2.0 SITE HISTORY
Terracon completed this report to summarize the findings of assessment activities performed on October
28 ,2020, January 12, 2021, and February 12, 2021 at the site located at 610 West Main Street in
Jamestown, Guilford County, North Carolina (Exhibit 1). Terracon previously completed UST Closure
Activities at the site and provided our findings, conclusions and recommendations in a report dated
October 30, 2020 (Terracon Project No. 75207046). Terracon collected two soil samples beneath the
centerline, at approximately 10 feet below ground surface (bgs), of one 2,000-gallon UST in October
2020 (Exhibit 2 and Exhibit 3). The UST was located near the east side of the on-site building. The soil
sample collected beneath the southern end of the UST contained total petroleum hydrocarbons – diesel
range organics (TPH-DRO) at a concentration of 2,190 mg/kg, which exceeds the NCDEQ regulatory
action level of 100 mg/kg. TPH - gasoline range organics (TPH-GRO) were not detected above the
Method Detection Limit. Neither TPH-GRO nor TPH-DRO constituents were detected in the remaining
soil samples. The purpose of this report is to evaluate the current status of soil and groundwater quality at
the North State Telephone site in accordance with NCDEQ requirements.
3.0 RISK CHARACTERIZATION

drinking purposes? No

3. Is a water-supply well not used for drinking water (e.g., irrigation, washing cars, industrial cooling
water, filling swimming pools) located within 250 feet of the source area of the release or
discharge? Yes, one water-supply well is located at 607 West Main Street
approximately 250 feet south of the former UST location on the site. According the City
of Jamestown Water Department, the landowner has a water account, but may also still
use the water supply well for other purposes.
4. Does groundwater within 500 feet of the source area of the discharge or release have the
potential for future use in that there is no other source of water supply other than the
groundwater? No
5. Do vapors from the discharge or release pose a threat of explosion due to an accumulation of the
vapors in a confined space or pose any other serious threat to public health, public safety or the
environment? No
6. Are there any other factors that would cause the discharge or release to pose an imminent
danger to public health, public safety, or the environment? No
Intermediate Risk Criteria
1. Is a surface water body located within 500 feet of the source area of the release? Yes, High Point
Lake is located approximately 420 feet north of the source area of the release. The
maximum groundwater contaminant concentration did not exceed the surface water
quality standard and criteria found in 15A NCAC 2B .0200 by a factor of 10.
2. Is the source area of the release located within a designated wellhead protection area as defined in
42 USC 300h-7(e)? No
3. Is the release located in the Coastal Plain region as designated on the map entitled “Geology of
North Carolina” published by the NCDENR in 1985? No
4. Do the levels of groundwater contamination for any contaminant exceed the gross contamination
levels established by the NCDEQ? No
Part II – Land Use
The questions in Part II pertain to the “source area” property.
1. Does the property contain one or more primary or secondary residences (permanent or
temporary)? No
2. Does the property contain a school, daycare center, hospital, playground, park, recreation area,
church, nursing home, or other place of public assembly? No

