The scope of services included the collection of groundwater samples from monitor wells MW-3 and MW-5 (Figure 1), from water supply wells WSW-2 and WSW-4 (Figure 1), for analyses by Method 6200B and 504.1, and a field blank sample analyzed by Method 6200B; and measure depth to water in monitor wells MW-1, MW-2, and MW-4; and summarize findings in a Letter Report. The scope of services was completed as proposed.  Water Supply Well Sampling and Results Two water supply wells, (WSW-2 and WSW-4), were sampled. The approximate locations of the sampled water supply wells are shown in Figure 1, Attachment I. On June 11, 2020, S&ME personnel sampled active potable water supply well WSW-2 and WSW-4. The groundwater samples and one field blank sample were placed in laboratory-supplied containers, placed in a cooler on ice, and shipped under standard chain-of-custody procedure to ConTest Analytical for analysis of volatile organic compounds (VOCs) by EPA Methods 6200B and 504.1. The Water Supply Well Sampling Forms are provided in Attachment III. The laboratory analytical results of the groundwater samples collected from WSW-2 and WSW-4, and for the field blank sample, were each less than the laboratory method reporting limits. A summary of the current and historical water supply well analytical results are presented in Table 1, Attachment II. A copy of the laboratory analytical report and chain of custody form is provided in Attachment IV. On June 22, 2020, upon receipt of the laboratory analytical report for WSW-2 and WSW-4, S&ME informed Mr. Herbert Berger of the NCDEQ of the water supply well samples via email.
