EXECUTIVE SUMMARY The purpose of this report is to evaluate the current status of the groundwater quality at the former Stephen’s Exxon site (Incident No. 16457) by sampling monitoring wells MW-5R, MW-13, MW-14, MW-15, MW-16, and MW-17. Terracon completed this report to summarize the findings of assessment activities performed on September 26, 2021. The release incident is situated on the property located at 20655 Highway 88 West in Creston, Ashe County, North Carolina in an area developed with rural residences. The site currently operates as a workshop for the current property owner. Terracon mobilized to the site on September 26, 2021, to conduct groundwater sampling activities. Terracon gauged the groundwater elevations in monitoring wells MW-5R, MW-13, MW-14, MW-15, MW-16, and MW-17. Depth to groundwater in the monitoring wells ranged from approximately 1.25 feet below top of casing (btoc) in well MW-16 to 7.65 feet btoc in well MW-15. Based on top of casing elevations, groundwater flow is to the southeast. In addition, monitoring well MW-11 was gauged, which has historically had various thicknesses of light non-aqueous phase liquids (LNAPL). Based on information obtained during the completion of our assessment activities, Terracon provides the following conclusions and findings. Based on water level measurements, groundwater flow is calculated to be towards the southeast. There are four private water supply wells (one inactive) and five water supply springs within a 1,500-foot radius of the site. Municipal water is not available to the site or the immediately surrounding properties. All past sampling of the three water supply wells (WSW-6, WSW-7, and WSW-8) located cross-gradient from the source release reported contaminant concentrations below the laboratory method detection limits (MDLs). LNAPL was observed in the monitoring well MW-11 (0.25 inches) and has decreased in thickness when compared to the last groundwater sampling event in October 2019. Petroleum constituents were not reported above GCLs in any of the sampled wells. N-propylbenzene was reported in wells MW-14 and MW-15 above the 2L Standards. Contaminants have generally decreased in concentration in monitoring well MW-5R when compared to historical groundwater sampling data. Contaminants have generally increased in concentration in wells MW-14 and MW-15, with certain concentration values exceeding 2L Standards, when compared to historical groundwater sampling data. Based on the data presented in this monitoring report, Terracon recommends the following activities be completed at the site. Install an air sparge well and an extraction well west of the site building near the former kerosene UST in an effort to remove LNAPL detected in well MW-11. Three months subsequent to installation of the air sparge well and extraction well, conduct a Mobile Multi-Phase Extraction (MMPE) air sparging event. Three months subsequent to the MMPE air sparging event, sample monitoring wells MW-11, MW-13, MW-14 and water supply well WSW-8. C. SITE HISTORY AND CHARACTERIZATION The purpose of this report is to evaluate the current status of the groundwater quality at the former Stephens Exxon site. Terracon completed this report to summarize the findings of assessment activities performed on September 26, 2021 at the former M&P Grocery located at 20655 Highway 88 West in Creston, Ashe County, North Carolina (Exhibit 1). This Groundwater Monitoring Report was prepared for the site in response to a request by the Incident Manager, Mr. Herb Berger with the NCDEQ Division of Waste Management, Trust Fund Branch. The Federal/State Lead Monitoring Report was prepared in general accordance with 15A NCAC 2L .0115 regulations and the NCDEQ Underground Storage Tank (UST) Section Assessment Guidelines, dated January 19, 2021, under the supervision of Mr. Mark I. Miller, a North Carolina licensed geologist (North Carolina License No. 1715). C.1 Site Setting The Stephen’s Exxon site is located at 20655 Highway 88 West in Creston, Ashe County, North Carolina. The site is bordered to the north by NC Highway 88 West with surrounding residential and agricultural parcels in a rural setting. The property has been developed with a single one-story structure that serves as a workshop for the current property owner with asphalt and concrete parking areas. Historically, the site operated as Joe Stephen’s Grocery that maintained three 2,000 -gallon USTs and one 560-gallon Kerosene UST. The site is depicted in Exhibit 2. Stephen’s Exxon Creston, North Carolina Terracon Project No. 75207132 Incident No. 16457 December 30, 2021 Responsive ■ Resourceful ■ Reliable 4 C.2 Release Summary According to historical assessment information, three 2,000-gallon gasoline USTs were installed in May 1975 and one 560-gallon Kerosene UST was installed in May 1976. The gasoline USTs were located near the northeast corner of the building on the property and the kerosene UST was located along the west side of the building. The gasoline USTs, most of the product lines, and approximately 100 cubic yards of contaminated soil were removed from the property in October 1996. Upon removal of the