3. Does the property contain a commercial (e.g., retail, warehouse, office/business space) or
industrial (e.g., manufacturing, utilities, industrial research and development, chemical/petroleum
bulk storage) enterprise, an inactive commercial or industrial enterprise, or is the land
undeveloped? Yes: The property is currently occupied by a telecommunications
company (North State Telephone Company) office building.
4. Do children visit the property? Yes, occasionally children accompany adult patrons of the
company.
Is access to the property reliably restricted consistent with its use (such as fences, security
personnel or both)? Access to the site is open to the public.
5. Do pavements, buildings, or other structures cap the contaminated soil? The former UST was
located in a planter area that is being converted to an asphalt parking lot.
If yes, what mechanisms are in place or can be utilized to ensure that the contaminated soil will
remain capped in the foreseeable future? Based on conversations with the property owner,
the site will continue to be used as an office building for the foreseeable future.
6. What is the zoning status of the property? The property is currently zoned as
Commercial/Main Street Transitional (C/MST).
7. Is the use of the property likely to change in the next 20 years? No. Based on conversations
with the property owner, the business will continue to operate under similar conditions.
The questions below pertain to nearby properties within 1,500 feet of the source area of the source area
(excluding the property containing the source area of the release).
1. What is the distance from the source area of the release to the nearest primary or secondary
residence (permanent or temporary)? The nearest residences are located within Parkview
Terrace Apartments, approximately 170 feet south of West Main Street and the source
area (Exhibit 2).
2. What is the distance from the source area of the release to the nearest school, daycare center,
hospital, playground, park, recreation area, church, nursing home or other place of public
assembly? The north-adjacent City Lake Park (including park, dock, swimming pool, and
community center) is located approximately 380 feet north and 420 feet east of the
source area of the release. Restoration Medical Clinic and Wake Forest Baptist Health
Internal Medicine are located immediately adjacent to the west and east of the site,
respectively. Forest Grove Baptist Church is located approximately 680 feet southwest
of the source area of the release.

3. What is the zoning status of properties in the surrounding area?
Surrounding properties are zoned as Commercial/Main Street Transitional (C/MST),
Civic (CIV), RS-9-Residential- moderate to high density single-family dwellings (SFR),
Light Industrial (LI), Multi-family Residential (MFR), Commercial (C), and Public and
Institutional (PI). Properties zoned as PI are generally located to the east of the site,
while those zoned as C, CIV, CMST, and LI are located to the west of the site adjacent to
West Main Street. Several MFR and SFR communities are located west and south of the
site.
4. Briefly characterize the use and activities of the properties in the surrounding area.
Land use for the surrounding area consists of a recreational facility (e.g., park, dock,
swimming pool, and community center) to the north and east, single-family residences
to the south, multi-family residences to the west, as well as commercial and light
industrial businesses to the east, west and south of the site.
4.0 RECEPTOR INFORMATION
4.1 Water Supply Wells
Terracon completed a walking/driving reconnaissance in the site vicinity on February 16, 2021. Based on
our review of information obtained from the site vicinity reconnaissance and the Guilford County
Geographic Information System (GIS) website, Terracon did identify one water supply well located at 607
West Main Street, approximately 250 feet south of the former UST location on site. The site and
surrounding areas receive public water supplies from the Town of Jamestown. According to Town of
Jamestown Utilities Department staff, municipal water is available to the site area.
4.2 Public Water Supplies
The site and surrounding areas are serviced by the Town of Jamestown. According to Town of
Jamestown Utilities Department staff, municipal water is available to the site area. The sources of water
are High Point Lake and the Deep River. High Point Lake is fed by the Deep River and Oak Hollow Lake.
High Point lake is located approximately 420 feet north of the site while the Deep River is located
approximately 1,300 feet east of the site. Municipal water sources are not located near the site.
4.3 Surface Water
Based on a review of the USGS Topographic Map, the site is located within the Cape Fear River Basin
and Deep River Sub-basin. At its closest point, High Point Lake is located about 450 feet north of the
site. Surface waters are depicted on Exhibit 1.
4.4 Wellhead Protection Areas

According to the NCDEQ public water supply website, the source area for the release is not located
within 1,500 feet of a wellhead protection area as defined in 42 USC 300h-7(e).

