Dear Mr. Moody: The Winston-Salem Region Office (WSRO) referred the subject site to the State Underground Storage Tank (UST) Trust Fund Branch, State-Lead program for inclusion on the list of sites awaiting State-Lead cleanup pursuant to G.S. 143-215.94G(a). Two monitoring wells have been installed at the site. Review of analytical data from groundwater samples collected at the site indicates that it is necessary to install at least two additional monitoring wells. Review of the project file indicates that a signed access agreement is not on file with the Division. An access agreement granting permission to enter the property for the purpose of conducting the required assessment is attached for your signature.