USTs, representatives from the Winston-Salem Regional Office, Division of Water Quality noted the soil at the excavation site to be discolored and odorous. C.3 Previous Investigations Upon removal of the three 2,000-gallon gasoline USTs in October 1996, soil samples were collected from the excavation site and the former fuel dispenser location by representatives of the Winston- Salem Regional Office, Division of Water Quality. Laboratory analysis of the soil samples yielded petroleum contaminants exceeding the North Carolina State Action Level of 10 parts per million. During the same month, Salem Environmental installed a monitoring well (MW-1) in the proximity of the former gasoline USTs which yielded groundwater with a distinct petroleum odor. Due to evidence of contamination, the North Carolina Department of Environment and Natural Resources (NCDENR) requested further investigation to evaluate the extent of the release. Between March and June 1997, Geonetics Corporation (Geonetics) installed five shallow monitoring wells (MW-2 through MW-6) around the building on the property, two temporary wells (TW-2 and TW-3) east of the building on the property, and a deep aquifer monitoring well (MW-7) south of the former UST location to further delineate the extent of petroleum impact on the site. Laboratory analysis from subsequent groundwater sampling in monitoring wells MW-1, MW-5, and TW-2 identified concentrations of petroleum constituents above Title 15A of the North Carolina Administrative Code subchapter 2L (15A NCAC 2L) groundwater standards, hereafter referred to as the 2L Standards. To further delineate petroleum contamination, Geonetics installed two additional shallow monitoring wells (MW-8 and MW-9) further east on the property towards the North Fork New River. In August 1998, Geonetics facilitated the removal of the 560-gallon Kerosene UST on the site along with approximately 7.3 cubic yards of contaminated soil. Laboratory analysis from collected soil samples reported concentrations that exceeded the Soil-to-Water Maximum Soil Contaminant Concentrations (MSCCs) with detections of volatile and semi-volatile total petroleum hydrocarbons in excess of the North Carolina State Action Level of 10 parts per million. Geonetics concluded that groundwater contamination was due to releases from the UST systems and submitted a Corrective Action Plan (CAP) that proposed further remedial excavations. To further delineate petroleum contamination, Geonetics installed one additional shallow monitoring well (MW-10) in the vicinity of the removed kerosene tank basin. Stephen’s Exxon Creston, North Carolina Terracon Project No. 75207132 Incident No. 16457 December 30, 2021 Responsive ■ Resourceful ■ Reliable 5 Between June and July 2003, Geonetics excavated approximately 832 tons of contaminated soil above the water table from the former UST basins and dispenser locations. Laboratory analysis from soils collected at the bottom of the excavation area indicated the continued presence of residual petroleum contaminants exceeding the Soil-to-Water Maximum Soil Contaminant Concentrations (MSCCs) several feet below the water table, as well as in the soil beneath the building on the property. To further delineate petroleum contamination, Geonetics installed two additional shallow monitoring wells (MW-11 and MW-12) around the vicinity of the on-site building. In March 2004, the NCDENR requested a soil excavation event near the former fuel dispenser island. Samples of soil collected from the bottom of the excavation pit yielded concentrations of benzene, toluene, ethylbenzene, and xylenes (BTEX) in excess of Soil-to-Water MSCCs. In May 2004, Geonetics conducted a groundwater monitoring event at the site. Laboratory analysis indicated the presence of elevated BTEX concentrations above 2L Standards in monitoring wells MW-5 and MW-11, which are located south of the former UST basin. In February 2011, Terraine Environmental (Terraine) conducted a groundwater monitoring event at the site. One foot of measurable light non-aqueous phase liquid (LNAPL) was present in monitoring well MW-11. Laboratory analysis confirmed the presence of petroleum contaminants in monitoring wells MW-2, MW-3, MW-4, MW-7, and MW-12. However, petroleum constituents in the groundwater samples did not contain analytes at concentrations exceeding 2L Standards. In August 2011, Terraine returned to the site to supervise an Aggressive Fluid Vapor Recovery (AFVR) event on monitoring well MW-11. After the event, approximately 175 gallons of petroleum impacted groundwater were recovered. Approximately 0.2 feet of LNAPL continued to be observed in MW-11. Laboratory analysis from groundwater sampling following the AFVR event indicated a decrease in petroleum impact in monitoring wells MW-4 and MW-12. Following sampling, Terraine installed a Universal Remediation, Inc., PRP Well Boom in monitoring well MW-11 in an effort to lower petroleum contaminant concentrations. Throughout 2012 and 2013, Crawford Environmental Services (CEM) conducted a series of groundwater monitoring events. Measurable amounts of LNAPL continued to be observed in monitoring