Post Office Box 769 Marshville, North Carolina 28103 Prepared by: Resolve Environmental Services, PLLC 500 Devonport Drive Matthews, North Carolina 28104 June 21, 2021 Laura Minor Project Manager i EXECUTIVE SUMMARY The Kwick Mart-Mint Hill is an underground storage tank (UST) site located at 10308 Albemarle Road in Charlotte, Mecklenburg County, North Carolina. A total of three (3) USTs (one 12,000-gallon gasoline UST, one 8,000-gallon gasoline UST and one (1) 4,000-gallon kerosene UST) have existed at the property since June 1994 to the present. The adjoining properties are commercial and residential. A previous release at the site was reported on August 19, 2002, following a Site Check assessment. The release was identified as NCDEQ Incident 24539. The USTs were owned and operated by Brewer-Hendley Oil Company at the time of the release. A monitoring well was installed during LSA activities at the site in December 2002. No detectable concentrations of target compounds were reported in the ground water sample collected during the 2002 LSA. The incident was issued a no further action status on February 19, 2003. During a routine compliance inspection conducted by the North Carolina Department of Environmental Quality (NCDEQ) on August 27, 2019, the spill bucket at the 12,000-gallon regular gasoline UST was noted to need repair/replacement. A NCDEQ Notice of Violation dated August 28, 2019 requesting a Site Check was sent to Mr. John N. Hendley, Registered Agent, of Brewer-Hendley Oil Company, Inc. (BHOC) of Marshville, North Carolina. A soil sample was collected adjacent to the regular gasoline spill bucket on September 18, 2019. The sample was submitted for analyses for gasoline-range organics (GRO) by Method 8015C. TPH-GRO was not detected in the soil sample. Therefore, no further assessment was required. In May 2020, Resolve was contracted to perform a Phase II Environmental Site Assessment (ESA) at the site by BHOC. Four (4) temporary monitoring wells were installed at the site. Soil and ground water samples were collected from each sampling point. No detectable concentrations of TPH-GRO or diesel- range organics (DRO) were reported in the soil samples. A concentration of benzene that exceeded the maximum allowable concentration (MAC) was reported in one of the ground water samples (TMW-4). Based on the ground water quality violation, a UST-61 form dated September 10, 2020 was submitted to the NCDEQ. A Notice of Regulatory Requirements (NORR) dated September 15, 2020 was prepared by the NCDEQ which requested Limited Site Assessment (LSA) activities. Resolve supervised the installation of one (1) ii Type II monitoring well (MW-1) on September 16, 2020 in the former location of Phase II ESA sampling point TMW-4. The well was completed to a depth of 35 feet on September 18, 2020. With the exception of an estimated concentration of acetone in soil sample MW-1-5’, no other requested method constituents were reported in the collected soil samples. Concentrations of benzene, MTBE, EDB and lead that exceeded the MACs were reported in the ground water sample collected from MW-1. None of the concentrations exceeded the gross contamination levels (GCLs) nor 10 times the 2L ground water quality standards. In addition, detectable and/or estimated concentrations of other requested method constituents were reported in the ground water sample collected from MW-1; however, none of the concentrations exceeded the MACs. During the September 2020 receptor survey, a total of thirteen water supply wells (WSW-1 through WSW- 13) were identified within 1,500-feet of the source area. Four (4) potable wells (WSW-1, WSW-2, WSW- 5 and WSW-10) are located with 1,000 feet of the sources area. An additional potable well, WSW-13 is located approximately 1,200 feet from the source area. The remaining identified ells are presumed to be used for irrigation purposes only, since all of the corresponding properties are connected to municipal water. County water is available to all properties in the surrounding area of the site. No surface water body was identified with a 500-foot radius of the site. Azusa Grotto Lake is located approximately 1,300 feet to the southwest of the site and an unnamed tributary of Clear Creek is located approximately 1,300 feet east of the site. County water is available to all properties in the surrounding area of the site. Based on the receptor survey information and ground water quality data collected to date, the site meets the criteria for a high-risk classification. It should be noted that the detectable concentrations of identified compounds exceeding 2L and the distance to the nearest receptor (480 feet) make the potential for affecting the receptor at or above the EPA’s MCL extremely low. In addition, the presence of MTBE indicates that the release occurred prior to 2009. Based on the high-risk classification, Phase II LSA activities, including the installation of two (2) additional Type II monitoring wells (MW-2 and MW-3) were installed on April 7, 2021. MW-2 and MW-3 were installed to 35 feel BGS. Please note that monitoring