4.5 Deep Aquifers in the Coastal Plain Physiographic Province
The site is not located within the Coastal Plain Physiographic Province.
4.6 Subsurface Structures
Subsurface structures beneath and adjacent to the site include utility lines (municipal water, electrical,
sewer, and natural gas) and an underground stormwater culvert running through the northwest portion of
the site.
4.6 Property Owners and Occupants
The site consists of one parcel of land located at 610 West Main Street in Jamestown, Guilford County,
North Carolina (Exhibit 1). The parcel (Guilford County GIS Parcel No. 7811879952) comprises 3.10-
acre of land developed with a 7,700 square foot North State Telephone office building and paved parking
area, constructed in 1967. The site and surrounding properties are depicted on Exhibit 2.
5.0 ASSESSMENT INFORMATION
5.1 Site Geology and Hydrogeology
The following resources were reviewed to evaluate site-specific geological and hydrogeological
characteristics:
USGS 7.5-minute series topographic map, High Point East, North Carolina Quadrangle, dated
2002 (Exhibit 1);
Geologic Map of North Carolina, dated 1985;
The Geology of the Carolinas, Carolina Geological Society Fiftieth Anniversary Volume, edited by
J. Wright Horton, Jr., and Victor A. Zullo, The University of Tennessee Press, dated 1991.; and
US Department of Agriculture, Natural Resources Conservation Service, Soil Survey of Guilford
County, North Carolina, Web Soil Survey via the Internet,
http://websoilsurvey.nrcs.usda.gov/app/.
Based on the Geologic Map of North Carolina, the site is located within the Carolina Slate Belt of the
Piedmont Physiographic Province. The Piedmont Physiographic Province generally consists of low to
medium elevation, rounded hills and long rolling northeast to southwest trending ridges with incised
creeks and river channels. The site is underlain by metamorphosed granitic rock. Soil lithology at the site
was evaluated from macrocore samples obtained during the soil boring process. The soils consisted of
reddish brown to tannish brown sandy silt or silt with sand soils. No recovery was obtained at MW-01

from 23 feet bgs to boring termination due to the presence of partially weathered rock. Free product was
not observed in the monitoring well.
5.2 Soil Boring Activities
On January 12, 2021, one soil boring was advanced using direct-push drilling techniques. Soil samples
were collected from microcores. Photo-ionization detector (PID) readings of the soils from the boring
ranged from less than 0.1 parts per million (ppm, referenced to an isobutylene standard) to 0.3 ppm. PID
readings are included within the boring log provided in Appendix A. A total of three (3) soil samples
were selected for laboratory analysis:
◼ TMW-01 at 10 feet bgs;
◼ TMW-01 at 15 feet bgs; and
◼ TMW-01 at 20 feet bgs
The boring was terminated at a depth of approximately 23 feet bgs due to the presence of partially
weathered rock. Groundwater was not encountered in the boring.
5.3 Groundwater Monitoring Well Installation
On February 12, 2021, Terracon installed a 1-inch diameter temporary monitoring well (TMW-01) to the
east of the former UST. The temporary monitoring well was installed using air-rotary drilling methods.
The temporary monitoring well was installed in the borehole as described below.
◼ Installation of 5 feet of one-inch diameter, 0.010-inch machine slotted sch. 40 PVC well screen
with a threaded bottom cap
◼ Installation of one-inch diameter, threaded, flush-joint PVC riser pipe to surface
◼ Addition of pre-sieved 20/40 grade silica sand for annular sand pack around the well screen from
the bottom of the boring to approximately one to two feet above the top of the well screen
◼ Addition of approximately one to two feet of hydrated bentonite chips on top of the sand pack
Following installation of the temporary well, the well was developed by purging until the discharge water
flowed relatively clear. Drill cuttings derived from the advancement of the boring were spread on the site.
The well construction details are summarized on Table 1.
6.0 SAMPLING ACTIVITIES
6.1 Soil Sampling Activities
The soil samples were placed into laboratory-supplied glassware, labeled, stored in an insulated container
and packed with ice. The samples, along with chain-of-custody documentation, were delivered to Pace
National Center for Testing and Innovation (Pace National) in Mount Juliet, Tennessee for analysis of