well MW-11. Laboratory analysis identified detectable levels of petroleum contaminants in monitoring wells MW-3 and MW-12. In October 2014, ATC Associates of North Carolina, P.C. (ATC) conducted a groundwater monitoring event. Measurable amounts of LNAPL continued to be observed in monitoring well MW-11. Laboratory analysis from groundwater sampling identified multiple constituents exceeding 2L Standards in monitoring well MW-5. In August 2018, ATC returned to sample monitoring wells and identified concentrations of isopropyl ether and sec-butylbenzene below 2L Standards. Between March and October 2019, ATC installed five shallow monitoring wells (MW-13 through MW- 17) to further delineate the horizontal extent of the petroleum impact and installed shallow monitoring Stephen’s Exxon Creston, North Carolina Terracon Project No. 75207132 Incident No. 16457 December 30, 2021 Responsive ■ Resourceful ■ Reliable 6 well MW-5R near the location of MW-5 as it was no longer found on the site. Measurable LNAPL continued to appear in monitoring well MW-11. Laboratory analysis from groundwater sampling identified concentrations of petroleum constituents above 2L Standards in monitoring well MW-5R. ATC concluded that there appeared to be two plumes: one associated with kerosene (near MW-11) and another with gasoline (near MW-5R). In August 2021, Terracon Consultants, Inc. (Terracon) conducted a Mobile Multi-Phase Extraction (MMPE) event at monitoring well MW-7. A total of 3.77 gallons of mixed product and water were removed from the site. Approximately 8 gallons of free product was recovered as liquid and 3.33 gallons of Volatile Organic Compounds (VOCs) were removed as vapor. After the event, no free product was identified in any of the on-site monitoring wells. C.4 Water Supply Wells Based on receptor survey information provided by Geonetics in 2012, there are four private water supply wells (one-inactive) and five water supply springs within a 1,500-foot radius of the site. The on-site water supply well (WSW-8) is located approximately 60 feet southwest of the former kerosene UST release source area. Geonetics did not find evidence at the time of the assessment that the water supply springs or potential receptors were affected by the UST release. The locations of the water supply wells are shown on Exhibit 3 and water supply well information is provided in Table 1. In 2015, ATC identified and sampled three water supply wells and springs (WSW-6, WSW-7, and WSW-8) located within 1,000 feet from the site. Wells WSW-6 and WSW-7 are located cross- gradient and approximately 600 feet and 950 feet from the source release, respectively. Analytical results did not identify detectable concentrations of petroleum constituents in the tested water supply wells. In March 2019, ATC sampled water supply wells WSW-7 and WSW-8 and did not identify petroleum contaminants above MDLs. Historical water supply analytical groundwater results from previous sampling events are provided in Table 2. C.5 Surface Water A small intermittent stream runs along the western border of the property. The North Fork of the New River is located approximately 720 feet southeast of the former UST basin. The North Fork of the New River is depicted as Class C, Trout Waters. Surface runoff on the site is generally to southeast. A spring is located north of well MW-2 and is piped underground heading southeast towards the river. Surface waters are depicted on Exhibit 1. C.6 Land Use Information The site is bound to the north by NC Highway 88 and single-family residences, to the east by agricultural land and single-family residences, to the west by single-family residences, and to the south by agricultural land and the North Fork of the New River. ASSESSMENT INFORMATION D.1 Site Geology and Hydrogeology The following resources were reviewed to evaluate site-specific geological and hydrogeological characteristics. Baldwin Gap Quadrangle, North Carolina, USGS 7.5-minute series topographic map, dated 2003. (Exhibit 1). Geologic Map of North Carolina, dated 1985. The Geology of the Carolinas, Carolina Geological Society Fiftieth Anniversary Volume, edited by J. Wright Horton, Jr., and Victor A. Zullo, The University of Tennessee Press, dated 1991. Soil Survey of Ashe County, North Carolina, US Department of Agriculture, Natural Resources Conservation Service, Web Soil Survey via the Internet, http://websoilsurvey.nrcs.usda.gov/app/ The site is located within the Blue Ridge Physiographic Province which generally consists of tall, high-relief topography and long northeast to southwest trending ridges with incised creeks and river channels. According to the geologic map, the site is specifically underlain by an unconformity with bedrock consisting of biotite granitic gneiss. In-place chemical and physical weathering of parent rocks typically form residual soils in the Blue Ridge region. Weathering is facilitated by the presence of fractures, joints, and less resistant minerals in the rock. Saprolite derived from the underlying bedrock is generally thinner in the Blue Ridge than in the Piedmont. Furthermore, soils on mountain slopes are moved down-slope