well MW-1A (installed during the 2002 LSA was iii located on the property and used during Phase II LSA activities, in lieu of the installation of proposed monitoring well MW-4. Concentrations of benzene, toluene, ethylbenzene, xylenes, naphthalene, EDB, 1,2-DCA, 2-Hexanone, isopropylbenzene, MIBK, n-propylbenzene, 1,2,4-trimethylbenzene, 1,3,5-trimethylbenzene, C5-C8 aliphatics and C9-C10 aromatics that exceeded the MACs were reported in the ground water sample collected from MW-3. A concentration of MTBE that exceeded the MAC was also reported in the ground water sample collected from MW-2. The concentration of benzene in MW-3 also exceeded the GCL. Detectable and/or estimated concentrations of other requested method constituents were reported in the ground water samples collected from MW-1 through MW-3; however, none of the concentrations exceeded the MACs. iv SITE INFORMATION 1. NO Explain: Property Surrounding Source Area of Release The questions below pertain to the area within 1,500 feet of the source area of the release (excludes property containing source area of the release): 9. What is the distance from the source area of the release to the nearest primary or secondary residence (permanent or temporary)? A mobile home park is located on the adjacent property to the east with the closest trailer being approximately 325’ east/southeast from the source area. 11. What is the distance from the source area of the release to the nearest school, daycare center, hospital, playground, park, recreation area, church, nursing home or other place of public assembly? Wilson Grove Baptist Church is located approximately 960 feet south of the source area. 12. What is the zoning status of properties in the surrounding area? According to the City of Charlotte, the area surrounding the site to the east, west and south is zoned Business and the area to the north is zoned Industrial. 13. Briefly characterize the use and activities of the land in the surrounding area. Used auto lots are located across Wilgrove Mint Hill Road to the west and across Albemarle Road, north of the site. A medical office and mobile home park is located to the south and west of the site, respectively. 2. RECEPTOR INFORMATION 1. Water Supply Wells: Thirteen (13) water supply wells (WSW-1 through WSW-13) were identified within a 1,500-foot radius of the subject site. Four potable water supply wells (WSW-1, WSW-2, WSW-5 and WSW- 4 10) were identified within 1,000 feet of the source area. Potable well, WSW-13, was identified within 1,200 feet of the source area. The remaining identified wells are presumed to be used for irrigation purposes only, since the corresponding properties are connected to municipal water. A site location map is provided as Figure 1 and a Site Vicinity Map showing property boundaries for the site and surrounding properties have been included as Figure 2. Water supply well owner information is summarized in Table 3. Water supply well usage questionnaires dated October 22, 2020 were mailed to all property owners located within 500-feet of the source area. The questionnaires will be forwarded to the NCDEQ incident manager upon receipt. Please note that addresses tied to each parcel that a water supply well was identified to be present were verified of either having active water accounts with the City of Charlotte or alternatively, not possessing active water accounts with the City of Charlotte by Resolve personnel on October 27, 2020. 2. Public Water Supplies: Are public water supplies available within 1,500 feet of the source area of the release? YES If yes, where is the location of the nearest public water lines and the source(s) of the public water supply. Explain: Municipal water is available to the site and the surrounding properties. Water lines run parallel to Albemarle Road and Wilgrove Mint Hill Road, north and west of the onsite structure, respectively. 3. Surface Water: Identify all surface water bodies (e.g., ditch, pond, stream, lake, river) within 1,500 feet of the source area of the release. This information must be shown on the USGS topographic map. Azusa Grotto Lake is located approximately 1,300 feet southwest of the source area. An unnamed tributary of Clear Creek is located approximately 1,300 feet east of the source area. 4. Wellhead Protection Areas: Identify all planned or approved wellhead protection areas in the vicinity of the source area. This information must be shown on the USGS topographic map. Wellhead protection areas are defined in 42 USC 300h-7(e). On October 24, 2020, Resolve personnel accessed the North Carolina Department of Environmental Quality (NCDEQ) Source Water Assessment and Protection (SWAP) Program’s website to identify wellhead protection areas in the vicinity of the subject site. There were no designated wellhead protection areas near the subject site. 5 5. Deep Aquifers in the Coastal Plain Physiographic Region: The site is not located in the Coastal Plan Physiographic Region. 6. Subsurface Structures: No structures with basements were observed in the vicinity of the source area. Utilities in the area do not appear to intersect the seasonal high water table. 