Phase II LSA Report 1 October 24, 2014 1.0 INTRODUCTION Cary Oil Company (Cary Oil) retained ATC Associates of North Carolina, P.C. (ATC) to conduct groundwater assessment activities at the Yogi Foods site located at 4018 Highway 321A in Hudson, Caldwell County, North Carolina. A United States Geological Survey (USGS) topographic map that shows topographic contours, site location, surface water bodies, buildings, and adjacent roads in the site vicinity is included as Figure 1. 1.1 Site Description The subject site currently operates as a Shell service station with retail fuel sales and a convenience store called Market Basket. A site map that indicates monitoring well locations and other site features is included as Figure 2. The site is accessible from Highway 321A to the west and Adams Drive to the southeast. Properties within 1,500 feet of the site are mainly commercial and residential on the east side Highway 321A and mainly industrial on the west side of Highway 321A. Two 8,000-gallon gasoline underground storage tanks (USTs) and one 8,000-gallon diesel UST were installed in April 1985 and are currently in use at the site. A separate basin contains one 2,000-gallon kerosene UST, which was installed in February 1976 and is also currently in use. UST system and owner/operator information is provided in Tables B-1 and B-2. The site also consists of a single-story convenience store building and fuel dispensers associated with the USTs. Surface cover at the site consists mostly of asphalt and concrete in the fueling and driveway areas. The surface cover on the east portion of the property, behind the convenience store building, consists mostly of grass. The site and surrounding areas are supplied municipal water by the Town of Sawmills. An updated receptor survey was conducted in 2012, which provided supplemental information to an initial receptor survey conducted during a Phase I Limited Site Assessment (LSA) in 2006. Based on information obtained from the updated receptor survey, one private well (WSW-4) is being used for irrigation purposes and is located approximately 315 feet from the source area. Thirteen additional water supply wells were identified within 1,500 feet of the site but were not in use. A large concrete cover was observed at the property adjacent to the subject site and was identified as a potential water supply well. However, the property owner could not provide information because the cover was there when he bought the property. Due to the size, weight, and security (apparently cemented in place) of the cover, confirmation of whether the object was a well could not be validated. Without the ability to confirm the object was not a potential receptor, the North Carolina Yogi Foods ATC Associates of North Carolina, P.C. Phase II LSA Report 2 October 24, 2014 Department of Environment and Natural Resources (NCDENR) has assigned a high risk classification to the site based on the object’s proximity to the subject site. The property with the suspected well, located at 2338 Adams Drive, is supplied municipal water by the Town of Sawmills. Poplar Fish Lake is located approximately 1,100 feet to the east of the subject site. The drainage feature that supplies Poplar Fish Lake originates closer to the site but does not appear to be a perennial stream over the closer interval. Soils encountered during installation of monitoring wells on-site have been described as sandy clay, sandy silt, and silty clay to a termination depth of approximately 30 feet below grade. Depth to groundwater in site monitoring wells ranged from 15.55 feet to 23.05 feet below top of casing (TOC) on August 6, 2014. Groundwater flow appears to be generally towards the east based on elevation data obtained on August 6, 2014. 1.2 Site History A due diligence investigation was conducted by Cary Oil on June 30, 2006. Soil samples were collected at nine locations (YF-1 through YF-9) and submitted for laboratory analysis of total petroleum hydrocarbons – gasoline range organics (TPH – GRO). Laboratory analytical results of a soil sample collected adjacent to a gasoline and diesel dispenser (YF-4) indicated a TPH – GRO concentration that exceeded the NCDENR action level of 10 mg/kg. Cary Oil submitted a 24-Hour Release and UST Leak Reporting Form (UST-61) to the NCDENR on July 14, 2006. ATC conducted a Phase I LSA on August 10 and 11, 2006, based on the results of the due diligence investigation. Monitoring well MW-1 was installed with 1-inch diameter polyvinyl chloride (PVC) pipe to a depth of 28 feet below grade, with a 15 foot screened section. Soil samples were collected through split spoon samplers during well installation activities and submitted for laboratory analysis of volatile organic compounds (VOCs) by EPA Method 8260, semi-VOCs by EPA Method 8270, and Massachusetts Department of Environmental Protection volatile petroleum hydrocarbons (MADEP VPH). Laboratory analytical results identified contaminant concentrations above soil-to-groundwater maximum soil contaminant concentrations (MSCCs). A groundwater sample was collected from well MW-1 and submitted for laboratory analysis of VOCs by EPA Method 624, MADEP VPH, and lead by EPA Method 6010. Laboratory analytical results indicated groundwater contaminant concentrations above NCDENR 2L Standards, and the benzene concentration exceeded the Gross Contamination Level (GCL). Yogi Foods ATC Associates of North Carolina, P.C. Phase II LSA Report 3 October 24, 2014 ATC recommended additional assessment to determine the extent of petroleum contaminant impact. This report documents the results of the subsequent Phase II LSA. 2.0 RISK CHARACTERIZATION In accordance with 15A NCAC 2L .0115, a completed Limited Site Assessment Risk Classification and Land Use Form provided by the NCDENR is included in Appendix A. Answers and explanations included on the form are based on information obtained by ATC during Phase I and II LSA activities. 