(ATC) has completed a Groundwater Monitoring Report (GMR), documenting the February 4, 2021, sampling of five monitoring wells and one water supply well associated with the Moody’s Grocery incident, located at 912 Old Highway 21, State Road, Wilkes County, North Carolina. The GMR is being performed based on a January 1, 2021, Task Authorization (TA-1) by the Trust Fund Branch of the Underground Storage Tank (UST) Section, Division of Waste Management, North Carolina Department of Environmental Quality (NCDEQ). The work is being performed as part of the State Lead Contract #N122019A between NCDEQ and ATC. Former petroleum USTs at the site released product to the subsurface prior to their decommissioning in 1998 and subsequent removal in 2000. During January 2019 site activities, ATC conducted soil gas sampling in MW-1, and installed Provectus® Oxygen Releasing Socks (ORS) in MW-1. The approved work included the installation of one monitoring well (MW-5); the measurement of the top of casing of MW-5; the sampling of wells MW-1 and MW-5; the measurement of water levels in wells MW-2, MW-3, and MW-4; and the sampling of five water supply wells (WSW 1, WSW-2, WSW-4, WSW-937, and WSW-955). On February 20 and 21, 2020, ATC installed groundwater monitoring well MW-5, at a location downgradient of the former UST basin. On February 21, 2020, ATC gauged and sampled groundwater monitoring wells MW-4 and MW-5. On April 10, 2020, ATC sampled monitoring well MW-1 and measured water levels in the five monitoring wells. ATC also sampled water supply well WSW-1 on February 20, 2020. The scope of work for TA-01 included the sampling of monitoring wells MW-1, MW-2, MW-3, MW-4, and MW-5, and water supply wells WSW-1 and WSW-2. All work was completed except the sampling of water supply well WSW-2, since the well was not functioning (power shut off by owner). All samples collected were analyzed using SM 6200B, and the water supply well sample WSW-1, field blank and trip blank were also analyzed using EPA Method 504.1 for ethylene dibromide (EDB). Based on the recent and historical assessment activities, the following site conditions are noted:  Municipal water is not available to the area. Twenty-one water supply wells are reported to be located within 1,000 feet of the source area, including nine within 500 feet of the source area. Two of the wells are considered to be most at risk: WSW-1 and WSW-2;  Light non-aqueous phase liquid (LNAPL) was not detected in the monitoring wells;  Benzene and EDB concentrations in well MW-5 exceeded the GCLs;  Multiple VOCs exceeded the 2L Standards in wells MW-1 and MW-5;  VOCs were not detected above the 2L Standards in well MW-3;  VOCs were not detected in monitoring wells MW-2 and MW-4, or in water supply well WSW-1;  The depth to groundwater ranged from approximately 32 feet below ground surface (ft-bgs) in the monitoring wells in the source areas and approximate 26 to 27 ft-bgs in wells MW-4 and MW-5 (lower groundwater surface elevations). Based on the February 4, 2021 groundwater elevations, groundwater flow is to the southeast; and  Based on the analytical results from monitoring well MW-5 the extent of VOCs in groundwater is not delineated vertically, or laterally to the east and southeast of MW-1 Groundwater Monitoring Report April 28, 2021 Moody’s Grocery, Incident # 22446, State Road, Wilkes Co., NC ATC Project #SLC2244601 3 and MW-5. In addition, the VOC concentrations in MW-5 are approximately 9 times higher than those in MW-1. Based on the well MW-5 sampling results and the depth to groundwater in well MW-5, ATC recommends the installation and sampling of five additional monitoring wells. These additional wells include one shallow well (MW-5S) adjacent to MW-5 to be screened across the groundwater surface to evaluate possible higher concentrations in the shallow groundwater; one deeper well (DMW-5) adjacent to MW-5 to evaluate the vertical extent of groundwater impact; one shallow well (MW-6) west of MW-1 to evaluate possible higher concentrations in the source area; and two shallow wells to the east of MW-1 (MW-7) and to the southeast of MW-5 (MW-8) to evaluate the horizontal extent of groundwater impact. Air rotary drilling will be needed for the installation of the deep monitoring wells. Subsequent assessment and remediation activities would be evaluated following receipt of the groundwater quality data from the new monitoring wells. Analytical results from soil samples obtained during UST removal in May 2000 indicated petroleum impact above the state action level of 10 milligrams per kilogram (mg/kg) in the area of the UST basin for the two 3,000-gallon gasoline USTs, one 550-gallon kerosene UST, 550- gallon UST of unknown content and in the vicinity of the kerosene dispenser. In December 2002, monitoring wells MW-1 and MW-2 were installed to a depth of 60 feet below ground surface. Soil samples obtained during the well installation were analyzed by Methods 8260 and 8270. Multiple constituents were found to be present in excess of their Soil to Groundwater MSCCs. ATC recommends screening soils with a PID during the installation of MW-5S, MW-6. Soil samples will be selected for analysis based on PID screening results for analysis by Methods 8260 and 8270, to evaluate the lateral extent of soil contamination. 