volatile organic compounds (VOCs)by USEPA Method 8260, semi-volatile organic compounds (SVOCs)
by USEPA Method 8270E, and volatile and extractable petroleum hydrocarbons (VPH and EPH) by
MADEP methods.
6.2 Groundwater Sampling Activities
Prior to sample collection, Terracon gauged the groundwater level. During the sampling event, depth to
groundwater was 34 feet bgs in monitoring well TMW-01.
The monitoring well was purged using a peristaltic pump using low flow sampling techniques (i.e., <200
milliliters per minute) prior to sampling. The following parameters were measured during purging: pH,
temperature, oxidation reduction potential (ORP), dissolved oxygen (DO), and conductivity. After the
wells were purged, a groundwater sample was collected from each well in general accordance with
NCDEQ sampling guidelines. The groundwater sample was collected directly into laboratory-supplied
glassware, labeled and stored in an ice-packed cooler. The sample, along with chain-of-custody
documentation, were delivered to Pace National in Mount Juliet, Tennessee for analysis of VOCs by
USEPA Method 8260, SVOCs by USEPA Method 8270, and VPH and EPH by MADEP methods. Site
photographs and field notes are provided in Appendix B.
6.3 Soil Sample Analytical Results
Soil sample analytical results were compared to the NCDEQ, UST Section Guidelines for Assessment
and Corrective Action, dated 2008 (updated January 2021). The results are summarized below:
◼ Targeted analytes were not detected in soil samples from boring TMW-01 at concentrations
exceeding the laboratory Minimum Detection Limits (MDLs), Soil-to-Water MSCCs, or
Industrial/Commercial Cleanup Standards.
An analytical soil sample results map is presented as Exhibit 4. The laboratory report is provided in
Appendix C.
6.4 Groundwater Analytical Results
Groundwater analytical results were compared to their respective 15A NCAC 2L Groundwater Quality
Standards (2L Standards) and Gross Contamination Levels (GCLs). The results are summarized below:
◼ Extractable petroleum constituents including C9-C22 aliphatics and C19-C36 aliphatics were
detected in the groundwater sample from TMW-01 at concentrations exceeding the laboratory
MDLs, but below their 2L Standards.
◼ Detected analytes did not exceed their respective GCLs and free product was not detected in the
well.

7.0 CONCLUSIONS AND RECOMMENDATIONS
Based on information obtained during the completion of these assessment activities, Terracon provides
the following conclusions and findings:
One non-potable water supply well was identified approximately 250 feet south of the site;
Soil samples collected as part of the LSA did not contain petroleum constituents at
concentrations exceeding the laboratory MDLs;
The groundwater sample collected on the site contained the EPH constituents C9-C22 aliphatics
and C19-C36 aliphatics at concentrations exceeding the laboratory MDLs but below the 2L
Standards.
Based on the findings of our assessment activities, the following recommendations are provided for the site.
Based on the soil and groundwater sample analytical results being below their respective unrestricted
use action levels, Terracon requests closure of the petroleum release incident with no additional
remedial action.

appropriate preservatives, and within method specified holding times, unless qualified or notated within
the report. Where applicable, all MDL (LOD) and RDL (LOQ) values reported for environmental samples
have been corrected for the dilution factor used in the analysis. All Method and Batch Quality Control
are within established criteria except where addressed in this case narrative, a non-conformance form