3.0 RECEPTOR INFORMATION 3.1 Water Supply Wells An updated receptor survey was conducted in 2012, which provided supplemental information to an initial receptor survey conducted during a Phase I LSA in 2006. Water supply well forms were sent via certified mail to property owners within 500 feet of the site and to owners of properties where water supply wells had been previously identified. ATC subsequently conducted a visual site reconnaissance within a 1,500 foot radius of the site to confirm the existence of water supply wells in the area. Based on information obtained from the updated receptor survey, one private well (WSW-4) is being used for irrigation purposes and is located approximately 315 feet from the source area. Phase II LSA Report 4 October 24, 2014 3.2 Public Water Supplies Potable water is provided to the area by the Town of Sawmills and the City of Lenoir. The public water source (Lake Rhodhiss) is not located within 1,500 feet of the subject site. 3.3 Surface Water ATC reviewed a USGS 7.5-Minute Quadrangle Topographic Map (Figure 1) to identify surface water bodies in the site vicinity. In addition, ATC conducted a visual reconnaissance for surface water bodies in the area. Poplar Fish Lake is located approximately 1,100 feet to the east of the subject site. The drainage feature that supplies Poplar Fish Lake originates closer to the site but does not appear to be a perennial stream over the closer interval. 3.4 Wellhead Protection Areas According the Division of Water Quality Wellhead Protection Website, the site is not located within a wellhead protection program area. 3.5 Deep Aquifers in the Coastal Plain The risk classification criteria specified in Title 15A NCAC 2L .0115(d) requires an evaluation for the presence of unconfined or semi-confined deep aquifers in the Coastal Plain region which may be used as a source of drinking water. The site is not located in the Coastal Plain Physiographic region. 3.6 Subsurface Structures Prior to drilling activities, a private utility locating company and One-Call of North Carolina, a public utility locating service, were contacted to locate underground utilities at the site. Underground utilities (water, electric, natural gas, telephone) were located on-site; however, the utilities did not intersect the water table. Phase II LSA Report 5 October 24, 2014 3.7 Land Use The subject site is located in Caldwell County, North Carolina. The site is accessible from Highway 321A to the west and Adams Drive to the southeast. The site operates as a retail petroleum and convenience store, and contains two gasoline USTs, one diesel UST, and one kerosene UST. The site also consists of a single-story convenience store building and fuel dispensers associated with the USTs. Surface cover at the site consists mostly of asphalt and concrete in the fueling and driveway areas. The surface cover on the east portion of the property, behind the convenience store building, consists mostly of grass. A Limited Site Assessment Risk Classification and Land Use Form is included in Appendix A. Properties within 1,500 feet of the site are mainly commercial and residential on the east side Highway 321A and mainly industrial on the west side of Highway 321A. The closest residential building is located adjacent to the northeast of the subject site. The closest place of public assembly is the Sawmills Town Hall located approximately 1,250 feet south-southeast of the release area. The subject property is zoned as H-B (Highway Business). Areas surrounding the site are zoned as H-B, R- 15 (Residential), and G-M (General Manufacturing). 4.0 SITE CHARACTERISTICS 4.1 Regional Geology According to the Geologic Map of North Carolina, produced by the State of North Carolina in 1985, the site lies in the Inner Piedmont Belt of the Piedmont Physiographic Province of North Carolina. The rock types of the Inner Piedmont have been identified as predominatly mica shist with a variety of other rock types depending on location. The Inner Piedmont is characterized as crystalline meta-sedimantary rock. The shallow subsurface in most areas of the Piedmont contains residual soil overburden, including structure-free residuum, saprolite, and partially weathered rock (PWR) that derive from in-place weathering of the crystalline bedrock. Occasional areas containing recent deposits of alluvium in the uppermost subsurface are found near streams and rivers. Saprolite and PWR typically contain some relict structures from the original rock material. Depth to rock ranges from ground surface at occasional outcrops to depths of greater than 100 feet in areas of easily weathered rock. Yogi Foods ATC Associates of North Carolina, P.C. Phase II LSA Report 6 October 24, 2014 4.2 Site Geology Shallow subsurface soils observed during drilling activities were described as sandy clay, sandy silt, and silty clay to a termination depth of approximately 30 feet below grade. Soil boring logs from the Phase II LSA are included as Appendix B. 4.3 Regional Hydrogeology The shallow aquifer occurrence varies in depth from ground surface at springs, creeks, and rivers to as deep as 50 feet or more beneath upland surfaces in some parts of the Piedmont. Water in the alluvium or unconsolidated residual material, including saprolite and PWR, usually behaves as an unconfined aquifer, or water table, and will yield water with head elevation equivalent to the first elevation where water was encountered. Permeability varies with lithology and is typically relatively low in residual soils, with higher permeability in saprolite or PWR due to relict rock texture and variable susceptibility to weathering exhibited by