Mr. Ryals,
WithersRavenel, Inc. (WR) has prepared this Letter Report documenting the completion of monitoring
well abandonment activities at the Former L.D. Roach Grocery site located at 2070 Nelson Road,
Grifton, Craven County, North Carolina. Activities outlined herein were completed under NCDEQ
Contract # N70522‐C, Task Authorization (TA) 02 for the site, issued by NCDEQ on August 17, 2023.
WR mobilized to the site on September 15, 2023 to complete the abandonment of monitoring wells
MW‐1, MW‐2, MW‐3, MW‐4R, MW‐5, MW‐6, MW‐7, MW‐8, MW‐9, MW‐10, MW‐11, MW‐12
and MW‐13. WR Personnel could not locate well MW‐9 despite the use of historical maps and a
metal detector. Therefore, MW‐9 was not abandoned. The activities completed in association with
this TA are discussed in the following paragraphs.
Well Abandonments ‐ MW‐1 through MW‐13, Inclusive.
Monitoring wells MW‐1, MW‐2, MW‐3, MW‐4R, MW‐5, MW‐6, MW‐7, MW‐8, MW‐10, MW‐11,
MW‐12 and MW‐13 were gauged on July 25, 2023 using a decontaminated electronic oil/water
interface meter. No LNAPL/free product was measured within the wells. Groundwater levels in
the monitoring wells measured on July 15, 2023 are shown below:
Well ID Depth to Groundwater (ft) Total Well Depth (ft)
MW‐1 7.23 9
MW‐2 6.22 8
MW‐3 5.95 7
MW‐4R 6.13 8.5
MW‐5 5.96 8
MW‐6 6.16 8

Each monitoring well was abandoned through the emplacement of 3/8‐in dia. bentonite pellets to
within 1‐ft of each wells’ top of casing (TOC). The remainder of each well was filled with neat
cement grout extending out of the TOC filling the well manholes. The well abandonment record
(NCDEQ GW‐30) is included as Attachment A, while field notes and site photos for completion of
the activities are provided as Attachment B.