3.0 SITE DESCRIPTION 3.1 Site Characterization The NCDEQ retained ATC to provide environmental consulting services for Moody’s Grocery, located at 912 Old Highway 21, in State Road, Wilkes County, North Carolina. Figure 1 depicts the site on a topographic map. The site currently operates as a retail convenience store that no longer has retail petroleum operations. The site is located near the intersection of Mining Church Road and Old Highway 21. 3.2 Site Background The site was developed with a single-story gasoline station in circa 1962, and is currently occupied by Moody’s Grocery. Historical records indicate that Moody’s Grocery was a retail petroleum facility until 1998, including one 6,000-gallon gasoline UST, two 3,000-gallon gasoline USTs, and one 550-gallon kerosene UST and associated piping and dispensers. The UST system ceased operation in 1998 and was removed in March 2000. During the 2000 UST removal, a 550-gallon UST of unknown content was found below the dispenser island. According to the property owner, Don Moody, the UST had been taken out of service in 1962 due to non-use. Soil analytical results indicated petroleum impact above the state action level of 10 milligrams per kilogram (mg/kg) in the area of the UST basin for the two 3,000-gallon gasoline USTs, one 550-gallon kerosene UST, and 550-gallon UST of unknown content. Groundwater Monitoring Report April 28, 2021 Moody’s Grocery, Incident # 22446, State Road, Wilkes Co., NC ATC Project #SLC2244601 4 During a modified Limited Site Assessment (LSA) in December 2002, monitoring wells MW-1 and MW-2 were installed to a depth of 60 feet below ground surface. Monitoring well MW-1 was installed in the former gasoline UST basin and monitoring well MW-2 was installed in the former kerosene UST basin. Groundwater analytical results from both monitoring wells indicated target petroleum constituents exceeded the 2L Standards. In addition, a well receptor survey identified 31 potable wells within 1,500 feet of the source area, including on-site potable well WSW-1. Because municipal water was not supplied to the area and due to the groundwater impact, the site was ranked as High Risk. Between 2002 and 2005, additional assessment was performed that included monitoring of the two site monitoring wells, water supply well WSW-1, and six nearby water supply wells. Groundwater analytical results from 2005 indicated detectable concentrations of naphthalene and total xylenes in well WSW-1 that did not exceed the 2L Standard. Groundwater impact above 2L Standards for benzene and 1,2-dichloroethane were identified in potable well WSW-6, located approximately 500 feet east southeast of the source area, and 2L Standards were exceeded for benzene, methyl tertiary butyl ether (MTBE), 1,4-dicholorobenzene, and 1,2- dichloroethane for potable well WSW-2, located approximately 170 feet east of the source area. Due to the impact in the potable wells, a point of entry (POE) filtration system was installed by the NCDEQ on wells WSW-2 and WSW-6. A March 2008 NCDEQ correspondence indicated that the NCDEQ planned to remove the POE filtration system from well WSW-6 because petroleum concentrations had not been detected since February 2008. The POE system was removed on August 5, 2013. ATC performed groundwater monitoring at the site in July 2014. Groundwater impact was identified above 2L Standards in monitoring wells MW-1 and MW-2. None of the water supply wells that were sampled contained impact above 2L Standards. An updated receptor survey was completed identifying 32 water supply wells within 1,500 feet of the site, 21 of which are located within 1,000 feet of the site. Two water supply wells (WSW-4 and WSW-4616) are located downgradient and within 500 feet of the site. Five additional wells (WSW-5, WSW-250, WSW-255, WSW-259, and WSW-4562) are also located downgradient from the site, and are within 1,000 feet of the site. In July 2015, ATC completed a groundwater monitoring event for the site. Results from the groundwater monitoring event indicated remaining impact above 2L Standards in monitoring well MW-1. Other petroleum constituents in wells MW-1 and MW-2 were below 2L Standards. No petroleum constituents were detected in wells MW-3, MW-4, or in the three water supply wells (WSW-1, WSW-3, and WSW-6) which were sampled. In November, 2017, ATC completed a groundwater monitoring event which indicated remaining VOC impact above 2L Standards in monitoring wells MW-1 and MW-3. Following the sampling event, ATC installed ORS in monitoring well MW-1. On June 14, 2018, ATC collected groundwater samples from monitoring wells MW-1, MW-2, MW- 3, and MW-4. Benzene, 1,2-dichloroethane, ethylene dibromide (EDB), and isopropyl ether concentrations exceeded the 2L Standards in monitoring well MW-1. Note that the concentrations decreased by 37 percent (isopropyl benzene) to 87 percent (benzene) compared to the November 2017 sampling results, excluding EBD which was not detected in November 2017 (elevated detection limit). Groundwater Monitoring Report April 28, 2021 Moody’s Grocery, Incident # 22446, State Road, Wilkes Co., NC ATC Project #SLC2244601 5 On January 8, 2019, ATC collected a soil