SUBJECT: Request for Health Risk Evaluation for Well Sampling Results
Smith Well
Moores Exxon, Incident #6146
Glendale Springs, Ashe County, WSRO

Water samples were collected from a supply well located at the Larry Smith property on February
26, 2019. The Smith well is identified as WSW-7414 on the attached analytical report. Please
perform a health risk evaluation on the attached analytical results and send your determination to
our office. Our office will then forward your evaluation to the well owner. Please note that a water
treatment system is installed at the Stewart Property.