by water and gravity more readily than in the more moderately sloped Piedmont Physiographic Province, resulting in relatively thicker colluvial deposits in valleys. According to available excavation records for the site, soils encountered consisted of sandy silt to silty sand. In much of the Blue Ridge, groundwater generally occurs in pore spaces within soils and in the structural features present in the underlying rock (i.e. joints, fractures, and faults). Recharge to the water table is primarily by precipitation infiltrating the surficial, unsaturated soils (vadose zone) and percolating downward under the influence of gravity to the water table (saturated zone). Typically, the water table is not a level surface, but a subdued reflection of the land surface. The depth to the water table is not consistent and is dependent upon several factors that include the amount of rainfall, permeability of the soil, the extent of fracturing in the underlying rock and the influences of groundwater pumping. Groundwater typically flows in directions perpendicular to lines of equal head and under the influence of gravity, migrates to discharge points such as surface water features, the toes of slopes or natural springs. Please note that fluctuations in the depth of the water table can be expected depending on variations in precipitation, surface water run-off and other factors not evident at the time of our subsurface exploration. Normally, the highest levels of the water table occur in late winter and spring and the Stephen’s Exxon Creston, North Carolina Terracon Project No. 75207132 Incident No. 16457 December 30, 2021 Responsive ■ Resourceful ■ Reliable 8 lowest levels occur in the late summer and fall. Based on groundwater elevation measurements taken on September 26, 2021, groundwater is estimated to flow to the southeast toward the North Fork of the New River (Classified as Class C, Trout Waters). D.2 Sampling Activities Terracon mobilized to the site on September 26, 2021, to conduct groundwater sampling activities. Terracon gauged the groundwater elevations in monitoring wells MW-5R, MW-11, MW-13, MW-14, MW-15, MW-16, and MW-17. Depth to groundwater in the monitoring wells ranged from approximately 1.25 feet below top of casing (btoc) in well MW-16 to 7.65 feet btoc in well MW-15. Based on top of casing elevations, groundwater flow is to the southeast. D.2.1 LNAPL Evaluation Approximately 0.02 feet of LNAPL was detected in monitoring well MW-11. D.2.2 Groundwater Sampling Activities Terracon gauged the monitoring wells using the electric water level meter with accuracy of 1/100th of a foot to collect groundwater elevation data in monitoring wells MW-5R, MW-13, MW-14, MW-15, MW-16, and MW-17. Depth to groundwater in the monitoring wells ranged from approximately 1.25 feet below top of casing (btoc) in well MW-16 to 7.65 feet btoc in well MW-15. Approximately 0.25 inches of LNAPL was present in monitoring well MW-11, and therefore was not sampled. Based on top of casing elevations, groundwater flow is to the southeast. The approximate locations of the wells are shown on the Site Diagram (Exhibit 2). A groundwater elevation map depicting groundwater flow contours and direction is shown on Exhibit 4. Current and historical groundwater elevation measurements are provided in Table 3. The groundwater samples and a field blank were placed in laboratory-supplied glassware, labeled and stored in an ice-packed cooler. The samples, along with chain-of-custody documentation, were delivered to Pace Analytical Services, Inc. in Mount Juliet, Tennessee for analysis of VOCs by Standard Method 6200B, including MTBE, EDB and IPE. D.3 Analytical Results Based on the laboratory analytical results, wells MW-14 and MW-15 reported concentrations of n- propylbenzene exceeding the 2L Standards (158 µg/L and 101 µg/L, respectively). Wells MW-5R, MW-14, and MW-15 reported concentrations of 1,2,4-trimethylbenzene at concentrations exceeding the 2L Standards (630 µg/L, 797 µg/L, and 584 µg/L, respectively). Wells MW-5R, MW-14, and MW- 15 reported concentrations of total xylenes at concentrations exceeding the 2L Standards (737 µg/L, 845 µg/L, and 1,255 µg/L, respectively). Well MW-15 reported concentrations of ethylbenzene at a concentration exceeding the 2L Standards (788 µg/L). Various petroleum constituents were reported in wells MW-5R, MW-13, MW-14, MW15, and MW-17 above the MDLs but below the 2L Standards. When compared to historical groundwater data, analytical results from this sampling event in well MW-5R indicate a general decrease in contaminant concentrations, with certain analytes displaying concentrations below 2L standards. However, groundwater contaminant concentrations from this sampling event have generally increased in wells MW-14 and MW-15 since the installation and subsequent sampling of the wells in 2019. Monitoring wells MW-13, MW-16, and MW-17 all reported contaminant concentrations below 2L Standards. The groundwater analytical results from the sampled monitoring wells and the historical groundwater analytical results from previous sampling events are summarized in Table 4. The analytical results are