(ESP) is pleased to present this letter report documenting results of the monitoring well abandonment confirmation at the Jerry’s Place site in accordance with North Carolina Department of Environmental Quality (NCDEQ), Task Authorization No. 1, approved on October 27, 2023. The subject site is located at 983 US Highway 21 in State Road, North Carolina. Monitoring Well Abandonment Confirmation Based on a report provided by previous consultant Terracon Consultants Inc. (Terracon) titled “Annual Monitoring Report, Jerry’s Place, 983 US Highway 21, State Road, Surry County, North Carolina” dated September 22, 2023, the site was recommended for closure based on constituent concentrations below Gross Contamination Levels and results of a biodegradation model. Based on this recommendation, the release incident was closed with a Notice of No Further Action along with a Notice of Residual Petroleum on January 3, 2023. In 2023, Terracon reportedly mobilized to the site along with a North Carolina licensed driller to abandon all site monitoring wells (i.e., MW-2/RW-2, MW-3/RW-3, MW-4/RW-4, MW-5, RW-5, MW-6, MW-7, and MW-8) as depicted on Figure 1. As of the time of this report, ESP was unable to determine when these closure actions took place. On November 15, 2023, ESP personnel mobilized to the site and were able to visually confirm the abandonment of all site monitoring wells, except for monitoring well MW-8, which was covered by a solid waste dumpster. ESP personnel were able to confirm the abandonment of monitoring well MW-8 with the current property owner, who was on site during the abandonment event. Conclusions Based on our visual observations or property owner confirmation, ESP can confirm that all site monitoring wells have been abandoned.