Guide to Reading and Understanding Your Laboratory Report
The information below is designed to better explain the various terms used in your report of analytical results from the Laboratory. This is not
intended as a comprehensive explanation, and if you have additional questions please contact your project representative.
Results Disclaimer - Information that may be provided by the customer, and contained within this report, include Permit Limits, Project Name,
Sample ID, Sample Matrix, Sample Preservation, Field Blanks, Field Spikes, Field Duplicates, On-Site Data, Sampling Collection Dates/Times, and
Sampling Location. Results relate to the accuracy of this information provided, and as the samples are received.
Abbreviations and Definitions
(dry)Results are reported based on the dry weight of the sample. [this will only be present on a dry report basis for soils].
MDL Method Detection Limit.
ND Not detected at the Reporting Limit (or MDL where applicable).
RDL Reported Detection Limit.
RDL (dry)Reported Detection Limit.
Rec.Recovery.
RPD Relative Percent Difference.
SDG Sample Delivery Group.
(S)
Surrogate (Surrogate Standard) - Analytes added to every blank, sample, Laboratory Control Sample/Duplicate and
Matrix Spike/Duplicate; used to evaluate analytical efficiency by measuring recovery. Surrogates are not expected to be
detected in all environmental media.
U Not detected at the Reporting Limit (or MDL where applicable).
Analyte The name of the particular compound or analysis performed. Some Analyses and Methods will have multiple analytes
reported.
Dilution
If the sample matrix contains an interfering material, the sample preparation volume or weight values differ from the
standard, or if concentrations of analytes in the sample are higher than the highest limit of concentration that the
laboratory can accurately report, the sample may be diluted for analysis. If a value different than 1 is used in this field, the
result reported has already been corrected for this factor.
Limits
These are the target % recovery ranges or % difference value that the laboratory has historically determined as normal
for the method and analyte being reported. Successful QC Sample analysis will target all analytes recovered or
duplicated within these ranges.
Original Sample The non-spiked sample in the prep batch used to determine the Relative Percent Difference (RPD) from a quality control
sample. The Original Sample may not be included within the reported SDG.
Qualifier
This column provides a letter and/or number designation that corresponds to additional information concerning the result
reported. If a Qualifier is present, a definition per Qualifier is provided within the Glossary and Definitions page and
potentially a discussion of possible implications of the Qualifier in the Case Narrative if applicable.
Result
The actual analytical final result (corrected for any sample specific characteristics) reported for your sample. If there was
no measurable result returned for a specific analyte, the result in this column may state “ND” (Not Detected) or “BDL”
(Below Detectable Levels). The information in the results column should always be accompanied by either an MDL
(Method Detection Limit) or RDL (Reporting Detection Limit) that defines the lowest value that the laboratory could detect
or report for this analyte.
Uncertainty
(Radiochemistry)Confidence level of 2 sigma.
Case Narrative (Cn)
A brief discussion about the included sample results, including a discussion of any non-conformances to protocol
observed either at sample receipt by the laboratory from the field or during the analytical process. If present, there will
be a section in the Case Narrative to discuss the meaning of any data qualifiers used in the report.
Quality Control
Summary (Qc)
This section of the report includes the results of the laboratory quality control analyses required by procedure or
analytical methods to assist in evaluating the validity of the results reported for your samples. These analyses are not
being performed on your samples typically, but on laboratory generated material.
Sample Chain of
Custody (Sc)
This is the document created in the field when your samples were initially collected. This is used to verify the time and
date of collection, the person collecting the samples, and the analyses that the laboratory is requested to perform. This
chain of custody also documents all persons (excluding commercial shippers) that have had control or possession of the
samples from the time of collection until delivery to the laboratory for analysis.
Sample Results (Sr)
This section of your report will provide the results of all testing performed on your samples. These results are provided
by sample ID and are separated by the analyses performed on each sample. The header line of each analysis section for
each sample will provide the name and method number for the analysis reported.
Sample Summary (Ss)This section of the Analytical Report defines the specific analyses performed for each sample ID, including the dates and
times of preparation and/or analysis.
Qualifier Description
C3 The reported concentration is an estimate. The continuing calibration standard associated with this data responded low.
Method sensitivity check is acceptable.
J The identification of the analyte is acceptable; the reported value is an estimate.
J3 The associated batch QC was outside the established quality control range for precision.
J4 The associated batch QC was outside the established quality control range for accuracy.
J6 The sample matrix interfered with the ability to make any accurate determination; spike value is low.

All sample aliquots were received at the correct temperature, in the proper containers, with the
appropriate preservatives, and within method specified holding times, unless qualified or notated within
the report. Where applicable, all MDL (LOD) and RDL (LOQ) values reported for environmental samples
have been corrected for the dilution factor used in the analysis. All Method and Batch Quality Control
are within established criteria except where addressed in this case narrative, a non-conformance form

