2R, 4R, 5R, 6, 7, 8, 10R, 11, 20, 21R, & MW-23 were sampled for analyses utilizing Standard Method 6200B + oxygenates.