vapor sample from monitoring well MW-1 to assess remaining soil impact in the vadose zone soils. Following collection of the soil-gas sample, ATC installed new ORS in monitoring well MW-1. Based on the January 2019 work, ATC recommended installation of one additional monitoring well (MW-5) to further characterize groundwater impact; and conducting soil vapor extraction using a new SVE well near MW-1 in an effort to remove petroleum from the subsurface and expedite subsequent remediation by natural attenuation. On February 20 and 21, 2020, ATC installed groundwater monitoring well MW-5, at a location downgradient of the former UST basin. On February 21, 2020, ATC gauged and sampled groundwater monitoring wells MW-4 and MW-5. Monitoring well MW-1 was sampled on April 10, 2020 along with the measurement of water levels in all monitoring wells. Site water supply well WSW-1 was also sampled. Groundwater monitoring wells MW-1 and MW-5 contained VOCs in excess of the 2L standard and EDB was detected in MW-5 in excess of the GCL. Laboratory analysis of the sample obtained from WSW-1 did not identify any detectable concentrations of petroleum constituents. 3.3 Site and Regional Geology According to the Geologic Map of North Carolina, dated 1985, the property lies within the Blue Ridge Physiographic Province, in the Slate of Copperhill Formation of the Ocoee Supergroup. The metamorphic rock of the Slate of Copperhill Formation is comprised of slate, phyllite, and metagraywacke that indicates graded bedding. In most areas, the upper Blue Ridge subsurface consists of unconsolidated residual soil, variously known as residuum, saprolite, or partially weathered rock (PWR) that has developed from in-place weathering of the underlying metamorphic bedrock. The residual soils vary in consistency and depth based on local conditions. Bedrock can be found at the surface as outcrops and to depths of greater than 100 feet in areas of easily weathered rock types. Sites near streams and rivers in the Blue Ridge may also contain alluvium deposited during flood events. Boring logs from the May 2010 letter report by Terraine indicated that the site subsurface consisted of brown to red clay in the upper approximately 14 feet of subsurface soils, followed by reddish brown to yellowish brown silt or clayey silt from approximately 20 ft bgs to the termination of the boring, which varied from 52 ft bgs in boring MW-3 to 45 ft bgs in boring MW- 4. Bedrock was encountered at well MW-2 at a depth of 50 ft bgs. During the drilling for well MW-5, clayey silt and silt was observed to the termination depth of 50 ft bgs. Air rotary drilling was needed to drill deeper than 29 ft bgs, where auger refusal was encountered due to dense silt. Neither partially weathered rock nor bedrock were encountered, and depth to top of bedrock is unknown. 3.4 Site and Regional Hydrogeology The shallow aquifer occurrence varies in depth from ground surface at springs, creeks, and rivers to as deep as 50 feet or more beneath upland surfaces in some parts of the Blue Ridge. ATC encountered groundwater between 26.80 (MW-5) and 32.42 (MW2) feet below tops of casing (TOC) in the monitoring wells at the site during the February 4, 2021, monitoring event. Permeability varies with lithology and is typically relatively low in residual soils, with higher permeability in saprolite or PWR due to relict rock texture and variable susceptibility to weathering exhibited by different minerals in the rock. Groundwater flow in residual soils or Groundwater Monitoring Report April 28, 2021 Moody’s Grocery, Incident # 22446, State Road, Wilkes Co., NC ATC Project #SLC2244601 6 alluvium is usually in rough concurrence with local topographic conditions and is toward local drainage features. Based on the existing site wells, the groundwater flow appears to channel to the central part of the site and flows south-southeast. The bedrock fractures or other planar features of the Blue Ridge Physiographic Province generally constitute the bedrock aquifer, with the surrounding rock material being effectively impermeable. Along with fractures, contacts between rock bodies probably constitute zones of significant groundwater occurrence in the bedrock. The surrounding material and overlying residuum tend to make the bedrock aquifer a semi-confined aquifer. That is, the overlying water and soil weight normally results in pressure that causes water in a borehole, which intersects a fracture or other feature, to rise above the elevation of the fracture or feature. Most modern water supply wells in the Blue Ridge consist of a casing set through the residual soils into the upper bedrock followed by an open borehole into the bedrock where water bearing fractures were encountered during the drilling. Except for attempted sampling of nearby water supply wells, evaluation of bedrock water quality or hydrology was beyond the scope of the GMR. 4.0 GROUNDWATER CHARACTERIZATION 4.1 Water Supply Wells Twenty-one water supply wells have been identified within 1,000 feet of the site, with nine of the wells within 500 feet of the site. Groundwater flow at the site is to the south-southeast, with two most at-risk wells: on-site well WSW-1 and the well immediately to the east of the site, WSW-2. Figure 2 depicts the locations of the water supply wells on an aerial overlay with radial distances from the site. Table 1 summarizes water supply well and other receptor information. On February 4, 2021, ATC collected a groundwater sample from the on-site water supply well (WSW-1). Monitoring well MW-2 was not sampled since the well was not functioning (power shut off by owner). Field notes from the February 4, 2021 sampling event, Appendix A. Laboratory analysis of the water supply well sample did not identify any detectable concentrations of petroleum constituents. A summary of current and historical water supply well analytical data is included in Table 2. 