Guide to Reading and Understanding Your Laboratory Report
The information below is designed to better explain the various terms used in your report of analytical results from the Laboratory. This is not
intended as a comprehensive explanation, and if you have additional questions please contact your project representative.
Results Disclaimer - Information that may be provided by the customer, and contained within this report, include Permit Limits, Project Name,
Sample ID, Sample Matrix, Sample Preservation, Field Blanks, Field Spikes, Field Duplicates, On-Site Data, Sampling Collection Dates/Times, and
Sampling Location. Results relate to the accuracy of this information provided, and as the samples are received.
Abbreviations and Definitions
MDL Method Detection Limit.
ND Not detected at the Reporting Limit (or MDL where applicable).
RDL Reported Detection Limit.
Rec.Recovery.
RPD Relative Percent Difference.
SDG Sample Delivery Group.
(S)
Surrogate (Surrogate Standard) - Analytes added to every blank, sample, Laboratory Control Sample/Duplicate and
Matrix Spike/Duplicate; used to evaluate analytical efficiency by measuring recovery. Surrogates are not expected to be
detected in all environmental media.
U Not detected at the Reporting Limit (or MDL where applicable).
Analyte The name of the particular compound or analysis performed. Some Analyses and Methods will have multiple analytes
reported.
Dilution
If the sample matrix contains an interfering material, the sample preparation volume or weight values differ from the
standard, or if concentrations of analytes in the sample are higher than the highest limit of concentration that the
laboratory can accurately report, the sample may be diluted for analysis. If a value different than 1 is used in this field, the
result reported has already been corrected for this factor.
Limits
These are the target % recovery ranges or % difference value that the laboratory has historically determined as normal
for the method and analyte being reported. Successful QC Sample analysis will target all analytes recovered or
duplicated within these ranges.
Original Sample The non-spiked sample in the prep batch used to determine the Relative Percent Difference (RPD) from a quality control
sample. The Original Sample may not be included within the reported SDG.
Qualifier
This column provides a letter and/or number designation that corresponds to additional information concerning the result
reported. If a Qualifier is present, a definition per Qualifier is provided within the Glossary and Definitions page and
potentially a discussion of possible implications of the Qualifier in the Case Narrative if applicable.
Result
The actual analytical final result (corrected for any sample specific characteristics) reported for your sample. If there was
no measurable result returned for a specific analyte, the result in this column may state “ND” (Not Detected) or “BDL”
(Below Detectable Levels). The information in the results column should always be accompanied by either an MDL
(Method Detection Limit) or RDL (Reporting Detection Limit) that defines the lowest value that the laboratory could detect
or report for this analyte.
Uncertainty
(Radiochemistry)Confidence level of 2 sigma.
Case Narrative (Cn)
A brief discussion about the included sample results, including a discussion of any non-conformances to protocol
observed either at sample receipt by the laboratory from the field or during the analytical process. If present, there will

Quality Control
Summary (Qc)
This section of the report includes the results of the laboratory quality control analyses required by procedure or
analytical methods to assist in evaluating the validity of the results reported for your samples. These analyses are not
being performed on your samples typically, but on laboratory generated material.
Sample Chain of
Custody (Sc)
This is the document created in the field when your samples were initially collected. This is used to verify the time and
date of collection, the person collecting the samples, and the analyses that the laboratory is requested to perform. This
chain of custody also documents all persons (excluding commercial shippers) that have had control or possession of the
samples from the time of collection until delivery to the laboratory for analysis.
Sample Results (Sr)
This section of your report will provide the results of all testing performed on your samples. These results are provided
by sample ID and are separated by the analyses performed on each sample. The header line of each analysis section for
each sample will provide the name and method number for the analysis reported.
Sample Summary (Ss)This section of the Analytical Report defines the specific analyses performed for each sample ID, including the dates and
times of preparation and/or analysis.
Qualifier Description
B The same analyte is found in the associated blank.
C3 The reported concentration is an estimate. The continuing calibration standard associated with this data responded low.
Method sensitivity check is acceptable.
J The identification of the analyte is acceptable; the reported value is an estimate.
J3 The associated batch QC was outside the established quality control range for precision.
J5 The sample matrix interfered with the ability to make any accurate determination; spike value is high.
J6 The sample matrix interfered with the ability to make any accurate determination; spike value is low.
