The North Carolina Division of Waste Management is conducting an environmental assessment at the Smith’s Grocery Store site located near your property. As part of the assessment, a water sample was collected from the supply well located at your property on August 16, 2011. The sample was submitted
for laboratory analysis of volatile organic compounds by Standard Method 6200B. Review of the analytical test data indicated that the water is not fit for use, and the DWM is prepared to install a treatment system on your well to remove the petroleum compounds from your water. The DWM
attempted to contact you on September 15, 2011 to discuss installing the treatment system; however the telephone calls were not successful. Please contact me at your earliest convenience to discuss the system requirements. Your well is identified as WSW4 on the attached lab report. In addition, the project name
is incorrectly identified as Black’s Grocery on the analytical report. If you have any questions, please contact me at [phone].
