Groundwater Incident Number 14916
Site Risk Ranking: High Risk
Dear Ms. Blalock:
Pursuant to Department of Environment and Natural Resources (DENR) directives, CES
performed a groundwater monitoring event at the subject site on December 30, 2011. Groundwater
samples were collected from monitoring well MW-1 and water supply wells WSW-1 and WSW-2. Prior
to sampling, CES purged the well to remove suspended solid material from the water column and to
collect samples representative of aquifer conditions. The well was purged until a minimum of three well
volumes had been displaced. Exterior spigots were purged for approximately 10 minutes prior to
collection of supply well samples. Groundwater samples were collected using dedicated, disposable,
high-density polyethylene (HDPE) bailers and were transferred directly into the appropriate sample
containers immediately upon collection. Disposable, nitrile gloves were used during all phases of sample
collection. The groundwater samples were labeled and maintained on ice pending delivery to an
independent laboratory. The supply well and monitoring well samples were submitted to Prism
Laboratories, Inc. (Prism) in Charlotte, North Carolina for volatile organic compounds (VOCs) methyl
tert butyl ether (MTBE), diisopropyl ether (IPE), and 1,2-dibromoethane (EDB) via SM 504.1. Strict
sample security and chain-of-custody documentation were maintained during all phases of transport.
Chain-of-custody documentation and the laboratory Certificates of Analyses are included as Attachment
A. Groundwater samples collected during this investigation did not yield compounds in excess of
respective laboratory method detection limits. In accordance with NCDENR directives, a comprehensive
groundwater monitoring report (GMR) was not prepared during this phase of work.