also depicted on Exhibit 5. Isoconcentration maps of petroleum contaminant analytes on the site were not generated for this report due to insufficient analytical groundwater data collected for each contaminant plume. Copies of the laboratory analytical reports are provided in Appendix A. E. This is not intended as a comprehensive explanation, and if you have additional questions please contact your project representative. Results Disclaimer - Information that may be provided by the customer, and contained within this report, include Permit Limits, Project Name, Sample ID, Sample Matrix, Sample Preservation, Field Blanks, Field Spikes, Field Duplicates, On-Site Data, Sampling Collection Dates/Times, and Sampling Location. Results relate to the accuracy of this information provided, and as the samples are received. Abbreviations and Definitions MDL Method Detection Limit. ND Not detected at the Reporting Limit (or MDL where applicable). RDL Reported Detection Limit. Rec.Recovery. RPD Relative Percent Difference. SDG Sample Delivery Group. (S) Surrogate (Surrogate Standard) - Analytes added to every blank, sample, Laboratory Control Sample/Duplicate and Matrix Spike/Duplicate; used to evaluate analytical efficiency by measuring recovery. Surrogates are not expected to be detected in all environmental media. U Not detected at the Reporting Limit (or MDL where applicable). Analyte The name of the particular compound or analysis performed. Some Analyses and Methods will have multiple analytes reported. Dilution If the sample matrix contains an interfering material, the sample preparation volume or weight values differ from the standard, or if concentrations of analytes in the sample are higher than the highest limit of concentration that the laboratory can accurately report, the sample may be diluted for analysis. If a value different than 1 is used in this field, the result reported has already been corrected for this factor. Limits These are the target % recovery ranges or % difference value that the laboratory has historically determined as normal for the method and analyte being reported. Successful QC Sample analysis will target all analytes recovered or duplicated within these ranges. Qualifier This column provides a letter and/or number designation that corresponds to additional information concerning the result reported. If a Qualifier is present, a definition per Qualifier is provided within the Glossary and Definitions page and potentially a discussion of possible implications of the Qualifier in the Case Narrative if applicable. Result The actual analytical final result (corrected for any sample specific characteristics) reported for your sample. If there was no measurable result returned for a specific analyte, the result in this column may state “ND” (Not Detected) or “BDL” (Below Detectable Levels). The information in the results column should always be accompanied by either an MDL (Method Detection Limit) or RDL (Reporting Detection Limit) that defines the lowest value that the laboratory could detect or report for this analyte. Uncertainty (Radiochemistry)Confidence level of 2 sigma. Case Narrative (Cn) A brief discussion about the included sample results, including a discussion of any non-conformances to protocol observed either at sample receipt by the laboratory from the field or during the analytical process. If present, there will be a section in the Case Narrative to discuss the meaning of any data qualifiers used in the report. Quality Control Summary (Qc) This section of the report includes the results of the laboratory quality control analyses required by procedure or analytical methods to assist in evaluating the validity of the results reported for your samples. These analyses are not being performed on your samples typically, but on laboratory generated material. Sample Chain of Custody (Sc) This is the document created in the field when your samples were initially collected. This is used to verify the time and date of collection, the person collecting the samples, and the analyses that the laboratory is requested to perform. This chain of custody also documents all persons (excluding commercial shippers) that have had control or possession of the samples from the time of collection until delivery to the laboratory for analysis. Sample Results (Sr) This section of your report will provide the results of all testing performed on your samples. These results are provided by sample ID and are separated by the analyses performed on each sample. The header line of each analysis section for each sample will provide the name and method number for the analysis reported. Sample Summary (Ss)This section of the Analytical Report defines the specific analyses performed for each sample ID, including the dates and times of preparation and/or analysis. Qualifier Description B The same analyte is found in the associated blank. J The identification of the analyte is acceptable; the reported value is an estimate. J4 The associated batch QC was outside the established quality control range for accuracy. Q Sample was prepared and/or analyzed past holding time as defined in the method.