7. Property Owners and Occupants: The names and addresses of the owners of properties adjacent to the site are presented in Table 4. Property boundaries are shown on Figure 2. 3. SITE GEOLOGY AND HYDROGEOLOGY According to the 1985 Geologic Map of North Carolina, the site is located in the Charlotte Belt of metamorphic rocks. The underlying rock consists of interbedded felsic to mafic tuffs and flowrock. Ground water in the area moves through fractures and cleavage planes within the metamorphic rock. Red clayey silt was encountered during the borehole drilling for MW-2 and MW-3. Water was encountered during the installation of MW-2 and MW-3 at a depth of approximately 20 feet and the wells were set to total depths of 35 feet BGS with a 20-foot section of screen. Copies of the well construction records and subsurface logs have been included as Appendix A. A Site Map is included as Figure 3. Well construction and gauging information is presented in Table 5. 4. SITE INVESTIGATION A previous release at the site was reported on August 19, 2002, following a Site Check assessment. The release was identified as NCDEQ Incident 24539. The USTs were owned and operated by Brewer-Hendley Oil Company at the time of the release. A monitoring well was installed during LSA activities at the site in December 2002. No detectable concentrations of target compounds were reported in the ground water sample collected during the 2002 LS. The incident was issued a no further action status on February 19, 2003. Following the identification of a faulty spill bucket at the 12,000-gallon gasoline tank during a routine compliance inspection conducted by NCDEQ on August 27, 2019, Resolve conducted site check activities in September 2019. A soil sample was collected near the 12,000-gallon gasoline spill bucket on September 18, 2019. TPH-GRO was not detected in the soil sample. Per BHOC’s request, Resolve conducted a Phase II ESA at the site in May 2020. Four (4) temporary monitoring wells (TMW-1 through TMW-4) were installed at the site on August 20, 2020. Soil and ground water samples were collected from each sampling 6 point. No detectable concentrations of TPH-GRO or TPH-DRO were reported in the soil samples. A concentration of benzene that exceeded the MAC was reported in one of the ground water samples (TMW- 4). Based on the ground water quality violation, a UST-61 form dated September 10, 2020 was submitted to the NCDEQ. A Notice of Regulatory Requirements (NORR) dated September 15, 2020 was prepared by the NCDEQ which requested Limited Site Assessment (LSA) activities. Resolve supervised the installation of one (1) Type II monitoring well (MW-1) beginning on September 16, 2020. MW-1 was initially installed to a total depth of 25 feet and completed with a 10-foot screened interval set to bracket the water table at the time of installation. However, due to lack of water, MW-1 was re-installed on September 18, 2020 to a total depth of 35 feet and completed with a 20-foot screened interval set to bracket the water table. MW-1 was installed in the former location of the Phase II ESA sampling point, TMW-4. Based on ground water quality violations detected in MW-1, Phase II LSA activities commenced at the site in April 2021. Two additional monitoring wells (MW-2 and MW-3) were installed at the site on April 7, 2021 to total depths of 35 feet BGS and completed with 20-foot screened intervals. Previously installed monitoring well from the 2002 LSA currently identified as MW-1A was sampled and surveyed during Phase II LSA activities. According to the well tag, MW-1A was installed in 2002 to a total depth of 40 feet BGS and completed with a 20-foot screened interval. The depths to water in MW-1, MW-2, MW-3 and MW-1A were 21.72 feet, 21.33 feet, 21.44 feet and 19.28 feet, respectively. Ground water elevations ranged from 78.24 feet to 79.66 feet in April 2021. Based on that, ground water flow direction is to the north/northwest. A Ground Water Flow Direction Map is included as Figure 4. 1. Soil Sampling: On September 16, 2020, three (3) soil samples (MW-1-5’, MW-1-10’ and MW-1-15’) were collected during the initial installation of monitoring well MW-1 on September 16, 2020. The soil samples were submitted for laboratory analyses of volatile organic compounds (VOCs) by SW846 Method 8260B and for volatile petroleum hydrocarbons (VPH) using the MADEP Method. With the exception of an estimated concentration of acetone in MW-1-5’, no other detectable concentrations of requested method constituents were reported in the soil samples. The concentration of acetone did not exceed the soil-to-ground water maximum contaminant concentration (MCC). No additional soil samples were collected during Phase II LSA activities. 