4.2 Groundwater Levels On February 4, 2021, ATC measured the depth to groundwater in monitoring wells MW-1, MW- 2, MW-3, MW-4 and MW-5. Well construction details are presented in Table 3, and groundwater level and elevation data are presented in Table 4. The well locations are indicated on Figure 3. The depth to groundwater ranged from approximately 32 feet below ground surface (ft-bgs) in the monitoring wells in the source areas and approximate 26 to 27 ft-bgs in wells MW-4 and MW-5. LNAPL was not detected in the monitoring wells. Based on the results of the February 4, 2021 groundwater measurements, groundwater flow is to the southeast with a gradient of approximately 0.06 feet/foot. A groundwater elevation contour map is presented in Figure 4. Field notes from the February 2020 monitoring event are presented in Appendix A. 4.3 Groundwater Quality On February 4, 2021, ATC collected groundwater samples from monitoring wells MW-1, MW-2, MW-3, MW-4 and MW-5. During sampling activities, a three-volume well purge was implemented prior to groundwater sample collection to ensure a representative groundwater sample. The samples were collected in laboratory-supplied, preserved containers; entered into chain of custody control; placed on ice; and shipped to North Carolina accredited Pace Groundwater Monitoring Report April 28, 2021 Moody’s Grocery, Incident # 22446, State Road, Wilkes Co., NC ATC Project #SLC2244601 7 Analytical Services, Inc. in Huntersville, North Carolina. The water samples were analyzed by EPA Method 6200B for VOCs including petroleum constituents. The analytical report is included in Appendix B. Groundwater field parameters including temperature, dissolved oxygen, conductivity, pH, and oxidation-reduction potential were measured during the monitoring event. Field parameter readings for each well are presented in Table 5. The results do not indicate a pattern of field parameters with VOC concentrations. Relative low dissolved oxygen levels were observed in all wells excluding well MW-4, although higher dissolved oxygen levels were expected for wells MW-2 and MW-3 with VOC concentrations reported as less than reporting limits. Well MW-5 had the highest positive oxidation reduction potential even though the VOC concentrations in this well were higher than the other wells. Groundwater analytical results from the February 4, 2021 monitoring event are summarized in Table 6. Historical groundwater analytical results are shown in Table 7. The groundwater analytical results were compared to the 2L Standards and GCLs. Groundwater analytical results indicate the following:  Benzene and EDB concentrations in well MW-5 exceeded the GCLs;  Multiple VOCs exceeded the 2L Standards in wells MW-1 and MW-5;  VOCs were not detected above the 2L Standards in well MW-3; and  VOCs were not detected in monitoring wells MW-2 and MW-4, or in water supply well WSW-1. The February 2021 analytical results for wells MW-1 and MW-5 indicate a general increase in VOC concentrations compared to the February 2020 analytical results. Figure 5 depicts a groundwater quality map with detected constituents above 2L Standards. As part of ATC’s quality assurance and quality control (QA and QC) measures, a field blank was collected and a data review of the laboratory reports was conducted. During the review, it was found that the samples were received from the laboratory in acceptable condition and properly preserved. The samples were analyzed within correct holding times and reported within the lab report, and the chain of custody was properly completed by all parties. The laboratory QC report shows that an internal blank was analyzed for each method that was used for the original samples. The laboratory fortified blank (LFB) and laboratory control sample (LCS) data results were also provided for each analytical method. ATC collected a field blank during the February 4, 2021 groundwater monitoring activities. VOCs were not detected in the field blank, the trip blank, or the laboratory method blanks. Refer to the laboratory reports in Appendix B for details regarding QA results for the laboratory samples and any noted exceptions. Overall, ATC concludes that the laboratory results are usable and any exceptions do not significantly impact the conclusions presented in this report. 5.0 CONCLUSIONS AND RECOMMENDATIONS On behalf of the Trust Fund Branch of the UST Section, ATC has completed a GMR documenting the February 4, 2021, groundwater monitoring event for Moody’s Grocery, located at 912 Old Highway 21, State Road, Wilkes County, North Carolina.