different minerals in the rock. Groundwater flow in residual soils or alluvium is usually in rough concurrence with local topographic conditions and is toward local drainage features. The bedrock fractures or other planar features of the Piedmont Physiographic Province generally constitute the bedrock aquifer, with the surrounding rock material being effectively impermeable. Along with fractures, contacts between rock bodies probably constitute zones of significant groundwater occurrence in the bedrock. The surrounding material and overlying residuum tend to make the bedrock aquifer a semi-confined aquifer. That is, the overlying water and soil weight normally results in pressure that causes water in a borehole, which intersects a fracture or other feature, to rise above the elevation of the fracture or feature. Modern water supply wells in the Piedmont, likely including those in the site vicinity, are typically installed with a surface casing through the unconsolidated material, and an open borehole into the bedrock where water-bearing fractures are encountered. 4.4 Site Hydrogeology Depth to groundwater in site monitoring wells MW-1 through MW-4 ranged from 15.55 feet to 23.05 feet below TOC on August 6, 2014. Free product was not observed in the site wells. Based on elevation data obtained on August 6, 2014, groundwater flow appears to be generally towards the east at an average horizontal hydraulic gradient of 0.009 along a flow path that crosses the source. Phase II LSA Report 7 October 24, 2014 5.0 SUBSURFACE INVESTIGATION 5.1 Soil Sampling On June 30, 2006, soil samples were collected at nine locations (YF-1 through YF-9) and submitted for laboratory analysis as part of a due diligence investigation. Laboratory analytical results of a soil sample collected adjacent to a gasoline and diesel dispenser (YF-4) indicated a TPH – GRO concentration that exceeded the NCDENR action level of 10 mg/kg. Boring locations are indicated on Figure 4 and soil analytical results are provided in Table B-3. On August 10, 2006, split spoon samplers were advanced at a location adjacent to the gasoline and diesel dispenser, in the vicinity of soil sample YF-4. The boring location is indicated on Figure 4. Continuous soil samples were collected from grade to a depth of 25 feet below grade. The soil samples were characterized for lithology and screened with a photo-ionization detector (PID). Petroleum compounds were reported in the soil samples at concentrations above soil-to-groundwater MSCCs. Soil analytical results are provided in Table B-3. Soil samples were collected on August 5, 2014, during installation of wells MW-2 through MW-4 and characterized in the field for lithology and screened with a PID. Soil boring logs are provided in Appendix B. Soil samples were not submitted for laboratory analysis, per NCDENR guidelines for Phase II LSAs. 5.2 Well Installation Three groundwater monitoring wells (MW-2 through MW-4) were installed at the subject site during the Phase II LSA. Due to the presence of multiple underground and overhead obstructions, the location of well MW-3 was adjusted in the field. The wells were installed to a depth of 30 feet below grade, with 15 feet of 2-inch diameter 0.010-inch slotted screen and 15 feet of PVC riser casing. The wells were secured at the surface with steel manhole covers and concrete pads. Boring logs and well records are provided in Appendix B. ATC returned to the site on August 6, 2014, to gauge and collect groundwater samples from wells MW-1 through MW-4. Depths to groundwater at the site ranged from 15.55 to 23.05 feet below TOC. Groundwater flow was generally towards the east at the site based on data obtained on August 6, 2014. A groundwater elevation map is provided as Figure 5. Yogi Foods ATC Associates of North Carolina, P.C. Phase II LSA Report 8 October 24, 2014 Prior to sampling, each well was purged of three standing water columns, or until the well was purged dry. Samples were then collected with new disposable bailers and poured directly into laboratory-provided containers with appropriate preservatives. The samples were placed in a chilled cooler and submitted to North Carolina-accredited Pace Analytical Services, Inc. of Huntersville, North Carolina, using proper chain- of-custody procedures for laboratory analyses. The groundwater samples were analyzed for VOCs by EPA Method 6200B, semi-VOCs by EPA Method 625, lead by EPA Method 6010, ethylene dibromide (EDB) by EPA Method 504.1, and MADEP VPH and extractable petroleum hydrocarbons (EPH). Petroleum compounds were reported above NCDENR 2L Standards in wells MW-1, MW-2, and MW-4. Concentrations of lead were reported above the 2L Standard in wells MW-1 through MW-4. The concentrations of benzene and EDB reported in well MW-1 exceeded their respective GCLs. Groundwater analytical data is presented in Table B-4 and on Figures 6 through 9. The laboratory analytical report is provided in Appendix C. 6.0 CONCLUSIONS ATC conducted a Phase II LSA at the Yogi Foods site in August 2014 to obtain groundwater flow and contaminant information. Groundwater contaminant concentrations were identified above 2L Standards in site monitoring wells, and benzene and EDB were identified in monitoring well MW-1 above their respective GCLs. Based on the concentrations of groundwater contamination, the site would be ranked as intermediate risk. However, due to a potential water supply well located within 250 feet of the site, the incident has been assigned a high risk. ATC recommends additional assessment to delineate the extent of the petroleum release in soil and groundwater. Potential exists to abandon water supply wells and connect the users to municipal supply, if not already provided, or to conduct remediation of groundwater impact to lower site risk.