7 2. Ground Water Sampling: On September 22, 2020, a ground water sample was collected from MW-1 and submitted for laboratory analyses of VOCs by EPA Method 6200B, VPH/EPH by the MADEP Method, EDB by Method 504.1, 625 BNAs + 10 tics and lead. Concentrations of benzene, MTBE, EDB and lead that exceeded the MACs were reported in the ground water sample collected from MW-1. None of the concentrations exceeded the GCLs. In addition, detectable and/or estimated concentrations of other requested method constituents were reported in the ground water sample collected from MW-1; however, none of the concentrations exceeded the MACs nor 10 times the 2L ground water quality standards. On April 8, 2021, ground water samples were collected from MW-1 through MW-3 and MW-1A and submitted for laboratory analyses of VOCs by EPA Method 6200B, VPH by the MADEP Method, EDB by Method 504.1 and lead. Concentrations of benzene, toluene, ethylbenzene, xylenes, naphthalene, EDB, 1,2-DCA, 2-Hexanone, isopropylbenzene, MIBK, n-propylbenzene, 1,2,4-trimethylbenzene, 1,3,5-trimethylbenzene, C5-C8 aliphatics and C9-C10 aromatics that exceeded the MACs were reported in the ground water sample collected from MW-3. A concentration of MTBE that exceeded the MAC was also reported in the ground water sample collected from MW-2. The concentration of benzene in MW-3 also exceeded the GCL. Detectable and/or estimated concentrations of other requested method constituents were reported in the ground water samples collected from MW-1 through MW-3; however, none of the concentrations exceeded the MACs. A summary of ground water sample analytical results is presented in Table 6. A Ground Water Quality Map is included as Figure 5. A copy of the laboratory analytical report is presented as Appendix B. 3. Quality Control: The soil and ground water samples were immediately placed in laboratory-supplied containers, which were then placed in iced coolers. New disposable bailers were used to sample the monitoring well and new nitrile gloves were worn during soil and ground water sampling. Samples were logged using proper chain-of-custody procedures. The samples were transported to a North Carolina certified laboratory for analysis. 8 5. CONCLUSIONS AND RECOMMENDATIONS  The site contains a petroleum retail facility/convenience store. Land use in the area primarily consists of commercial properties mixed with residential properties.  During the September 2020 receptor survey, a total of thirteen (13) water supply wells (WSW-1 through WSW-13) were identified within 1,500-feet of the source area. One (1) potable well (WSW-1) is located approximately 480 feet of the release. Four (4) additional potable wells (WSW-2, WSW-5, WSW-10 and WSW-13) were identified between 700 feet and 1,200 feet from the source area. The remaining wells are presumed to be used for irrigation purposes only. County water is available to all properties in the surrounding area of the site.  Azusa Grotto Lake is located approximately 1,300 feet southwest of the source area. A stream is located approximately 1,300 feet east of the source area. Underground utilities in the source areas are not likely to intersect the water table.  An estimated concentration of acetone was reported in soil sample MW-1-5’ collected at the site in September 2020; however, the concentration did not exceed the MCC. Concentrations of benzene, MTBE, EDB and lead that exceeded the MAC were reported in the ground water sample collected from MW-1 in September 2020. None of the concentrations exceeded the GCLs.  Two additional Type II monitoring wells (MW-2 and MW-3) were installed at the site April 7, 2021. One existing monitoring well, labeled MW-1A was identified and utilized during Phase II LSA activities. Ground water elevations ranged from 78.24 feet to 79.66 feet in April 2021. Based on that, ground water flow direction is to the north/northwest.  Based on the April 2021 ground water sampling event, concentrations of benzene, toluene, ethylbenzene, xylenes, naphthalene, EDB, 1,2-DCA, 2-Hexanone, isopropylbenzene, MIBK, n- propylbenzene, 1,2,4-trimethylbenzene, 1,3,5-trimethylbenzene, C5-C8 aliphatics and C9-C10 aromatics that exceeded the MACs were reported in the ground water sample collected from MW- 3. A concentration of MTBE that exceeded the MAC was also reported in the ground water sample collected from MW-2. The concentration of benzene in MW-3 also exceeded the GCL.  Based on this information, the site risk should be classified as high risk. The presence of MTBE and EDB indicates that the release occurred prior to 2009. A CSA should be conducted at the site. 6. LIMITATIONS This report has been prepared for the exclusive use of Mr. John Hendley and Brewer-Hendley Oil Company, Inc. for specific application to the referenced site in Mecklenburg County, North Carolina. The assessment was conducted based on the scope of work and level of effort specified by NCDEQ and with resources 9 adequate only for that scope of work. Our findings have been developed in accordance with generally accepted standards of geology and hydrogeology practices in the State of North Carolina, available information and our professional judgment. No other warranty is expressed or implied. The data that are presented in this report are indicative of conditions that existed at the precise locations sampled and at the time the samples were collected.
